On 417/2023, I completed a face-to-face interview with Child A's Mother and Father at Child A's home accompanied by Child A's Mother said Child A has attended the child care center for about one year. Child A's Mother said on 4/5/2023 she picked Child A up from the center. When they got to the car Child A's Mother asked Child A about her day. Child A said that Child Care Staff Member (CCSM) 1 her when Child A was playing around on her cot during nap time. Child A's Mother observed a on the right side of Child A's Child A's Mother said the i s Mother returned to the center where she spoke with program director and license designee Hanan Dari. Child A's Mother told Ms. Dari Child A said CCSM 1 her during nap time. Ms. Dari told Child A's Mother that there is no way CCSM Child A. When Child A's Mother returned to her car, Ms. Dari came out to Chid A's Mother's car and told Child A's Mother she believes Child A obtained the Child A's Mother said Child A does not when going to sleep at home. Child A's Mother was not familiar with CCSM 1, as CCSM I is not a regular CCSM in Child A's classroom. One of Child A's regular "teachers" recently left employment at the center so Child A's Mother thought maybe CCSM I was filling in for the previous CCSM. Child A's Mother said she the only thing she did not like about the center is that parents were not allowed to go directly to their child's classroom to drop off or pick up their child. Parents are required to wait at the door while the CCSMs take the children to their classrooms or bring children to their parents at the end of the day. Child A's Mother said she did not have any major concerns about the center until this incident. Child A's Mother said she will not be taking Child A back to the center. Child A is enrolled to start at a new center on Monday. attempted to forensically interview Child A but was not successful. There were no visible marks on Child A's during the interview at her home. On 417/2023, I attempted an unannounced on-site inspection at the child care center with _ Upon arrival we discovered a note on the door indicating the child care center was closed for the day. On 4/13/2023, 1 completed an unannounced on-site inspection at the child care center accompanied We interviewed program director and licensee designee Hanan Darib. yM"ss.. Daann said on 4/5/2023, Child A's Mother came to talk to her at the end of the day because after they left the center Child A told her mother that CCSM 1 Child A during nap time that day. Ms. Dari said she was very surprised at the allegation. On 4/5/2023, Ms. Dari was in the classroom with Child A the entire day. Ms. Dari said CCSM I works in the infant classroom as a regular 3 CCSM. CCSM 1 does not float around to various classrooms. CCSM 1 assists with taking children to their classrooms during drop off time. The only interaction CCSM 1 had with Child A on 4/5/2023 was when she took Child A from the main entrance of the center to Child A's classroom just a feet away from the front door. CCSM 1 did not come to Child A's classroom during nap time on 4/5/2023, or any other day. Aside from helping with getting children where they need to go during drop off and pick up times, CCSM 1 remains in the infant room. Ms. Dari said Child A often requires frequent redirection at nap time because Child A likes to do flips on her cot ,causin"gn ,e snee t to come off the cot. Ms. Dari believes Child A could have obtained from flipping around and exposing the mesh material on t underneath the sheet. Ms. Dari spoke to all the CCSMs and none of them reported seeing any concerning interactions between Child A and CCSM 1. All the CCSMs said they did not observe any — on Child A's - on 4/5/2023. and I interviewed CCSM 1. CCSM 1 said the only interaction she had with Child on 4/5/2023 was when Child A was dropped off at the center. On November 19, 2022, I made an unannounced onsite inspection to the child care center. I discussed the allegations in the complaint with the program director Hanan Dari. Ms. Dari stated that the allegations in the complaint were completely false and that this parent was upset because of payment issues. Ms. Dari went on to say Child A only attended the center for about a week and during the time she was there Child A's Mother didn't have any complaints until she had questions about payment. Ms. Dari went on to show me a text message exchange between herself and Child A's Mother. The text message detailed information regarding payments needed and allegations regarding the center. During 2 the onsite inspection I viewed the infant and preschool classrooms. There were no infants in the classroom and Ms. Dari explained that they do not currently have any infants enrolled. Child A was the only infant who was in care. I observed the children in the toddler/preschool area and did not see any indication of children walking around with dirty noses or being neglected. During the onsite inspection I asked Ms. Dari if she had the log showing when Child A was fed and changed. The information was on the online application, and it appears from the information that Child A was fed and changed as needed during her time at the center. Ms. Dari indicated that since Child A was the only infant, she was the child care staff member who provided care for Child A while she was at the center. On December 24, 2021, I received an email from Ms. Dari with a screenshot of a message received from Child A's Mother. Ms. Dari stated that she wanted it documented that she was being harassed by Child A's Mother. On May 20, 2022, I called Child A's Mother to discuss the complaint. Child A's Mother stated she was concerned at the time that when Child A came home from the center, she would sleep from 4:00p.m. through the night. At the time Child A was only four months old and she did not sleep that long. She was also concerned that Child A was hungry, and her nose was dirty when she picked her up. She went on to say that when she asked Ms. Dari about Child A's dirty nose, she was defensive. On May 20, 2022, I called Ms. Dari to ask if the center had enrolled any infants since I was out in November. Ms. Dari stated that they did not have any infants and would not have any until September 2022. On May 20, 2022, I called Child B's Mother. Child B (male, age 2 years old) attends the center. Child B's Mother said she has had children in this center for approximately five years and had has never had any problems with the care provided to her children. Child B has never come extremely hungry, her husband routinely gives him fruit when he gets home. She believes the center gives the children enough food and believes that her older children would have told her if she didn't get enough to eat. Child B's Mother went on to say that her children have never come home with dirty noses and when they were little they always left with clean diapers. They may get dirty playing outside sometimes, but she expects that because they are children. 3
