On March 17, 2022, program director Erin Bradley called to report that Child A (female, age 5 years) was briefly left unattended in the hallway when child care staff member Safia Barre was walking the children outside to the bus for dismissal. Ms. Bradley followed up with a written incident report on March 18, 2022. During the onsite inspection on March 25, 2022, Ms. Bradley stated that on March 15, 2022, child care staff member Safia Barre was the only child care staff member with seven preschool children because the lead caregiver was out for the day. The children were outside on the playground in the back of the building when they returned to their classroom to get their backpacks for dismissal at 2:30 PM. Once inside Ms. Barre realized that the children's backpacks were outside on the playground. She took the children back outside to get their backpacks and then back inside, down the hall and then outside in the front of the building where the children would be dismissed onto the bus. At some point when Ms. Barre was walking with the children, Child A (female, age 5 years) left the group unnoticed by Ms. Barre. While Ms. Barre was outside with the children, child care staff member Kasie 2 Gritzmaker was in the building and saw Child A in the hallway. Ms. Gritzmaker brought Child A to Ms. Bradley and reported that she found her in the hall alone near her classroom. Ms. Bradley took Child A outside to Ms. Barre and let her know that she was found alone in the hall. Later that same day Ms. Bradley contacted Child A's Mother to report the incident. Ms. Bradley stated that Child A's Mother was very understanding about the situation. Ms. Bradley stated that Ms. Barre has been employed by the center since September 2021. She has been trained to record children during transitions using a name to face sheet. She did not follow this procedure during the transitions on March 15, 2022. Ms. Bradley stated that there is a language barrier which may have been prevented Ms. Barre from understanding the training that she has received. Ms. Bradley has arranged for training for Ms. Barre in her native language. The first training will begin on April 1, 2022. Ms. Barre will no longer be allowed to work with children alone, she will only assist as needed. Child care staff member Safia Barre stated that on March 15, 2022, she was working in the preschool classroom alone because the lead caregiver was out for the day. She stated that she took the children outside to play. She then brought the children inside to get their backpacks but realized that their backpacks were outside already. She stated that a volunteer from another class took the backpacks outside without her knowledge. She and the children returned outside to get their backpacks. While the children were going back outside Child A walked away. Ms. Barre did not see Child A walk away and only realized that she left the group when Ms. Bradley brought Child A to her. She has been trained on active supervision and counting children. Ms. Barre stated that she will do better about keeping track of children when they leave the room. Ms. Barre did not remember how many children were in the class on March 15, 2022, during the onsite inspection. Child care staff member Kasie Gritzmaker stated that at dismissal on March 15, 2022, she came into the building and saw Child A standing in the hall. She asked Child A where her "teacher" was, and Child A said outside. Ms. Gritzmaker took Child A to her classroom to see if Ms. Barre was there but no one was there. Ms. Gritzmaker then took Child A outside and told Ms. Bradley that Child A was alone in the hall. Ms. Bradley then took Child A to Ms. Barre. Ms. Gritzmaker stated that Child A was not upset or crying when she found her in the hallway. I attempted to contact Child A's Mother, but I did not receive a return call. During the onsite inspection I observed the hall where Child A was located when she was found by Ms. Gritzmaker. Child A was standing
