Child A was left without appropriate care and supervision when Teacher 1 exited the building to the playground with the GSRP 2 classroom and left Child A in the foyer.

On 11/17/2025, Teacher 1 and Teacher 2 failed to provide appropriate care and supervision of Child A when he was left unattended on the playground for approximately 30 to 50 seconds.

On 05/18/2023, I spoke with Child B's Mother. Child B's Mother said that Former Child Care Staff Member 1 (FCCSM 1) and Former Child Care Staff Member 2 were both let go from the center and reached out to her on Facebook to tell her about her son's mistreatment. She reported that she spoke with FCCSM 1 and was told that Child B was not being treated appropriately at the center. Child B's Mother did not have any details of the allegations. She sent me screenshots of the text conversation she had with FCCSM 1. Child B's Mother said that Child B was on the playground with no socks or shoes on; one time he had no pants on. She said that he often did not have shoes on. She sent me a photo of him in the classroom that the center had sent her of him playing with no shoes on. She also saw CCSM 6 pull Child B by his arm once when she was walking in the classroom, and he was pulling his arm away from CCSM 6. On 05/23/2023, I spoke with FCCSM 1 who reported that she saw CCSM 3 get on Child B's level, get close, and yell in his face. She saw CCSM 6 grab Child B by the arm when he "dead weighted." FCCSM 1 said that if Child B did not want to go to sleep, CCSM 3 would restrain him. If Child B ran away, CCSM 3 would grab him and put her arm over his butt, wrap him up like a burrito and pat him while holding him down. If Child B would cry, CCSM 3 would ignore him and would tend to other children. Child B was not attended to when he got hurt. FCCSM 1 said that she shared her concerns with the new program director Tammy Parks, but she did not remember what Tammy Parks' response was other than to ask for details. FCCSM 1 said that Tammy Parks said that they needed more stuff to do in their classroom because the children were bored and then tend to get into things. FCCSM 1 said that she would tell CCSM 3 and CCSM 6 that they were being too aggressive. FCCSM 1 said that CCSM 2 also had concerns about how Child B was treated. FCCSM 1 said that she was terminated and told it was because it "wasn't working out." She reported that she filed an Equal Employment Opportunity Commission complaint. On 05/25/2023, I completed an unannounced on-site inspection and spoke with program director Tammy Parks. Tammy Parks explained that she took over as program director last week. She said that she had multiple conversations with FCCSM 1 and FCCSM 1 never shared any concerns with her about other child care staff members. Tammy Parks said that she had informal conversations with FCCSM 3 1 in the classroom. FCCSM 1 told her that there were concerns with Child B's development, but she never said anything about how he was being treated. She has not witnessed any mistreatment of Child B. Child B's Mother withdrew him from the center without any explanation on 05/18/2023. I spoke with interim director Brandy Wasick who reported that FCCSM 1 and FCCSM 2 were both terminated. Both FCCSM 1 and FCCSM 2 worked in the classroom that Child B was enrolled in. Brandy Wasick said that Child B had issues with taking his shoes off all the time. He would like to climb on the counter and would not get down unless they moved him. She recalled that Tammy Parks went in the classroom once to help him put his shoes on and he threw them past her head. They had a hard time getting him to keep his shoes and clothing on. She was not informed that he had taken his pants off. She recalled Child B being on the playground without his shoes on, his mother coming to pick him up and she also told him to put his shoes on, he still refused, and she just left with him without his shoes being on. I spoke with CCSM 3 who reported that she never yelled in Child B's face and said that he was one of her favorites. She said that she used a stern voice with him and got down on his level. CCSM 3 said that she would yell if Child B was doing something unsafe and she was not able to reach him and wanted to get his attention in order to stop him. Child B's Mother said that Child B had rashes and it was because he was left sitting in dirty diapers because Child B did not want to get his diaper changed. Child B's Mother sent me a screenshot of a message she had sent to the center on 02/07/2023. Child B's Mother said that Child B "has a little rash in his butt crack and this isn't the first time I've noticed it. So if you guys could put some petroleum jelly on it when you change his pull-up that would be great. There's some on his backpack. Also if he gets another rash or has marks on his butt we'll have to setup a meeting to discuss this further." Former Child Care Staff Member 2 responded, "So sorry about that, we will keep an eye out with further changes. We did just change and check his bottom and we didn't see anything. But we will continue putting petroleum jelly on it until further notice." On 05/25/2023, I spoke with interim director Brandy Wasick who reported that the only issue she knew about regarding diapering was regarding FCCSM 1. She shared that there was a day when FCCSM 1 did not want to change Child B's dirty diaper because it was one minute after her out time. Brandy Wasick explained that FCCSM 1 was still clocked in but did not feel that it was her responsibility. FCCSM 1, CCSM 3, and CCSM 2 did not report any information pertaining to this allegation. CCSM 6 said that this allegation was false. She said that Child B consistently had a bowel movement in the afternoon when she worked. She said that he had loose bowel movements so they could not leave him in soiled diapers. She said that he did kick and scream because he did not want his diaper changed but he was not mistreated. She said that Child B was given toys or fidgets to keep him distracted during the changes. 5

Child B's Mother said that she was told that CCSM 3 got in Child B's face and that CCSM 6 grabs him by the arm. Child B's Mother said that she saw CCSM 3 pulling Child B by his arm once when she walked in the classroom. Child B's Mother said that Child B had bruises once and she thought maybe they were from the playground. I asked her what she thinks now; she said that she thinks maybe it was from a forceful diaper change. Child B's Mother said that Child B was called "bad" and that he should not have been doing that. She was told that Child B sneezed and hit his head, but the bruise was on his cheekbone. Child B's Mother said that she was leaving the center on Monday with Child B and he said "no, mean" when they passed CCSM 3. CCSM 3 said "no, I'm not mean." Child B's Mother said that Child B has a speech delay and is not able to say if something happened. She did not have a photo of the injury and she did not know the date. Child B's Mother said that the center documented every time Child B had an injury and gave her copies of the reports. Child B's Mother said that she thinks the center did not know what happened to Child B and just wrote something down on the reports. She said that she said something to the center and things got better afterwards. FCCSM 1 did not report any allegation information related to Child B. FCCSM 1 said that a child in the Wobbler classroom hit his head on a table and she saw CCSM 5 cleaning it up. FCCSM 1 said that the child had blood on his face and clothes. She said that she overheard Tammy Parks say to someone in the office, "hey, I didn't tell mom he was gushing blood." Tammy Parks said that the incident with the child in the wobbler classroom was from the child tripping over his own feet and hitting his head on a toddler sized table. She said that he was bleeding "tremendously", so they called 911 and he ended up getting two stitches. Tammy Parks said that when she spoke with the child's mother, she told her that he had a head injury, it was deep and that they called emergency services. She said that she did not tell his mother that her child was gushing blood because she did not want to alarm her more as she had to drive to go to her son. CCSM 3 and CCSM 2 did not report any information pertaining to this allegation. CCSM 6 said that Child B climbed up a lot, but she did not know of any time he was injured at the center when it was not reported to his mother. She did not think he had any unexplained bruises.

On 11/18/2022, I spoke with licensee designee Heather Walczewski about the allegation. Ms. Walczewski explained that the center had a new child enrolled that had just moved to Michigan. The parent reported the child having a food allergy but did not have a doctor note. The center did not feed the child dairy on his food, but they did give him milk. Ms. Walczewski said that the center has a form that must be filled out when a child has a food allergy. She 2 thought the form was from either licensing or the Child and Adult Care Food Program (CACFP). She has not spoken to the parent about the situation and was informed about it by the director and assistant director. On 11/23/2022, I completed an unannounced on-site inspection and interviewed assistant director CCSM 1 about the allegation. CCSM 1 reported that on 11/16/2022, She spoke with Child A's Mother at pick up time about the CACFP requirement for a doctor note regarding not serving dairy. CCSM 1 said that Child A's Mother asked if she could substitute food for Child A. CCSM 1 informed her that they had served Child A yogurt. Child A's Mother later sent the center an email and was upset about Child A being served dairy. CCSM 1 explained that Child A is not allergic to dairy but has a dairy intolerance. They will serve Child A dairy free meals now. Child A started at the center on 11/14/2022. I reviewed Child A's file and it was noted on enrollment paperwork "dairy intolerant: hives, eczema." CCSM 1 said that Child A was only served one spoonful of yogurt and was served dairy free milk. CCSM 1 did not discuss doctor note requirements with Child A's Mother. I interviewed CCSM 2 who is the lead caregiver in the toddler classroom. CCSM 2 reported that when Child A was dropped off on his first day, Child A's Mother provided oat milk and said that Child A had an intolerance, not an allergy. She told Child A's Mother that they needed a doctor note. Child A's Mother sent a message that she had provided a doctor's note already from the previous doctor, but they were not accepting the note and did not say why. The center had pizza for lunch that day, so she removed the cheese from the pizza served to Child A. The following day they had yogurt, so she asked CCSM 1 for a substitute because he had a muffin instead. CCSM 1 told her to still serve him yogurt because of the CACFP requirements. They are no longer serving any dairy to Child A. Last Friday, Child A was not present, but the kitchen sent a dairy free sandwich. I spoke with CCSM 1 about the messages. She said that she did not have a conversation with Child A's Mother about not accepting the documentation from the out of state doctor. I reviewed the rule requirements with CCSM 1 and showed her the technical assistance manual on the department's website. I reviewed the messages exchanged between Child A's Mother and CCSM 2. CCSM 2 notified Child A's Mother that Child A was served a "small bite" of yogurt because they had not received a doctor note yet. Child A's Mother responded that she was working on getting one, had provided one from his previous child care but "they won't accept that". On 12/07/2022, I spoke with Child A's Mother via telephone about the allegation. Child A's Mother reported that she informed the center of Child A's intolerance to dairy. She said that Ms. McCutchen told her that the doctor's note from their previous state was not adequate. Child A's Mother said that she communicated with Ms. McCutchen about this via email. Child A's Mother said that CCSM 1 told her that they had to serve dairy to Child A at every meal and snack because she had no 3 doctor note. On Child A's daily record, it showed that on the day they gave him yogurt, he had diarrhea. She said that she emailed them about it, and they then said that they would no longer serve dairy to him. As far as she knows, the center has not given any more dairy to Child A. Child A's Mother said that the center also told her that they would need an immunizat
