The facility did not have a fenced outdoor play area that was on or adjoining the day care property and was accessible without hazard as evidenced by the fence was not in good condition in that there were four nails protruding from the wooden fence along East John Avenue.

The facility did not have a fenced outdoor play area that was on or adjoining the day care property and was accessible without hazard as evidenced by the fence had openings that were between 3.5 and 9 inches and allow entrapment - the openings measured approximately 7 inches wide by 7 inches tall in three separate locations along the bottom of the wooden fence against East John Avenue. The wooden fence had rough edges that were splintering. Additionally, the chainlink fence on the left side of the back yard had an opening that measured approximately 10 inches. The gate in the back yard also had an opening measuring approximately 3.5 inches wide.

The facility's outdoor space was not safe/well-maintained for children's activities as evidenced by a piece of plexi glass, wooden boards and multiple pieces of siding were located in the fenced in backyard.
