On February 3, 2022, licensee designee, Ms. Jamie Miller, and central administrator, Ms. Melissa Sell, called and self-reported an incident that occurred during naptime in the Young Pre-K Classroom. Ms. Miller said Child A's Mother spoke with program director and licensee designee, Ms. Melissa Lagona, on February 2, 2022. Child A's Mother said Child A (female, 3 years 11 months old) told her that child care staff member, Ms. Jennifer Spurgeon, pulled on her neck. Child A's Mother described it like being strangled. Ms. Miller does not believe there were 2 any marks on Child A's neck. The incident occurred during naptime on February 2, 2022. On February 4, 2022, I received a copy of the incident report. According to the report, on February 3, 2022, Child A' Mother and Child A's Father emailed Ms. Lagona concerning Child A and Ms. Spurgeon. Child A's Mother and Child A's Father said when Child A arrived home, they asked Child A about her day at school and she said Ms. Jen "pulled on her neck during nap." This occurred on February 2, 2022 in the Young Pre-K Classroom. On February 8, 2022, I completed an unannounced on-site inspection. I interviewed program director and licensee designee, Ms. Melissa Lagona, and child care staff members, Ms. Jennifer Spurgeon and Mr. Rylie Jackson, separately. Ms. Lagona said on February 2, 2022, she received an email from Child A's Mother. She didn't read the email until February 3, 2022 because the email was received late in the evening. Ms. Lagona sent me a copy of the email. According to the email, Child A's Mother and Child A's Father requested a sit-down meeting in the morning. They said Child A told them some concerning things and that they would like to discuss it with Ms. Lagona in the morning. Ms. Lagona said she didn't have a sit-down meeting with Child A's Mother and Child A's Father because she was assisting with coverage in the classrooms in the morning. Ms. Lagona asked Child A's Mother if they could schedule a telephone meeting around mid-morning. Child A's Mother sent her a follow-up email in which she informed Ms. Lagona that Child A said, "Ms. Jenn pulled on her neck during nap." Child A's Mother and Child A's Father asked Child A what pulling on her neck was and according to Child A's Mother, it looked like strangulation. Child A said it hurt and it made her cough and cry. Ms. Lagona said she called and spoke with Child A's Mother and Child A's Father. Child A's Mother and Child A's Father reiterated theirs concerns and said they're uncomfortable with Ms. Spurgeon providing care for Child A. Ms. Lagona said Child A's Mother and Child A's Father did not indicate if there were any marks on Child A's neck. Ms. Lagona provided coverage in the Young Pre-K Classroom on Thursday, February 3, 2022. She did not see any marks on Child A's neck. Ms. Lagona said on Wednesday, February 3, 2022, the child care center closed early at 4:00 P.M. due to the weather. Ms. Spurgeon did not take a break and Mr. Jackson took his break from 11:00 A.M. until 11:36 A.M. Ms. Spurgeon and Mr. Jackson were both in the Young Pre-K Classroom during naptime. Ms. Spurgeon left the center at 2:20 P.M. 3 Ms. Lagona said she has never had any issues with Ms. Spurgeon. Ms. Spurgeon has been with the child care center since January 22, 2019. I interviewed Ms. Spurgeon who said she was in the Young Pre-K Classroom with Mr. Jackson. They were transitioning to naptime and the children were on their cots. Ms. Spurgeon said she was cleaning and saw Child A standing and bouncing on her cot. Child A threw her blanket and took her sheets off of the cot. Ms. Spurgeon said she used Teacher Help. Teacher Help gives the child three warnings to correct their behavior before a caregiver intervenes. Ms. Spurgeon said she gave three warnings before she walked over to Child A's cot.
