Infants and toddlers did not have constant care and supervision as evidenced by two infants were in separated rooms with closed doors napping while a provider went upstairs to move the dishes after the lunch and accompany the CI during inspecting. During the return inspection on 11/8/2023 the door to one of the small bedrooms was still closed. The 1 year old child was in the room.The provider reported that she just placed the child in the room to fall asleep , and will open the door as soon as the child will fall asleep..

Only sleep clothing that is designed to keep an infant warm without the possible hazard of covering the head or face may be used during sleep or nap time.
