5 CSR 25-500.082 · 5 CSR 25-500.082 Physical Requirements of Group Day Care Homes and Day Care Centers (2) (A) 6. states: Walls, ceilings and floors shall be finished with material which can be cleaned easily and shall be free of splinters, cracks and chipping paint. Floor covering shall be in good condition. Lead-free paint shall be used for all painted surfaces.

The ceilings located in classroom B was/were not in good condition as evidenced by there are a few missing shingles on the roof after a windy weather in mid March. The roof started leaking due to a rainy weather this week. There is a brown 1-1.2 ft spot beside one of the light fixtures in the room. Director reported it to her supervisors . Maintenance came by to fix the shingles one day, but the shingles got detached again .Maintenance workers tried working on the roof the morning of 4/2/25 but left due to the rain.

The facility's outdoor space was not safe for children's activities as evidenced by a pool noddle is taped to the metal car with a ducked tape. The tape is worn out , easy to peel of and presents potential chocking hazards to the children.

The facility orientation shall include--

A facility orientation was not conducted for a caregiver(s) before being left alone with children. The following staff need a facility orientation: Catherine Przygoda and Kayla Semler did not have the forms on file. During the 4/1/2024 inspection both staff members went through the form with an area supervisor and a director and filled up/signed the forms.
