Three children under 1 year of age were present in the classroom. Director Souad Bathami attempted to secure the duct tape but it would not hold to the door.

Bottom of fence is approximately 1' off the ground. Directly on the other side of the fencing is a drop off point with varying distances between the bottom of the fence and the ground below.

When asked Malaika Firdous did not know the names or ages of all the children in her care. CI observed a redness to the child's left hand and pointed it out to Malaika. Malaika did not know how that happened and went to touch the child's hand and the child pulled it away from the caregiver.

A caregiver exhibited behavior that did not show the individual to be of good character and intent and qualified to provide care to children as evidenced by Malaika stated that it was her first day. During the encounter the director had to tell the staff member the name of the child she was holding and that the child was fearful of strangers. When the CI interviewed the director asking why the staff member did not know the names and ages of the children in her care, the director stated Malaika was new. CI observed personnel file for Malaika and the new hire orientation stated the start date as May 30th, 2023. CI asked the director if Malaika had been trained, the director responded and said they don't do that kind of training but they do the state training online. The director had provided a completed and signed new hire orientation for Malaika (signed by Director and Malaika). Later the director stated again they do not have that kind of training but assured CI that Malaika had taken safe sleep and health and safety training. Another staff member Howah Alhassan interjected saying that Malaika did receive in person facility training by her for approximately 9 days in May. Then the director agreed with Howah, that Malaika did receive training.

While holding the child in the air child Malaika was shaking the child. The CI observed the child's head moving back and forth and the child did not appear to be in control on their head. The child was observed to be upset with a flush face and making sounds like crying but with no visible tears.
