On 01/12/2022, I spoke with Child A's (10-year-old, male) Mother. She stated that the Champion Yake Elementary Site is Child A's regular school. Child A was diagnosed with type 1 diabetes in May 2021. Child A's Mother was told that she was responsible for providing the training for the Champion staff, therefore she was requested a copy of the policy and she had not received it. There 3 was a training provided to eight child care staff members at the Champion site in 06/18/2021. Child A's Mother stated that she made sure to inform April Standish, licensee designee of Child A condition and needs. She stated that there have been numerous incidents where April Standish failed to accommodate Child A's medical condition. Child A's Mother stated that there have been incidents where there was not a trained staff at the Champion program at Yake. Child A's Mother said Chelsea Carr, Site Director is the only staff trained at Yake that has experience and hands on training with Child A. Ms. Standish is also trained. Child A's Mother said Ms. Carr could be reassigned without notice as she was at the beginning of the school year. However, Ms. Carr has let her know if there are last minute changes in the staff schedules. According to Child A's Mother she was told that she is responsible for providing training for new staff. She offered to conduct the training and was informed that she could not provide the new training. She stated that she was told by April Standish that the training must be conducted by a qualified medical professional. During the Christmas break Child A was enrolled to attend the Champion Erving Elementary Site, winter break program. When Child A's Mother arrived to drop Child A off on 12/27/2021, Karen Suprunuk, Child Care Staff Member (CCSM) and Karlina Kontos, Site Director appeared surprised to see Child A. Ms. Suprunk was trained 06/18/2021 on diabetic management and insulin administration. She told Child A's Mother that she had a conversation with Ms. Kontos, and she had relevant medial experience. Child A's Mother asked Ms. Kontos if she has been trained on diabetic management and insulin administration, and she stated that she had not been trained. According to Child A's Mother, Karen Suprunk did not appear confident in her training with diabetic management and Karlina Kontos did not have any training. Karen Suprunk, called Alyssa Gasparotto, regional manager to get assistance to reach April Standish. Child A's Mother stated that Child A was present during the conversation, and they were not comfortable that if there was a life-threatening medical emergency the staff would be able to provide care. She also did not believe that the CCSMs are capable of handling basic diabetic care. Child A's Mother and Child A decided not to leave Child A at Champion Erving Elementary Site, and he would did participate in the program. Child A's Mother explained that Child A was enrolled in the winter camp weeks prior to 12/27/2021. This was plenty of time to ensure a trained staff person would be available for Child A. She was very concerned that there was not a trained staff present on 12/27/2021 and she was unable to report to work because of this. 4 On 01/24/2022, I spoke with April Standish. She explained that Child A is enrolled at Yake during the regular school year. She stated that Karlina Kontos, site manager was off work due to being ill and Ms. Suprunk was available at Champion Erving Elementary Site. Ms. Standish explained that on 12/27/2021, she was informed by Ms. Suprunk that Child A would not be attending the winter break camp at Champion Erving Elementary due to Ms. Carr not being at the program. Ms. Standish told her that Ms. Carr requested time off, however there were staff members capable of assisting Child A with his meds. Ms. Standish explained that she reached out to Child A's Mother and was unable to speak with her. She texted Child A's Mother to let her know that that she spoke with Ms. Suprunk, and she was in

On 01/12/2022, I spoke with Child A's Mother. She stated that she completed a medical permission slip for Champion Yake Elementary. According to Child A's Mother, the file following the children when they are schedule to attend a different site. Child A's Mother stated that on 12/27/2021, she arrived to drop Child A the center his file was not at the site. On 01/24/2022, I requested the child information cards for Child A and the children who attended the child care center from Champion Yake Elementary program. I was informed by Ms. Suprunk that the records were not at that site and are still at Erving. The records are sent back to Yake when the children complete the winter break program. On 03/14/2022, I spoke with Ms. Carr and Ms. Standish. They both stated that a copy of the child's enrollment packet, medical records (if applicable), and child information cards are sent over to the site that the child will be attending. The child information records are available electronically. The staff member can log in the system and print the records, pick up a copy from the child's regular site, or contact Ms. Standish so that she can mail a copy. Ms Carr stated that Child A was enrolled in the winter break program, however his attendance for the week of 12/27/2022 was not confirmed. Ms. Standish stated that she spoke with Child A's Mother on 02/16/2022 at the training. She explained that she apologized for the miscommunication, as Champions responsible for paying for the staff's trained. Ms. Standish stated that she was hoping that Child A's Mother could coordinate the training because her neighbor was a nurse. Ms. Standish stated that she never had a problem with paying for the training. Ms. Standish stated that she will ensure that all new and old staff are scheduled for the follow-up training with the nurse who conducted the training on 02/16/2022. There is an online agency training that is required annually. She will make sure that all new staff and old staff complete this training. rj

On 06/30/2021, I conducted a telephone interview with Child A and B's Mother. She said while Child A was at the center, he soiled his diaper. She received a call from child care staff member, Allison Roberts, stating Child A pooped his pants and needed to be picked up. Ms. Roberts told Child A and B's Mother that the staff were not allowed to change Child A, therefore, she sent her mother-n-law to the center. Child A and B's Mother said when her mother-n-law arrived Child A was still in the bathroom. She said her mother-n-law went into the restroom to assist Child A, then she took him home. On 07/08/2021, I conducted an unannounced inspection at the facility. I interviewed child care staff member, Allison Roberts. Ms. Roberts admitted that the allegations are true. She said Child A was enrolled through the center's online system and therefore, staff did not know that Child A was too young to be in the program. Ms. Roberts said 06/22/2021 was Child A's first and only day in the program. She did not know that Child A was not potty trained, nor that he was wearing a diaper until he pooped himself. This is also when she realized that Child A was only 3 years old. Ms. Roberts said she called Child A and B's Mother to inform her that Child A soiled his diaper. She told Child A and B's Mother that she was not allowed to change Child A's. Child A's oldest brother went into the bathroom with him to assist him in changing himself. Ms. Roberts said Child A's grandmother came to the center and cleaned him up, then took Child A home. On 07/13/2021, I conducted a telephone interview with program director/licensee designee, April Standish. Ms. Standish said the computer system does not flag ages, therefore, Child A and B's Mother was allowed to enroll Child A into the program, even though he was too young. Ms. Standish said Child A and B's mother claimed she told Karen Suprunuk, that Child A had pull-ups in his backpack. However, Ms. Suprunuk denied that she was told this. Ms. Standish said she spoke with the staff regarding diaper changes. She said she told the staff that they should have assist Child A in changing his diaper, regardless of his age. She provided guidance to staff for changing diapers when accidents occur in the future. I inquired about safe guards to ensure that children who do not meet the age requirement are not enrolled in the program. Ms. Standish said the site supervisor is responsible for reviewing enrollment packets that are completed online. She said parents are also given a written packet to complete and return, however, Child A and B's Mother never returned their packets. 3 On 07/14/2021, I conducted a telephone interview with Karen Suprunuk. She provided the same information as Ms. Roberts. She said when Child A arrived, he was on her list of students that were supposed to be in the program, therefore, she did not question anything. She said no red flags went off until Child A soiled his pull- up.

On 06/25/2021, I received an incident report from Ms. Standish. The report indicated the Child A and B's Mother said Child B was pulled by his arm to move him to another area. She also said a chair fell behind him. On 06/30/2021, I conducted a telephone interview with Child A and B's Mother. She said she spoke with her two oldest children separately and they both provided the same account of what happen. Child A and B's Mother said the children informed her that Ms. Suprunuk grabbed Child B by the arm, pulling him out of the chair. She said when Ms. Suprunuk let go of Child B's arm, he fell to the ground. Child A and B's Mother said she received a phone call from Ms. Roberts. Ms. Roberts informed her that Child B was playing with the toys inappropriately and when he was redirected, he hit one of the teachers. She asked to speak with the teacher involved and the phone was given to Ms. Suprunuk. Child A and B's Mother said Ms. Suprunuk told her that Child B was being disrespectful and said she had to redirected him several times. Child A and B's Mother said she did not like Ms. Suprunuk's tone, nor did she feel that she should have been talking to Child B in that 4 manner in front of the other children. She said, "If she talked to me that way, I would have hit her too". She was also concerned that Child B did not get to eat his lunch. Ms. Roberts said she did not see the entire events that occurred with Child B. However, Ms. Suprunuk came to her and told her that Child B hit her, and his mother needed to be contacted. Ms. Roberts saw that Child B was upset and sitting in a chair; therefore, she went over to him to intervene. Ms. Roberts said she took Child B into another room and attempted to talk to him, but he was not listening to her and refused to talk. She said Child A and B's Mother attempted to talk to him over the phone, however, he refused to talk to her. Ms. Roberts said Child B refused to talk to anyone for the rest of the day. She said at lunch time Child B refused to eat. She said he was offered lunch multiples times, however, he refused to eat. During the onsite inspection, I conducted a group interview with Child C (8-year-old female, Child D (7-year-old male) and Child E (8-year-old male). The children said they didn't remember a lot of details about what occurred. Child C and Child E said Child B was not listening to the Ms. Suprunuk. They remember Child B sitting in a chair rocking back and forth. Child E said Ms. Suprunuk grabbed Child B by the arm, and he pulled away. He does not remember if Child B fail to the ground after this. Child D said Child B did not get to eat his lunch and Child E said Child B refused to eat. All the children said the child care staff members are the center a "pretty nice" and they like attending the center. Ms. Standish provided the same details as Ms. Roberts. She said she arrived at the center and saw Child B balled up, sitting in a chair, not talking, or interacting with anyone. Ms. Roberts was trying to talk to him. She went over to him and asked him if he wanted to eat lunch and he refused. Ms. Standish said Ms. Roberts and Ms. Suprunuk told her that Child B was having a rough day and had to be redirected about his behavior. Child B was playing with two toy eggs, pretending that they were breast. Child B and his brothers were also being disruptive when they were doing a group activity. She said Ms. Suprunuk said she put her hand on Child B's shoulder to guide him to another chair. She also took his arm to get him to get out of the chair. However, she did not use force, nor did Child B fall to the ground. Ms. Suprunuk and Ms. Roberts said they spoke with Child A and B's Mother, and she was upset. Ms. Standish said she spoke with Child A and B's Mother on three separate occasions and each time her concerns changed. She said she initially called about Child A and was upset because he had not been changed. Then, she called and said Child B's lunch was withheld by the staff. Lastly
