The facility's outdoor space was not safe for children's activities as evidenced by rusted paint cans, with some cans full of paint, a door with screws protruding out, a piece of a door trim with nails sticking through, a container of joint compound, a wooden child's desk with desk chair that was warped and chipped, a mop bucket filled with frozen water, a broken metal ball hoop filled with various materials which included: pest spray, foam compound, foam installation spray, roofing materials, and pieces of fence. A large tree branch that measured approximately 4.5 feet by 1.5 feet with sharp wooden edges, multiple tree limbs that pose a puncture hazard to a child, and multiple pieces of trash. The hand rail on the staircase to the play space was observed with chipping/flaking paint. An excessive amount of gum balls were present throughout the play area creating a tripping hazard .
