Licensee designee Dorothy Morris stated she has not spoken with any parent regarding a complaint about a child with unexplained bumps or scratches. Program director Carolyn Layher stated that a written incident report is completed for every injury to explain how it occurred and how it was treated. If there is an injury on a child from the neck up the parent is called immediately to make a verbal report. If the injury is minor a message is sent on the Brightwheel application used the center to communicate with parents. Ms. Layher stated that she believes that this complaint regarding Child A (male, age 3 years). Ms. Layher stated that on March 17, 2023, Child A who is normally a very active child was not behaving normally. Child Care Staff Member 1 sent a message to Child A's Father on Brightwheel around 9:30 AM asking how Child A's night was because he did not seem himself. Child A's Father responded that Child A had a good night. Child A's Father then asked about a scratch that he saw the day before behind Child A's right ear. Child Care Staff 2 Member 1 stated that she was not aware of a scratch but that she would look at it. Later that morning Child A had loose stool, was shivering, had no appetite and was not behaving normally. At 11:00 AM Child Care Staff Member 1 contacted Child A's Father to report that Child A was not feeling well and that he needed to be picked up. Child Care Staff Member 1 took Child A to office to lay down on his cot to wait to be picked up. A few minutes after Child A came to the office Child A's Father called the center and asked Ms. Layher to verify that Child A was not feeling well, asking "Are you sure he's sick?" Ms. Layher called Child Care Staff Member 1 to the office who spoke with Child A's Father again informing him of Child A's symptoms. Just before 12:00 PM Child A was picked up from the center by Child A's Grandmother. Ms. Layher stated that Child A's Father was upset when he was told that Child A had to be picked up because he was sick, however Child A returned to care after he was feeling better on March 20, 2023. Child Care Staff Member 1 stated that on March 16, 2023, Child A was playing on the playground when he tripped on a slide and fell. When Child A got up, he was pointing to his head. Child Care Staff Member 1 looked at Child A's head and did not see any injuries. Child Care Staff Member 1 did not look behind Child A's ears. Child Care Staff Member 1 stated that Child A did not cry and returned to playing. The following day on March 17, 2023, Child A's Father asked about a scratch behind Child A's ear on a message on Brightwheel. Child Care Staff Member 1 looked behind Child A's right ear and saw a scratch that was scabbed over. Child Care Staff Member 1 stated that the scratch looked too old to have occurred the day before. Child Care Staff Member 1 stated that Child A did not point to or complain about pain behind his ear when he fell. After seeing the scratch Child Care Staff Member 1 contacted Child A's Father and told him that Child A fell on the playground the day before but that she did not see any marks on his head when she checked him over which is where he was pointing after he got up. Child A's Father told Child Care Staff Member 1 that he asked Child A how he got the scratch and he said that he tripped. I attempted to contact Child A's Father but did not receive a return call after leaving a voicemail message.

Licensee designee Dorothy Morris stated that when children are sick, they lay on a cot in the director's office until they are picked up so that they can be separated from the other children. Program director Carolyn Layher denied that Child A was made to lay on another child's cot. She stated that on March 17, 2023, when Child A was not feeling well, he was brought to her office with his own assigned cot and blanket to lay on until he was picked up. Ms. Layher stated that children are assigned their own cots and they are cleaned and sanitized weekly. Child Care Staff Member 1 stated that on March 17, 2023, when Child A was not feeling well, she brought Child A's cot to the program director's office where he laid down until he was picked up. Child Care Staff Member 1 stated that children do not share cots and each child is cleaned and sanitized weekly. Child A's cot was cleaned and sanitized when he left for the day on March 17, 2023, since he was not feeling well. I provided technical assistance to licensee designee Dorothy Morris about the use of the program director's office for Child A which is not child use space. I informed her that she needs to designate a space approved for child use to be used to separate a sick child from the other children.

On February 17, 2022, I made a home visit to the home of Child A and Child s Mother and Father. During the home visit made Child A's Mother and Child A's Father aware of the allegations in the complaint. Child A's Mother stated that Child A attended Dorothy's Discovery Daycare Center on Textile Rd. On February 15, 2022, she received a phone call at 1:39p.m. from child care staff member Lauren Thomas stating that [Child A] spilled soup on her leg. The incident occurred at 12:00p.m. Child A's Father arrived at the center at 2:00p.m. to pick Child A up. Child A's Father stated that he got to the center and they told him he couldn't come in, due to COVID-19 restrictions, however he went in anyway. He said he immediately went to [Child A and took off her pants and looked at her legs, he left from the center and Both Child A's Mother and Child A's Father were upset that they didn't immediately receive a phone call when the incident occurred. Child A's Mother stated that Ms. Thomas informed her that after Child A spilled the soup, they took her to the bathroom and lied a cold water and cream on her leas. Child A's Father stated that when he Child A was also present during the interviews. Child A stated that "they" gave her soup and she spilled it. She was wearing a nightgown at the time of the home visit and lifted the gown to show red marks on both her left and right legs near the thigh 3 area of her leg. The marks on her left leg were bigger and there was blistering present. On February 17, 2022 I made an unannounced onsite inspection to the child care center. We spoke to Karlee Morris, program director and made her aware of the allegations in the complaint. Ms. Morris indicated she was present with the incident occurred with Child A. Ms. Morris stated she prepared the soup for lunch by heating it on the stove, stirring it, and then placing small amounts in the bowls. She placed them on the cart and took them down to the classroom. Once she went to the classroom Ms. Lauren Thomas began serving the lunches to the children sitting at the tables. Ms. Thomas served the soup to Child A and the other children. As she was serving the other children, they saw Child A's soup was not in front of her anymore, they immediately took her pants off and took her to the bathroom to apply cold water on her legs. They applied the cold water by putting a towel in cold water and applying it to her legs. I asked Ms. Morris how she prepared the soup and how she tested the temperature prior to giving it to the children. Ms. Morris stated that soup comes already prepared and she just had to heat the soup on the stove. She did not test the temperature prior to giving it to the children, however she prepared it in the kitchen and then took it to the classroom on the cart. She was upset that the soup burned Child A and didn't realize it was so hot, she couldn't understand how it caused such a severe burn, but other children were eating the soup. During the onsite inspection on February 17, 2022, I interviewed child care staff member Lauren Thomas. Ms. Thomas was made aware of the allegations in the complaint. Ms. Thomas stated she was present at the time of the incident. The classroom was preparing to eat lunch, which was soup. She was serving the soup to the children in the classroom. She finished serving the right table and began serving the table on the left. I asked which table Child A was sitting at and she replied the right table. Ms. Thomas stated she heard Child A scream and turned to see what was wrong, she saw that her soup was not longer in front of her. She immediately picked Child A up and started taking her pants off of her as she carried her to the bathroom. When they got into the bathroom, she applied a cold towel to Child A's thigh and noticed the redness. Ms. Karlee Morris brought ice packets and cooling gel. Child A was crying and asked to go to sleep. After Child A calmed down, they allowed her to go lay down, as it was rest time. Ms. Thom

On March 15, 2022 child care staff member Karlee Morris did not test the temperature of the soup prior to serving it to the child care children. Child A spilled her soup in her lap causing second degree burns to her thigh area.
