On August 16, 2023, I completed an unannounced on-site inspection. I interviewed program director, Ms. Joanne Figot and child care staff members (CCSM), CCSM 1, CCSM 2, CCSM 3, and CCSM 4, separately. Upon my arrival, I inspected the infant and toddler classrooms. The classrooms are in the north end of the building. There is a large room that is divided by shelves into three separate spaces: Infant room, toddler room, and dining room. The infant room is approved for a capacity of five children. The toddler room is approved for a capacity of four children. The dining room is shared space between the infant and toddler classrooms. I observed CCSM 2 sitting in the dining room with her head on the table and CCSM 1 in the infant classroom with four children. There were five children in the toddler classroom with no child care staff members. The children were asleep. As I entered the dining room, CCSM 2 left. I interviewed CCSM 1 who said CCSM 2 and CCSM 3 are child care staff members in the toddler classroom. CCSM 2 and CCSM 3 were both on lunch breaks. CCSM 1 admitted that there is a ratio issue. CCSM 1 said during naptime, she watches both classrooms when CCSM 2 and CCSM 3 go on break at 12:30 pm. CCSM 1 said they usually go for an hour. She goes on break when CCSM 2 and CCSM 3 comes back to the classroom. CCSM 2 covers her break in the infant classroom. When asked if she has ever had more than five children in the infant classroom, CCSM 1 said yes. CCSM 1 said last Monday, August 7 she had six children. CCSM 1 said when she has more than four children, CCSM 2 helps her. CCSM 2 "hops back and forth over the shelves" from the toddler to the infant classrooms. I attempted to interview CCSM 2, but she indicated that she was ill and left during the inspection. I interviewed CCSM 3 who said she works in the toddler classroom with CCSM 2. asked if there have ever been more than four children in the toddler classroom. CCSM 3 said yes. There are usually four to seven children a day. CCSM 3 did not indicate a specific date. CCSM 3 said she takes an hour break around 12:30 pm to 1:30 pm. CCSM 2 also goes on break at the same time. When asked who supervises the toddler classroom when she and CCSM 2 go on break, CCSM 3 said CCSM 1 watches the infants and toddlers by herself. CCSM 3 said CCSM 4 helps in the morning, but she leaves around 9:00 am to be with the school-age children. I interviewed CCSM 4 who said she works in the infant and toddler classroom from 6:00 am to 9:00 am. From 6:00 am to 7:30 am, she and the children are in one 4 classroom. If the fifth child arrives, CCSM 1 clocks in early, or Ms. Figot helps, and they split up into their respective classrooms. By 8:30 am, there are four child care staff members in the classrooms. CCSM 4 said she leaves at 9:00 am to be with the school-agers. CCSM 4 said there's usually five infants on Monday, four infants on Tuesday, four infants on Wednesday, five infants on Thursday and four infants on Friday. When there are more than four children, CCSM 2 helps in the infant room. I interviewed Ms. Figot. I informed Ms. Figot that there were five children in the toddler classroom with no child care staff member. Ms. Figot said CCSM 2 should've been in the classroom and not on break. The child care staff members should take their breaks one at a time. Ms. Figot said the staff schedule their own break times. She said she will have to keep a closer eye on when child care staff members are going on lunch breaks. I reminded Ms. Figot that the ratio for infant and toddler is one child care staff member to every four children. There were five children in the toddler classroom. Even with CCSM 2 in the classroom, they would've still been out of ratio. Ms. Figot said CCSM 5 who is a staff at the Rosebrook in Wixom location told her that child care staff members may go on break if the children were all asleep. I told Ms. Figot that ratio must always be maintained. I also informed Ms. Figot that they're over
