    On 09/12/2022 1 conducted an unannounced on-site inspection to the child care home sata on u sii iruz, c;ntia A naa oeen evaivatea at me nospitai r concerns about a mark or forehead and concerns Upon evaluation at the emergency room there were no concerns. At Re child care home Deng Reng, licensee, was present with, children in care. Rachel Moses was not home, Deng Reng called her when we arrived, and she came back to the home a short time later. explained the allegations to Mr. Reng and he said on 09/0812022 Child was dropped off for care bm mother. At that time Child A's Mother said Child A had a bruise forehead. Mr. Reng took a picture of Child A and tole mother to take the choildn"inor medical treatment if she had concerns. He did not see any mark or bruise on Child A. Approximately an hour after she dropped 2 off Child A she returned and picke up and she has not returned to care. Mr. Reng showed the photos to us and ere is no discernable mark or bruise on Child A's face. Ms. Moses said Child A did not have any marks or injuries from any incident while in care. Child A is a who is The licensee's were asked about diaper changes with Child A and any noticeable Ms. Moses said Child A was in care the week of 09/05- 09/0812022. On Wednesday she recalled changing approximately four times and there were no issues . On Thurs ay she did not change as was in care for an hour. They provided the daily attendance sheets to ven attendance on these dates. On 09118/2022 l attempted to make telephone contact with Child A's Mother and the phone number was disconnected. provided the information from Child A's hospital t: [Child was seen b on 09111/22 for xam was completed and there are re is a healing bruise on the bridge o e nose. ere is no a reason an e to determine if this is abuse or neglect due to size and healing. On 10/11/2022 l completed a case conference witi and There were no findings from her investigation.

    R 400.1903 · R 400.1903 Licensee Responsibilities. (3) The licensee shall permit parents of enrolled children who are in attendance to visit the child at the child care home at any time during hours of operation, as required in section 3a of the act, MCL 722.113a.

    On 2/24/2022 I spoke to Child A's Mother. Child A's Mother informed me that Child A only attended the child care facility for one day, which was 2/23/2022. Child A's Mother reported that on the 2/23/2022 Ms. Moses contacted Child A's Mother prior to their agreed upon pick-up time that was noted in her contract and advised that Child A needed to be picked-up because Ms. Moses needed to get to her child's basketball game. Child A's Mother stated that after arriving at the child care home to pick-up Child A, Ms. Moses would not allow Child A's Mother to enter the home. Child A's Mother informed me that after Ms. Moses told her she could not come into the home, she stated she would bring Child A outside the home. Child A's Mother stated that Ms. Moses then closed and locked the door, and then brought Child A outside. Child A's Mother requested to enter the home as she needed to put Child A in her car seat prior to bringing Child A outside. Child A's Mother stated that Ms. Moses still refused to allow her in the home and told her that she would put Child A in the car seat. Child A's Mother then informed Ms. Moses that she would wear a mask inside the home, but Child A's Mother stated Ms. Moses still would not allow her in her home and stated, "that's just how I do things." Child A's Mother informed me that the only time she had entered the home was during her initial tour and walkthrough. Child A's Mother denied observing any concerns or hazards during the tour of the home, but Child A's Mother noted that it made her uncomfortable that Ms. Moses was adamant that she could not enter the home. On 2/25/2022 I made an onsite inspection at the home of Ms. Rachel Moses and Mr. Deng Reng, co-licensees for the Sunshine Family Childcare. Ms. Moses reported that approximately two weeks ago she was contacted by Child A's Mother who was seeking child care for Child A. Ms. Moses stated that on Child A's first day at the child care facility, Child A's Mother wanted Child A to attend the child care from 7:00 AM — 5:00 PM. Ms. Moses noted that while she does advertise providing child care for 24-hours throughout the day, Ms. Moses stated she informed Child A's Mother that she could only provide child care after 3:15 PM on weekdays if Ms. Moses co-licensee, Mr. Deng, is present. Ms. Moses stated that on this day, her co-licensee/husband was not present at the home, so Child A was not permitted to remain at the child care facility until 5:00 PM. I asked to see the contract signed by Child A's Mother, but Ms. Moses noted that the contract was supposed to be returned on the morning of Child A's first day of care; however, Child A's Mother stated that she forgot to bring the paperwork and that it would be dropped off at the time of pickup. Ms. Moses stated that Child A's Mother was upset because she had to pick-up Child A before 5:00 PM noting when Child A's Mother arrived, she failed 3 to give her the completed Child Information Card or signed contract for the child care services. Ms. Moses informed me that Child A was a no-call-no-show for her first two days of child care which were supposed to be on the February 21 and February 22. Ms. Moses stated that when Child A's Mother arrived on the February 23 at 6:20 AM to drop-off Child A, she then received a text from Child A's Mother at 6:36 AM who was upset that she was not permitted to come inside the home to bring Child A inside on her first day of care. Ms. Moses then noted that she was unsure why Child A's Mother needed to see inside the child care facility at the time of drop-off, as Child A's Mother had previously performed a walk-through of the home during the initial meeting. Ms. Moses then showed me the text message correspondence that occurred between the two. Upon review of the messages, Child A's Mother expressed frustration about not being permitted to bring the child inside the home and stated that this would make her more comfortable. Ms. Moses then responded to Child A's Mother that this
