Individual storage space requirements were not met as evidenced by there was no space for each child's belongings.

5 CSR 25-500.102 · 5 CSR 25-500.102 Personnel (1) (P) states: The child care provider shall conduct a Family Care Safety Registry screening prior to employment for any newly hired child care staff member who has a qualifying criminal background check result.

The facility did not conduct a Family Care Safety Registry screening for Dennis Pavlich, who was hired on 3/1/2023.

B. states: Once the center director or group child care home provider is certificated by the department, the Center Director/Group Child Care Home Provider Approval Request form shall be submitted to the department and maintained on file at the facility. 5 CSR 25-500.222 · 5 CSR 25-500.222 Records and Reports (2) . states: An individual file shall be kept to identify each child and enable the provider to communicate with the parent(s), guardian or legal custodian of the child in an emergency.

An individual file was not kept for 5 child(ren).
