On June 30, 2022, Ms. Ali denied knowledge of any staff member having a contagious disease. She reported that she is aware of the licensing statute. She said when they become aware of a child or staff becoming ill with anything contagious, they notify parents in writing on the Bright Wheel application. On August 3, 2022, Child A's Mother reported that they receive communications through the Bright Wheel app if a child becomes sick with something contagious. She stated that they receive most communications through the app. There are no concerns with the communication from the childcare center. On September 1, 2022, Child C's Mother also stated she receives communications through the Bright Wheel app if a child becomes sick with something contagious. There are no concerns with the communication from the childcare center. On September 22, 2022, childcare staff members Amanda Costa and Paige Mora were interviewed separately. They both reported that the center has a policy regarding notifying staff and parents when someone has a contagious illness. Both report that the information is sent out via the Bright Wheel app. Neither Ms. Mora nor Ms. Costa is aware of a staff member currently having or having a contagious disease in the past. On September 22, 2022, Mr. Khan reported that communication is done on the Bright Wheel application. He is not aware of any of his child care staff members having a contagious disease. 5

Child care staff member Courtney Grubaugh hit Child A (4-year- old, female) and Child B (2-year-old, male). In October 2021, program director Brittney Beitler slapped Child C (4-year-old, male) leaving a bruise on his face. 3 On March 22, 2022, I conducted an unannounced inspection We interviewed Brittany Beitler, and CCSMs, Courtney Grubaugh, Kelsey Hofman, Mary Colon and licensee designee, Mohammad Kahn. Ms. Beitler was interviewed. Ms. Beitler denied that any of the children are hit. Ms. Beitler said Child A and Child B are enrolled at the center and are redirected if discipline is required. Child C is no longer enrolled. Child C stopped attending in She denied hitting Child C as a form of punishment. Ms. Beitler said Child C would "throw fits". On one occasion, Child C "threw a fit" and hit his head on the table. She did not observe any marks or bruises from that incident. Ms. Beitler then said that Child C's Mother told her that Child C's Father hit him leaving a bruise to his face. The child information record was requested for Child C. Ms. Beitler said she did not have it onsite, as they recently moved some files to storage, but would send it via email. On March 22, 2022, Courtney Grubaugh said she has never hit any of the children in care, including Child A and Child B. Child B is in the infant/toddler class, and she does not work in that room on a regular basis. She only works in the infant/toddler room if help is needed. Ms. Grubaugh is aware of the center's discipline policy. They are to use redirection with children in care. There is also a "thought spot" used for children to sit and think about their actions. If these techniques do not work, they contact the parents. Ms. Grubaugh also stated that Child C has not been at the center for at least 6 months. She assumed that he stopped coming because of the cost of the childcare. Ms. Grubaugh never saw a bruise on Child C and had no knowledge of any of the staff members using physical discipline. On March 22, 2022, childcare staff member Kelsey Hofman said that she is the lead in the infant classroom. Ms. Hofman is aware of the center's discipline policy. She has no knowledge of Child A, Child B, or Child C being hit while in care. She said Child C is no longer enrolled but she did not have a lot of interaction with him. She did not recall any injuries to his face. On March 22, 2022, childcare staff member Mary Colon said that she works in the preschool classroom. She has not observed any of the children in care being hit. She said she remembered hearing that Ms. Grubaugh hit a child during that last investigation. But she did not remember what the outcome was. She also said Ms. Grubaugh has "spanked" her own son at the center. Ms. Colon reported this information to Ms. Beitler via telephone after work. CI On March 23, 2022, during a telephone interview, Mohammad Kahn, licensee designee, stated that he has never heard any concerns about Ms. Grubaugh using physical discipline with children in care until March 14, 2022, during the exit interview of a former employee. He said that person told him to "watch out for Courtney:'. He said that he was told that Ms. Grubaugh is "rough" with the children. He then conducted an internal investigation in which he found no evidence. He also had no knowledge of Ms. Grubaugh spanking her own son at the center. Mr. Kahn denied that his wife, Brittney Beitler, hit Child C causing an injury. He stated that if a child was injured at the center, it was not by Ms. Beitler. On March 23, 2022, Child A's Mother said she does not have any concerns with the childcare center. She has no concerns with discipline techniques used at the center. She has never been told that Child A was hit by anyone at the facility. Child A has been enrolled for over a year, and they are pleased with the care. On March 23, 2022, Child B's Mother said she does not have any concerns regarding the care that Child B is receiving. She said that she does not have any concer Child E (4-year-old, male) and Child F (4-year-old, male) are given melatonin to fall asleep at naptime. The childcare does not provide nutritional meals as indicated on the menu provided to the parents.

On April 5, 2022 1 initiated this investigation via tele hone On April 6, 2022, during an unannounced onsite inspection, the program director Brittany Kahn (also known as Brittany Beitler) and the owner Mohammad Kahn (also known as Asif Kahn) were present. The allegations were explained. They both denied knowledge of any child being sexually abused at the child care center. They each denied being perpetrators of sexual abuse. They both denied that children in care are prompted to hit one another. On April 6, 2022, childcare staff members Tawana Walker, Molly Peters, and Courtney Grubaugh were interviewed separately. They each denied that any staff members are sexually abusing children in care. They each denied that staff are telling children to hit one another. On April 13, 2022, Child A's Mother was interviewed via telephone. Child A's Mother said Child A no longer attends this child care facility. She said she attended for approximately a year. due to hearing about the complaints surrounding the childcare. Child A'S said she is not sure if children are being sexually abused. She said Ms. Kahn has told her that staff members watch children dress and undress when oottv training. She confirmed that 611 W. OTTAWA • P.O. On September 1, 2022, Child D's (3-year-old, male) Mother said she is aware of the complaints surrounding the child care. She has never had any problems with any form of abuse while her children are at the center. She has 2 children in care and will be enrolling her newborn soon. On April 6, 2022, Mohammad and Brittany Kahn both denied that Child A had been disciplined with a screw. They could not recall a toy or any object in the center that resembled a screw. The childcare staff members said that no one has ever reported these concerns to them. On April 14, 2022, Child A's Mother reported that she has had an "uneasy" feeling about the child care from "day 1". In the past, Child A has come home with unexplained scratches and bruises. Child A has told her that her arm hurt because "the screw bit me". Child A has also had a scratch on her hand stating the screw with the red tip hurt her. Child A's Mother said during the enrollment process, Brittany Kahn admitted to her that they hit children with an open hand. She was "made" to sign a waiver saying that Child A is allowed to be hit. She denied any knowledge of staff members hitting children. She had no knowledge of a waiver. On September 1, 2022, Child D's Mother said she has never had any problems with any form of abuse at the center. She has no knowledge of childcare staff members using inappropriate discipline. She had no knowledge of a waiver.

On 06/23/2021 there was an anonymous complaint made to the office. There was limited information provided in the complaint, and there was no contact information provided. The complaint alleged that there were children playing at a yellow play structure located near the center, there were adults (alleged childcare staff members present) but they had their backs turned to the children and were not paying the children any attention. I conducted an unannounced visit to the center on 06/28/2021. Upon my arrival, spoke to Brittney Beitler, the Program Director. I informed her of the complaint. She showed me the play areas. There was a play area located outside the infant room, that was fenced in, which they are currently not using at the time. There is another playground are located near the parking lot with a large purple structure, which is also not being used due to waiting on approval from the consultant. There is a large play area that also has other play structures located in the same area about 500 ft, or more away from the centers play structure. Those 2 play structures are available and accessible to the neighborhood children. There is a large yellow structure in the area. Ms. Beitler denied that they use the yellow structure mentioned in the allegation and stated that they have never taken their kids to utilize it. She reported that there are several adults and children using the structure throughout the day as it is the neighborhood playground, not the centers. She reported that they are currently using the gym as their play space, as they recently had both of their playground areas and equipment inspected and waiting for approval from their consultant before using them. I interviewed child care staff members Amanda Slusarczyk and Courtney Grubough. They both denied taking any of their children to the yellow structure to play. They reported that they had used the purple structure in the past but had to stop to get inspections and approval from the state. They also stated that they currently use the gym for play time. The Licensee Designee, Mohammad Khan, arrived to the center and I interviewed him. I informed him of the allegations, and he denied that their children use the yellow structure located at the end of the lot. On 07/27/2021 I spoke to Mr. Khan again, and he informed me that they have used the field area, and it may have been possible that some children may have run to the structure on an occasion, but they have "never purposely" allowed any children to go to the structure. He stated that they are fully staffed when they go outside, and they provide proper supervision to the children. He reported that he was unaware of any incident involving any children at the play structure, nor has any caregivers admitted that they have taken any of the children to the play structure. The complaint was made by an anonymous source and no information was provided to contact for additional information.

According to all the care givers interviewed, they denied being at the yellow play structure as alleged. They all reported that they are not allowed to utilize the current outdoor play structure and are using the inside gym at this time for play time. The program director, Ms. Beitler, reported that it is not possible that it was caregivers and young children from their program at the play structure in the field, and that it may have been children from the neighborhood or from other programs located in the building. She denied classrooms being out of ratio at any given time and reported she has more than enough staff members to meet the ratio on a daily basis and she also have floaters to assist when needed. At the time of the onsite inspection, there were 3 infants with 2 caregivers. There was 4 children age 3 years of age with 2 caregivers. There was 14 children ages 2 to 3 years of age with 4 caregivers. On Monday, 6/21/2021, the date identified, there was 16 kids present at the center during the time the allegations identified. Also, during the time identified, the children at the center are either taking naps or utilizing the gym and getting ready for dismissal. The center was within ration at the time of visit and according to the attendance record. The complaint was made by an anonymous source and no information was provided to contacted for additional information.

In October 2021, program director Brittney Beitler slapped Child C (4-year-old, male) leaving a bruise on his face. 3 On March 22, 2022, I conducted an unannounced inspection We interviewed Brittany Beitler, and CCSMs, Courtney Grubaugh, Kelsey Hofman, Mary Colon and licensee designee, Mohammad Kahn. Ms. Beitler was interviewed. Ms. Beitler denied that any of the children are hit. Ms. Beitler said Child A and Child B are enrolled at the center and are redirected if discipline is required. Child C is no longer enrolled. Child C stopped attending in She denied hitting Child C as a form of punishment. Ms. Beitler said Child C would "throw fits". On one occasion, Child C "threw a fit" and hit his head on the table. She did not observe any marks or bruises from that incident. Ms. Beitler then said that Child C's Mother told her that Child C's Father hit him leaving a bruise to his face. The child information record was requested for Child C. Ms. Beitler said she did not have it onsite, as they recently moved some files to storage, but would send it via email. On March 22, 2022, Courtney Grubaugh said she has never hit any of the children in care, including Child A and Child B. Child B is in the infant/toddler class, and she does not work in that room on a regular basis. She only works in the infant/toddler room if help is needed. Ms. Grubaugh is aware of the center's discipline policy. They are to use redirection with children in care. There is also a "thought spot" used for children to sit and think about their actions. If these techniques do not work, they contact the parents. Ms. Grubaugh also stated that Child C has not been at the center for at least 6 months. She assumed that he stopped coming because of the cost of the childcare. Ms. Grubaugh never saw a bruise on Child C and had no knowledge of any of the staff members using physical discipline. On March 22, 2022, childcare staff member Kelsey Hofman said that she is the lead in the infant classroom. Ms. Hofman is aware of the center's discipline policy. She has no knowledge of Child A, Child B, or Child C being hit while in care. She said Child C is no longer enrolled but she did not have a lot of interaction with him. She did not recall any injuries to his face. On March 22, 2022, childcare staff member Mary Colon said that she works in the preschool classroom. She has not observed any of the children in care being hit. She said she remembered hearing that Ms. Grubaugh hit a child during that last investigation. But she did not remember what the outcome was. She also said Ms. Grubaugh has "spanked" her own son at the center. Ms. Colon reported this information to Ms. Beitler via telephone after work. CI On March 23, 2022, during a telephone interview, Mohammad Kahn, licensee designee, stated that he has never heard any concerns about Ms. Grubaugh using physical discipline with children in care until March 14, 2022, during the exit interview of a former employee. He said that person told him to "watch out for Courtney:'. He said that he was told that Ms. Grubaugh is "rough" with the children. He then conducted an internal investigation in which he found no evidence. He also had no knowledge of Ms. Grubaugh spanking her own son at the center. Mr. Kahn denied that his wife, Brittney Beitler, hit Child C causing an injury. He stated that if a child was injured at the center, it was not by Ms. Beitler. On March 23, 2022, Child A's Mother said she does not have any concerns with the childcare center. She has no concerns with discipline techniques used at the center. She has never been told that Child A was hit by anyone at the facility. Child A has been enrolled for over a year, and they are pleased with the care. On March 23, 2022, Child B's Mother said she does not have any concerns regarding the care that Child B is receiving. She said that she does not have any concer Child E (4-year-old, male) and Child F (4-year-old, male) are given melatonin to fall asleep at naptime. The childcare does not provide nutritional meals as indicated on the menu provided to the parents.

Staff had their backs towards the children during outside recess while at a yellow structure. There was 14 children with 2 staff members approximately 18 months to 2 years of age observed at the play structure.
