Child Care Staff Members provided appropriate care and supervision to Child A on 2/5/2025. Child Care Staff Members were unaware Child A stuck his finger in the hinge of the storage room door. Child Care Staff Members acted quickly in applying first aid, notifying Child A's Parents, notifying the facility office, and notifying the department.

During on-site, it was observed that 3 rooms were over the assigned capacity. The young toddler room was over the capacity by 3 children. The toddler room was over the capacity by 3 children and the school-aged area/multipurpose room was over capacity by 6 children.

On 08/15/2022, I called the Complainant to obtain additional information. The Complainant observed a rule violation on 08/10/2022. CCSMs Narjas Abdul Razaq and Anya Saad took kids outside for play. When Complainant looked outside, Complainant counted the children and notice that one child was missing. The Complainant then walked back to the classroom and found Child A in the "do not enter" area. Child A was sleeping on a cot. The area is enclosed, and Child A could not be seen by anyone else in the area unless they went to the area that was "off limits". The Complainant informed the two CCSMs that Child A had been left in the area alone, but no one told the program director. 2 I conducted an unannounced on-site inspection at the center on 08/1/62022. At the time of my arrival, I interviewed the licensee designee, Nachwa Alhoussaini, I informed her of the allegations. She was unaware of the information and stated that nothing like that had been brought to her attention. She then stated that she had fired someone the previous week for not doing their job and felt that the report was made because of it. I interviewed CCSM, Narjas Abdul Razaq while on site. I asked if she recalled leaving Child A in the classroom while they went outside to play, and she said yes. Then I asked if Child A was left unsupervised for any amount of time. She stated, "We always tell the mother when she goes to sleep." I had to reiterate what I was asking as she did not seem to understand. I asked again, did Child A stay in the room alone, and she said yes. I asked, did you do a head count while outside, and she said " No". I then asked who was watching Child A while she was sleeping in the room that was "off limits", she stated, Child A's Mother asked them to let her sleep while she is at the center. On 08/30/2022, I interviewed CCSM, Zahara Saad. She was reported that she could only recall allowing Child A to go back into the building to use the bathroom. She checked on Child A, and Child A was sleeping in the room on a cot because she was tired. She said Child A was not unsupervised because someone from the kitchen staff was cleaning the multipurpose room and would have been able to see Child A. I asked if the kitchen staff was a CCSM, and she stated no. CCSM, Anya Saad was not present the day of the onsite inspection. I called the center again on 09/26/2022 and requested that Ms. Saad return my phone call. While at the center, I observed the multipurpose room. There was 27 children present. Child A was also present, in the "off limits area" sleeping on a cot. This area has a signed posted that says, "off limits". I opened the door and entered the area and Child A was laying on a cot next to the divider wall. There was no way for anyone to provide any direct supervision to Child A as she could not be seen unless CCSMs entered the area. I observed Child A to be in the exact area the complainant reported that Child A was left unsupervised. I returned to office to speak with Nachwa Alhoussaini to inform her of what I had observed. I also informed her of what Ms. Razaq stated. Ms. Alhoussaini said Ms. Razaq did not understand my line of questioning. She then called Ms. Razaq into the office. I asked Ms. Razaq again, if they had left Child A inside the classroom sleeping and she stated "yes" again. Ms. Razaq reported that they did not inform Ms. Alhoussaini or anyone else that it had occurred. On 08/30/2022, contact was made with Child A's Mother. She was informed of the allegation. She stated she does recall informing the staff that Child A had not been getting much sleep at home and asked if they would allow her to sleep in the 3 morning and wake her up for breakfast. She was not informed that Child A was left inside while the other children were taken outside for play. On the date of my on-site, the CCSM Anaya Saad was not present to be interviewed. I called the Program Director on 09/26/2022, and 09/29/2022 requesting that she have Ms. Saad give me a ca

(b) Sleeping patterns indicating when and how long the child slept. (c) Elimination patterns, including bowel movements, consistency, and frequency. (d) Developmental milestones. I (e) Chanaes in the child's usual behaviors.
On 2/11/2022, I conducted a joint investigation at the Dreamy Child Care center. The program director Ms. Alhoussani arrived at the center approximately 20 minutes after we arrived. Ms. Alhoussani stated children's diapers are checked regularly, and this information is given to the parents on the classroom dojo app. She denied Child A spent the whole day in one single diaper. We interviewed the primary caregiver, Ms. Mariam for Child A. She denied Child A went a whole day without being changed. She had not observed him with a diaper rash. She stated Child A was out for two weeks prior to the allegations being made. He was only back at the center a couple of days then stopped coming. We interviewed Ms. Ameerah; she helps in the infant room. She stated she had not observed Child A with a diaper rash. She stated diapers are changed regularly. They document diaper change on the app and the information is forwarded to the parents. On 2/18/22, I interviewed Child A's by telephone. Child A attended the center from until about the He was out for two weeks due to an illness. She stated she marked iM*s iaper before dropping him off. When she picked him up, he had on the same diaper. When asked, Child A's Mother stated she did not speak to his caregivers about him being in the same diaper all day. I asked her if she was getting the daily reports regarding Child A's diaper changing and food intake, she stated they are tracked through the app. She was only informed of one diaper change over two days. I reviewed the diaper changing log for February 8, 2022. Child A was changed at 9:45 am and again at 2:30 pm. It was documented he was asleep during the noon time diaper change. He left for the day around 3:00 pm. On 2/22/2022, I contacted Child B's Grandmother, Child C, Child D and Child E's Mother. None of them had any complaints regarding the children being wet at pick up and not being changed during the day. They stated they are informed of diaper changing through the classroom dojo app. contacted parents and was informed that they were pleased Pwith the center. They were kept abreast of the children daily information through the classroom dojo app. 3 While observing the infant room, I observed the CCSM's changing diapers and then documenting the change on the classroom dojo app. They also had handwritten sheet for diaper changes. Ms. Alhoussani provided me with a copy of the diaper log. On 03/14/2022, a conference was held with APPLICABLER ULE R 400.8137 Diapering; toileting. (6) Diapers and training pants must be checked frequently and changed when wet or soiled. ANALYSIS: There is not enough evidence to determine that diaper changing is not done regularly. I observed diaper changing while at the center. My interview with parents stated they were informed by the classroom dojo when diapers are changed. The complainant stated she received notice of diaper changes via the classroom dojo. The diaper log showed that Child A was changed twice on 2/8/2022. CONCLUSION: VIOLATION NOT ESTABLISHED

On 2/11/2022, Ms. Adamic and I were in the infant room and observed an infant in the swing. There was a blanket, and the bottle was propped on top of the blanket. On 2/11/2022, Ms. Alhoussani was informed of the allegations regarding bottle propping. Initially she denied staff would prop a bottle for an infant. We informed her what we observed and that we had pictures. She stated she would talk to staff about propping bottles. On 2/18/2022, I interviewed Child A's Mother. She stated she had seen Child A sitting in the swing with his bottle propped. She had also observed Child F next to Child A with his bottle propped with a blanket. She mentioned it to the CCSM who stated, "I thought he could hold his bottle." She also stated she took Child A four full 6-ounce bottles. At pick up time, she was given three full bottles and the one bottle that was full was the one that was propped in the swing. The CCSM's stated Child 3/4 A slept most of the day and had just woken up right before she arrived. He was being fed, the bottle was propped while he was in the swing. I interviewed Child C, Child D and Child E's Mother. She stated she never had an issue with her children not being fed their bottles. She had not witnessed bottles being propped.

Child A was fed on demand, the CCSM stated Child A was asleep for most of the day. Child A was fed upon waking up. Child A's Mother picked him up early and that is why all the bottles were not used.

Child B was not immediately removed from the swing after she fell asleep. Child B remained in the swing for at least 10 additional minutes after falling asleep.

On October 26, 2021, I initiated this investigation via telephone with Child A's mother. She stated that HFM spread throughout the child care center and the families were never notified. Child A also contracted pink eye (conjunctivitis). She was not aware if other children had pink eye. A notification of that a child had pink eye was not posted. Parents were not made aware. On November 8, 2021, during an unannounced on-site inspection, I interviewed Nashwa Alhoussani, licensee designee, Maryam Al-Shammary and Tabarek Al- Naseri, child care staff members. Ms. Alhoussani said that she was made aware of a child in the early preschool room having HFM on October 11, 2021. Once she was notified, there was a notice posted on the front door and a notification was sent out on the parent portal, ClassDojo. The notice remained posted on the front door until November 5, 2021. Child A's Mother reported Child A having HFM October 25, 2021. The same protocol was followed, the notice was posted on the front door and an announcement was sent on ClassDojo. Child A was the first child to contract HFM in his classroom. Child A's mother did not report Child A's pink eye infection. Ms. Al-Shammary said postings were up stating the children in care may have been exposed to HFM. She said that when this or any other infectious illnesses occur, the information is shared with parents. She knew that Child A had HFM but was not aware he had pink eye infection. Ms. Al-Nasari stated that she was aware of HFM being present at the childcare. She observed postings on the walls and doors informing parents. Ms. Al-Nasari was not aware of any children having pink eye. On December 9, 2021, I interviewed Child B's (4-year-old female) Mother. Child B's Mother said she does not have any concerns regarding the child care facility. Child B's Mother has been notified of communicable diseases when they occur. The information is sent via text through the parent app, ClassDojo. On December 9, 2021, I interviewed Child C's (1-year-old male) Father. Child C's Father said Child C has been enrolled at this facility for about 6 months. He also said the center has notified the families in writing through the ClassDojo application, as well as postings at the building. 3

R 400.8330 · 1 R 400.8330 Food services and nutrition generally. (4) A center shall ensure a child who has special dietary needs is provided with snacks and meals in accordance with the child's needs and with the instructions of the child's parent or licensed health care provider. 5
On October 26, 2021, Child A's Mother said Child A is lactose intolerant. She has made the child care center aware of this in writing. She supplies the center with lactose free milk for Child A. Child A has been having digestion issues for several months. Ms. Alhoussani admitted to her that Child A was mistakenly given whole milk on October 25, 2021. On November 8, 2021, Ms. Alhoussani said Child A's Mother brings lactose free milk for Child A. The last carton received was on October 26, 2021 and expires on November 11, 2021. She denied admitting to Child A's Mother that he was given whole milk, as he was never given whole milk. I observed the carton of lactose free milk with Child A's name on it. I also observed documentation indicating that "dairy" as an allergy for Child A. Child A's child information card states "dairy (no milk)" on the allergy line. Child A is given food as listed on the menu. Ms. Alhoussani stated that based on Child A's Mother always discussing milk with them and writing "no milk" on the child information card, Child A was not given whole milk. However, he was still given food with dairy in it, i.e., pizza, pasta, etc. Ms. Al-Shammary said Child A is never given regular whole milk. She said that Child A's Mother brings his milk that is lactose free. Child A is given the food on the menu. No food substitutes are made for his dairy free diet. Ms. Al Naseri stated that Child A is never given regular milk. She also indicated that Child A is given lactose free milk. Ms. Al Naseri said that Child A is not given substitute foods when dairy items are served. She said that Child A's Mother never said that he couldn't have food with dairy in it. On December 9, 2021, Child B's Mother said that Child B has food allergies. The allergies are documented at the child care center. There has never been an issue with Child B receiving the wrong foods. On December 9, 2021, Child D's Mother said that Child D has a food allergy. She stated that Child D is given food as documented in his paperwork. On December 9, 2021, I attempted to contact Child A's Mother to obtain follow-up information. At the time of this report, Child A's Mother has not returned the call.

On 04/12/2021, I conducted an unannounced investigation at the facility and interviewed the program director, Ms. Nashwa Alhoussaini. She denied the allegations. She works hard to ensure the well-being of everyone at the center. The facility does not have and would never allow children to run around with runny noses. If a child arrived in such a condition, they would be carefully screened. They would not be allowed to stay if they are sick or have a "constant" runny nose. She takes the health and safety of everyone seriously and has called parents to pickup children if they exhibited any signs of a cold, or serious illness. Children are never sent outdoors without being properly dressed in the winter. If 2 proper clothing is not sent by parents, a call would be made requesting the items be brought in. According to Ms. Alhoussaini, she is surprised a complaint was made since she has an open-door policy. Parents and staff talk to her about everything and she has heard no mention of either allegation. During the onsite inspection, I did not observe any children with runny nose. On 04/19/2021, I contacted child care staff member (CCSM) Kay Jaber, Jessica Schilling, Ciera Smiley and Mariam Bazzi. They have not witnessed children outside without proper clothing during the winter months or seen children running around the facility with a runny nose. On 04/20/2021, I interviewed Child A's Mother, Child B's Mother and Child C's Mother. Each parent stated their child(ren) has been enrolled at the facility for several years and they have no problems or concerns with the center. Child A's Mother "loves" the center. She believes the director to very approachable and available to discuss any issues. The idea of someone complaining about the alleged issues did not make sense to her since she has found the director easy to talk to. If someone had mentioned the concerns, she would have taken care of things immediately. She has never witnessed a child at the facility with a runny nose, but it would be of no concern if she had. According to Child A's Mother, "Children get runny noses all the time and it's no big deal unless it's constant and has other symptoms." I know they have called me to pickup my children if they showed any signs of sickness." She also stated, "I have two children there and they are properly dressed when outside." Child B's Mother states her children have been at the facility for several years and there's no problems or concerns. She has not witnessed or has knowledge of the reported allegations. She does not believe any children would be allowed to go outside without being properly dressed. The facility takes "good" care of the children and if she had an issue, it would've been discussed with the director. Child C's Mother is "surprised" to hear the allegations and finds them hard to believe. She has witnessed children dressed properly in the winter and has no knowledge of children being allowed in the facility with a runny nose. Children should be expected to have an occasional runny nose and it is not a big deal to her.
