The requirements for sleeping materials were not met as evidenced by three child(ren) 12 months or older was/were sleeping/napping on the carpet without cot or mats. The variance was approved in 2001 for using mats that have to be non-absorbent and a sheet over the mat. The mats should be long enough for child's head or feet not rest of the floor. Lisa has a cloth individually assigned mats , that can not be used, due to them being absorbent. One child was asleep on an non-absorbent mat , but without a sheet. The mat was placed on linoleum floor that is also not complying with a condition of the variance.

During the 12/18/2023 inspection CI observed carabines used to keep the gates closed. .

Children were not restricted to approved space as evidenced by a door to a storage room was open. There is a furnace room, paint , electrical cords. During the 12/18/2023 inspection CI witnessed that the lock was installed on the storage room and the door was locked.

During the 12/18/23 return inspection the baby was in a pack and play getting ready for nap. No soft blankets were in the pack and play.

Infants and toddlers did not have constant care and supervision as evidenced by a 7 month old infant was alone in pack and play. He was located behind closed door in the back of the house by the bathroom. During the 12/18/2023 return inspection CI observed the door to a room where the 7 month old was getting ready to sleep was open and propped with a door stop, to prevent older children closing the door.
