    R 400.8125 · 1 R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times.

    On 10/08/2022, program director Jennifer Dissmore sent me an email to inform me that Child A was unresponsive after rest time at approximately 2pm on 10/07/2022. Ms. Dissmore stated in her email that she was out of town and not present at the time of the incident. Staff reported to her that Child A was breathing but was stiff and unresponsive to multiple attempts to arouse her out of her sleep. Staff reported to her that Child A was really tired before rest time, so they did not think much of her not getting up right away once they turned the lights on. On 10/10/2022, I received an incident report from Ms. Dissmore via email. The incident report indicates the following. An unnamed child care staff member attempted to wake Child A up until approximately 2:10pm - 2:15pm. A second child care staff member was asked to step into the classroom to attempt to wake Child A up. Child A's Mother and the school principal were contacted at 2:34pm. The school principal entered the classroom and attempted to wake Child A up, but Child A was 2 stiff and unresponsive. Child A's Mother arrived at 2:45pm and 911 was called at 2:49. Emergency Medical Services (EMS) arrived at 2:58pm and loaded Child A on a stretcher. Child A woke up as EMS personnel lifted her onto the stretcher. Child A and Child A's Mother were transported to the Helen DeVos Children's Hospital by ambulance. Child A's Mother reported that Child A was medically cleared and discharged around 5:30pm. Child A's Mother also reported that discharge instructions indicated to follow-up with a neurologist. On 10/10/2022, Child A's Mother notified the center that the hospital thinks that Child A went into sleep paralysis due to Child A having sleep apnea. Child A's Mother requested that Child A not take part in rest time until she is seen by her doctor. On 10/11/2022, I completed an on-site inspection. I interviewed program director Jennifer Dissmore, Child Care Staff Member 1 (CCSM 1), Child Care Staff Member 2 (CCSM 2), and Volunteer 1. Ms. Dissmore stated that Child A's Mother reported to staff on 10/7/2022 that Child A was diagnosed with sleep apnea one year ago and that the same thing could have happened to Child A at home as well. Ms. Dissmore stated that she thinks that staff waited too long to call 911. She thinks that 911 should have been called almost immediately. She reported that the classroom is well-lit at rest time. I observed the classroom during rest time. The classroom was well-lit with the lights out and I could clearly see each child on their cot. CCSM 1 stated that she turned the classroom lights on at 2pm on 10/07/2022. Child A was usually the first child up after rest time but not on this day. However, CCSM 1 did not think much of it because Child A told her that she was really tired while on the playground. CCSM 1 kept checking on Child A as she helped the other children put their stuff away. Child A was laying on her stomach, so she placed her hand on her back to check her breathing. She gently shook Child A's shoulders and called her name five to six times. She described Child A as being limp. Child A's arm just dropped back to the cot when she picked it up and let it go. CCSM 1's hand was rising and falling so she knew Child A was breathing. She started to get nervous around 2:10pm so she called out to CCSM 2 for assistance. CCSM 2 came into the classroom and assisted her with trying to arouse Child A. They decided to turn Child A over onto her back. They attempted to sit Child A up, but her head went backwards. CCSM 1 tried to support her head and back but that did not work either. Child A's eyes were moving rapidly back and forth under her eyelids. Next, CCSM 2 called CCSM 3 into the classroom. CCSM 3 was in charge during Ms. Dissmore's absence. CCSM 1 stated that she then took the other children into a different classroom. CCSM 1 was not aware that Child A had sleep apnea prior to the incident. Volunteer 1 was present with the children while CCSM 1 was on he
