    On 11/24/2021 I spoke with program director, Alisa Vance. She stated Child A's Mother informed her of his disclosure that he was touched by another child when in the restroom. Child A's Mother advised Child A stated Child B crawled under the 2 bathroom stalls and touched his and _and this happened on three separate occasions. I advised Ms. Vance that an incident report would be required. On 11/24/2021 I received an incident report regarding the allegations pertaining to Child A and Child B. On 12/06/2021 I made an unannounced visit to the center. I spoke with child care staff member Erin Krzykwa. She stated she has been with the center since She stated she does not recall a date, but either a Tuesday or "iurs in November, Child A's Mother came to the center on a day Child A isn't scheduled to be there. Child A's Mother asked her if Ms. Vance was present at the center, but she was not. Child A's Mother stated she needed to speak with Ms. Vance, so Ms. Krzykwa stepped out of the classroom for a moment. Child A's Mother then told her that Child A made a comment to her after she picked him up from the center that another child was "messing with him in the bathroom", whom was Child B. Child A's Mother stated Child A reported Child B had crawled under the bathroom stalls and touched Child A inappropriately. Ms. Krzykwa asked in what way did Child B touch Child A inappropriately and she stated Child B touched Child A's and Child A's Mother stated Child A told her this happened three times and Child A's Mother noticed over the past few weeks Child A was not wanting to go to the center and he then disclosed this happened. Therefore, Child A's Mother believes he does not want to come to the center due this happening in the bathroom stalls. Child A's Mother had asked if she would wait at the center until Ms. Vance arrived to speak with her and Ms. Krzykwa advised this was okay and she returned to the classroom. Child A's Mother was able to speak with Ms. Vance about her concerns. Ms. Krzylcwa stated she has not witnessed any inappropriate touching between Child A and Child B nor has she noticed any behavioral changes with Child A. This incident was surprising to her that something was happening where it was affecting Child A's desire to come to school. She explained the process of children using the bathroom is that children can use the bathroom anytime, if a child asks to use the restroom staff members will stand outside of the classroom door and watch them enter the bathroom. However, she explained staff members cannot see inside the bathroom as she needs to keep an eye on the classroom as well. The bathroom is located next to the classroom where they have restrooms for boys and girls. However, all child care children are encouraged to use the rest room before using the center's multipurpose room. When this happens, staff members stand outside the bathroom entrances where staff can see roughly half of the bathroom stalls and hand dryers, but she cannot see the urinals and the other half of the bathroom stalls. Currently, staff members are now standing inside the bathrooms so staff can directly C1 see what is happening and no one is allowed to use the urinials as a result of this incident. 1 asked if this incident could have happened, but staff didn't witness it. Ms. Krzykwa stated yes. She explained these changes of having staff inside the bathrooms happened right after she met with Ms. Vance. She stated she has not spoken with other staff members regarding this, but she has spoke with Ms. Bledsoe to inform her of the new changes moving forward. She stated Ms. Vance has spoken with Child B's Mother and she was just as surprised as the staff members, but Child B's Mother stated a neighborhood child told Child B that it was funny to show people your private body parts. Child B's Mother believes this is where the ideas could have come from with him touching Child A's body. Staff members are monitoring everyone's bathroom use and since the
