On 04/11/2023, I attempted to contact the anonymous complainant with the phone number listed on the complaint. The phone number was not working. The complaint included that Child A attended the child care but there was no specific allegation regarding Child A. I attempted to contact Child A's Mother, but the phone number was not working. On 04/11/2023, I completed an unannounced on-site inspection and interviewed Tasha Dingier. Tasha Dingier denied the allegation and said that she did not know why someone would say that she was unfit and irrational. She has had some conflict recently with a tenant and thinks that may be why a complaint was reported. During my conversation with Tasha Dingier, I did not observe any indication that she was irrational. I reviewed the file for the group home license. Tasha Dingier has held the current child care group home license since 2011. For the last license renewal that occurred in 2021, Tasha Dingier provided a copy of a medical clearance dated 04/20/2021 that indicated that she had no physical/mental condition or health problem that would limit her ability to provide independent care of children in a child care setting. I attempted to contact Child Care Staff Member but was unable to reach her. I attempted contact Child B's Mother, but her phone number was not in service. I attempted to contact Child C's Mother. I left a message asking for a call back, but I did not hear back from her.

When I arrived at the child care home, Tasha Dingier was outside supervising a child who was playing in the front yard. Tasha Dingier explained that she was waiting for the child to be picked up as she had tested positive for a communicable disease and was closing for the day. Tasha Dingier 3 denied the allegation and explained that she has never left a child unattended in her home. She provides care on the main floor and does not leave the floor unless Child Care Staff Member is present. It is not a licensing requirement that the doors of a child care home be locked, therefore I did not investigate that part of the allegation. I asked Tasha Dingier about Child A. Tasha Dingier denied that Child A was enrolled in her child care. She had never heard of Child A or Child A's Mother. I requested that Tasha Dingier send me her child information cards and attendance records. I received and reviewed the records and found no record of Child A. I attempted to contact Child Care Staff Member but was unable to reach her. I attempted contact Child B's Mother, but her phone number was not in service. I attempted to contact Child C's Mother. I left a message asking for a call back, but I did not hear back from her.
