5 CSR 25-500.082 · 5 CSR 25-500.082 Physical Requirements of Group Day Care Homes and Day Care Centers (4) (A) states: A safe diapering table with a waterproof washable surface shall be used for changing diapers. The diapering table shall be located within or adjacent to the group space so the caregiver using the diapering table can maintain supervision of his/her group of children at all times.
Diapering requirements were not met as evidenced by the diapering table was not located where caregiver could maintain supervision of his/her group of children. The diapering table was located against the wall, away from the area where the teachers could maintain supervision of the children. During the 8/7/25 return inspection CI Gerasimova observed that a diaper changing table is standing against the window/wall . Teachers and director reported that they are actually keeping it that way when no children are changed. If the children in either of IT rooms downstairs are changed the DT is moved closer the the rooms where the teacher still can supervise other children in care. It can not go further than 8ft away from the handwashing sink .

Caregiver did not provide frequent, direct contact as evidenced by two teachers did not supervise all the children under their care. They didn't notice a 13 month old child crawling under the soft gate, up the steps and didn't notice that he is missing. They found out about a missing child only when their director Anais Harris found the child on the top of the stairs ,and brought him downstairs. During 8/72025 return inspection CI Gerasimova had no superfision concerns. Constant supervision and safety of the children should be the main and most important task of the teachers.

5 CSR 25-500.182 · 5 CSR 25-500.182 Child Care Program (1) (A) 1. states: Caregivers shall not leave any child without competent supervision.
A child was left without competent supervision as evidenced by two caregivers Alexis Christian and Kaitlyn Langforrd had three children in Infant toddler room D. By a report of Anais Harris , a director of the center, a 13 month old toddler crawled under the soft gate and climbed to the top of the stairs (about 10 steps up). When Anais who was supervising older children in her classroom upstairs found an unsupervised child on the top of the stairs, she took the child and went downstairs. She found one of two teachers changing another child's diaper. That teacher was turned with her face towards the window and wall, and her back to the stairs. That teacher didn't see the child crawling up the steps. A second teacher was found holding her own child , standing in a hallway with her side towards the stairs. Second teacher reported not seeing a child who was getting up the steps. During 8/7/25 return inspection CI Gerasimova had no supervision concerns. CI Gerasimova discussed with teachers an importance of constant supervision and knowing how many children each teacher is supervising at any given time.
