The requirements for protective outlet covers or twist-lock outlets were not met as evidenced by electrical outlets were not covered in that one electrical outlet was missing the cover in the Therapy 4 classroom.

Carpeting alone is not an acceptable resilient surface under indoor equipment.

There was no pad or mat under indoor equipment, a indoor play house is 36 inches high and is located in the back play room.
