Based on interviews with child care staff member and documentation review, there is no evidence to show that Child A was under the supervision of Champions at the time of this incident. Child A was also never left unattended and was outside with a child care staff member from Stottlemeyer Head start. Technical assistance was provided in effort to ensure a better transition.
