Except as otherwise provided in this subsection and subsection (4), records regarding children and facts compiled about children and their parents and relatives are confidential and disclosure of this information must be properly safeguarded by the child care organization, the department, and any other entity in possession of the information. The center kept facts and records about Child A confidential. Child A's date of birth was posted in the hallway with Child A’s Mother permission. Child A’s birth year was not posted.

Staff; volunteer (1) All staff and volunteers present at the center shall: (a) Provide appropriate care and supervision of children at all times.
On 5/11/2026, Program Administrator, Teacher 1, and Teacher 2 provided appropriate care and supervision to the children at all times. During the field trip to Domino Farms, a child did not get lost.

R 400.8112 · 1 R 400.8112 Comprehensive background check; fingerprinting. (4) For an individual who is determined ineligible by the department, a licensee shall immediately do all of the following: (a) Prohibit the individual from being on the premises of the child care center.
On December 10, 2020 I received information stating that this center has an employee working that has been declared ineligible. On December 10, 2020 I contacted the licensee, Mrs. Deata Mclemore, regarding the allegations in the complaint. Ms. Mcleamore stated that she did receive the ineligible notice; however, believed that Child Care Staff Member A had turned in the paperwork to become eligible. Ms. Mclemore went on to say that she thought Child Care Staff Member A could work in the center supervised pending becoming eligible. I referred Ms. Mclemore to the rule that states that once a person is declared ineligible they are prohibited from being at the child care center. I asked Ms. Mclemore if Child Care Staff Member A was working that day and she stated that she was. Ms. Mclemore said she would let her know that she couldn't work until she submitted the necessary documents and was made eligible to work at the center. On December 10, 2020 I received a phone call from Ms. McLemore with additional questions regarding the complaint. Ms. McLemore stated that she sent Child Care Staff Member A home and she was no longer at the center. Ms. McLemore wanted to know if Child Care Staff Member A could work as a supervised volunteer in the school age room pending the clearance. I explained that she couldn't be on the premises of the center at all until she was eligible. During the phone call on December 10, 2020, I conducted an exit conference with Ms. McLemore.
