On 01/12/21 The complainant stated that the Infant classroom has two sinks, one for diapering and one for dish washing. The staff wash the children's bottoms in the dish washing sink because it is a larger sink. On 01/21/2021 Ms. Taylor, Ms. Mallett, Ms. Newkirk, and Ms. Schaffrath stated that they have no knowledge of infants being washed in the dish washing sink. On 01/21/2021 I observed the childcare staff members washing the children's hands in the designated diapering sink. I did not observe the children's bottoms or hands being washed after diapering in the food prep sink. On 02/19/2021 and 03/02/2021 I left a message for Ms. Hite and I did not receive a return call back. On 03/02/2021 Ms. Coon and Ms. Foley have never observed anyone washing children in the infant classroom dish washing sink.

She has said that. was too heavy for her to pick-up but not in a negative way. On 01/21/2021 Ms. Taylor stated that her classroom is never out of ratio and she has not heard or seen the other classrooms being out of ratio because they will have floaters that will go into the classrooms when needed. Since Covid-19 the children are not moved to maintain ratio. Ms. Mallett, Ms. Newkirk, and Ms. Schaffrath stated that the center is very good with maintaining ratio; they are never out of ratio. Child care staff members will go to other classrooms, the children are not moved. On 01/21/2021 I observed the child care staff members to child ratio in all of the classrooms, and they were in compliance with the following: • Infant classroom ages birth- 12 months. child care staff members to child ratio is 1:4 and there were 5 children with 2 child care staff members. • Young Toddler classroom ages 12 months- 2 years. child care staff members to the child ratio is 1:4 and there were 8 children with 2 child care staff members. • Older Toddler classroom ages 2 — 2.9 years. child care staff members to child ratio is 1:4 there were 11 children with 3 child care staff members. • Beginning Preschool classroom ages 2.9 — 3.5 years. child care staff members to child ratio is 1:10 there were 13 children with 2 child care staff members. • Preschool classroom ages 3 — 4 years. child care staff members to child ratio is 1:10 there were 14 children with 2 child care staff members. • Pre-k classroom ages 4 — 5 years. child care staff members to child ratio is 1:12 there were 22 children with 2 child care staff members. • School-agers ages 5- 12 years. child care staff members to child ratio is 1:18 there were 3 children with 1 child care staff members. I was provided the daily attendance for the week of 01/18/2021. After review, I determined that the center maintained the required child care staff members to child ratio in all of the classrooms. On 03/02/2021 Ms. Coon and Ms. Foley stated that their classroom is not out of ratio, and she is not aware of the other classrooms being out of ratio. Before Covid- 7 19 they would move the children to maintain ratio. Currently, the center has additional caregivers to cover in the classrooms when needed.

I observed exposed dry wall in the bathroom under the toilet paper dispenser in the beginning preschool classroom where the soap dispenser was removed from the wall. There is a hole in the older toddler classroom wall by the bathroom door. I did not observe exposed nails or wires in the Pre-K, Beginning Preschool, and Older Toddler classrooms. On 03/02/2021 Ms. Coon and Ms. Foley had no knowledge of broken floor tiles in the Beginning Preschool and Pre-k classrooms or exposed nails or wires in the Pre- K, Beginning Preschool, and Older Toddler classrooms.
