I 722.115n Licensee Responsibilities. (1)Except as otherwise provided in subsection(13), when a person, partnership, firm, corporation, association, governmental organization, or nongovernmental organization applies to renew a license to operate a child care center, group child care home, or family child care home under section 5m and before a group child care home or family child care home allows an individual to be a member of the household, or a child care center, group child care home, or family child care home allows an individual to become a child care staff member, the department shall do all of the following:

On November 17, 2021 I made an unannounced onsite inspection to the family child care home of Amal Mustafa. Upon arrival I spoke to Ms. Amal Mustafa and made her aware of the allegations in the complaint. Ms. Mustafa stated that she had not been providing child care consistently since the death of her son. Adult Household Member 1 (AHM1) and Adult Household Member 2 (AHM2) moved in over the past weekend, as they were concerned about her living alone. Ms. Mustafa went on to say it was her intent to have background checks completed on both of them. During the onsite inspection on November 17, 2020, I completed an exit conference with Ms. Mustafa and explained the violations. Ms. Mustafa acknowledged that she knew she was supposed to inform the department when someone moved into the home, she also knew that fingerprinting needed to occur before anyone moved into the home. Ms. Mustafa went on to say she just didn't think about doing these things, before they moved in. Ms. Mustafa understood she was in violation of the licensing rules and would make an appointment for AHMland AHM2 to be fingerprinted and would send over the child care information forms. On November 23, 2020 I emailed a copy of the child care licensing request for information form and the notification of change in household composition forms to Ms. Mustafa. On December 8, 2020 I received a copy of the child care licensing request for information form and change in household composition form via email. The forms indicated that AHM1 and AHM2 had moved into the childcare home. On December 10, 2020 I emailed Ms. Mustafa to see if the fingerprinting appointments were made for AHM1 and AHM2. Ms. Mustafa replied that their appointments were scheduled for December 14, 2020. 3
