CI Duckworth observed the pointed corners and edges that had been sticking out to but cut to have rounded corners however where the metal had been cut to be rounded, there were jagged sharp pointed pieces/edges that were still not safe and is a cutting hazard. The trim is made of white metal.

The provider told her that Rachel Murphy did not live at the facility and was only visiting. On 7/21/25, the provider told CI Duckworth that Rachel Murphy is living in the home and has lived in the home since February of 2025.

A caregiver exhibited behavior that did not show the individual to be of good character and intent and qualified to provide care to children as evidenced by CI Duckworth observed an 18 year old and a 24 year old male in a bedroom that was not approved childcare space with the door locked from the outside with a hook and eye lock. When CI Duckworth asked the provider why the door was locked, the provider told her that the boys asked to have the door locked, they were in non-licensed space, and that no one told her that she had to take the lock off the boys' bedroom door. She thought she just had to take the lock off the first bedroom that is licensed childcare space. The provider unlocked the door to the bedroom where the two young men were and then, after CI Duckworth looked in the room, the provider locked the door again.

Arnetta Gauch told CI Duckworth that the adult, Rachel Murphy, who was in the locked master bedroom, lived at the facility and was caring for the 16 month old child that was in there with her. Arnetta explained that the child was related to Rachel, therefore did not need to be enrolled in her facility because Rachel is the one who was caring for the child, not her.
