Based on the on-site inspection, interviews with CCSMs, PD, W 1 and parents of children in care, the food service has met the nutritional needs of the children in care. The food provided is follows the child and adult care food program (CACFP) guidelines.

On 04/10/2025, an unscheduled on-site inspection was completed. The building was observed to be clean, free from clutter, and debris. There was no observed threat to the health and safety of the children.

Based on interviews with PD, CCSMs, and reviewing the attendance records for 04/2025, CCSMs were found to be out of ratio 16 days. There has not been a second CCSM when at least 3 children are present between the ages of birth and 3 years of age.

Based on interviews with PD, CCSMs, parents and reviewing the attendance records for 04/2025, CCSMs were found to be out of 16 days. CCSMs have been out of ratio in the infant room, toddler rooms, preschool room, and school age room. PD and CCSMs have stated that additional staff are needed to assist with ratios in the center. Parents have expressed that the center is understaffed. There has been communication with PD regarding the concerns related to ratio, but it has been an ongoing concern.

On 04/04/2023, the department received an incident report from program director, Shanalee Hughes. The incident report indicated that Child A's parents arrived at the center and reported that after Child A was picked up from the center on 04/03/2023, Child A's Mother notice blood in her underwear. Child A reported that she fell off a bike after naptime, around snack time, in the classroom. On 04/12/2023, I interviewed Child A's Mother over the phone. She stated when she went to bathe Child A, she noticed blood in her underwear. She took her to the emergency room the next day where a scratch was noticed on each inner lip of the vaginal area. Child A's Mother called the center to find out what occurred during the day. No one knew of a time in which such an injury could have happened. Child A uses the restroom unassisted and is generally potty trained. When Child A's Mother 2 was at the center the next day, she, the director and CCSM1 inspected the play and educational items in the classroom attempting to identify what could have caused the injury. One CCSM did recall a time when Child A fell off a riding toy the day prior. That toy was looked at but determined to not have caused that kind of injury child. Child A's Mother is concerned as to how her daughter could've been injured in this manner. On 04/14/2023, I conducted an unannounced inspection at the childcare center. I interviewed Shonalee Hughes and CCSM1 while at the center. Mrs. Hughes stated that she nor her child care staff members (CCMSs) are aware of an incident where Child A could have been injured. She stated she removed one or two items that could have a sharp edge. She noted that it would have to have two edges in close proximity in order to cause the injury as described. There was a toy dinosaur that had ears that could have caused an injury although the ears are rubber with no sharp edges. All play equipment inside and outside were inspected. CCSM1 said Child A did fall outside while riding a trike but the trike sits very low to the ground. No such incident has occurred in the past. All CCSMs are fingerprinted and aware that child abuse and neglect is against the law. While at the center I interviewed Pre-K teacher, CCSM1. She stated Child A showed no distress during the day. She does not know how or when she was injured. CCSM1 was working alone with the children on that day. I observed the classroom and play equipment throughout the center and found all equipment to be safe with no hazards. On 05/10/2023 I interview Child A's Mother over the phone, requesting the medical documentation. She stated that she had not yet received anything from the hospital. When asked if she could recall the discharge findings, she said findings were inconclusive as to how the injury could've occurred. The physician said her hymen was intact, indicating no sexual penetration. Child A continues to attend the childcare center. On 07/07/2023 an exit conference was held with Mrs. Hughes. She agreed with my findings and was receptive to reminding CCMSs to provide appropriate supervision at all times although it is unclear as to how the injury was sustained. She shared that Child A was heading to kindergarten in the Fall but did finish the school year at the childcare center.

On 10/31/2022, provided inappropriate care and supervision of Child B when she left her in the restroom alone without notifying her coworkers and leaving for the day. On 1013112022, was seen picking up at least two toddlers by their arm lifting them off their feet and placing them in a different area of the classroom. There is no evidence that provided inappropriate care of Child A on 1012712

While watching video footage from 10/31/2022, a mixed age group of children were served microwave popcorn by CCSMs. The children ranged from 2- to 3-year-olds.

On 6/10/2022, licensing consultant, Timika Haley contacted licensee designee, Jennifer Gundersen regarding an incident she reported one week prior. During that call, Ms. Gundersen informed Ms. Haley that the program director was no longer at the center. On 6/13/2022, I interviewed child care staff member (CCSM), Martia Hussey. She 2 was not aware of the exact date the program director last worked at the center. On 08/16/2022, area manager LaTanya Ellington spoke with Ms. Gundersen. Ms. Gundersen said that five days had not passed from the time that she learned that their program director was not returning and the time that she spoke with Ms. Haley. Ms. Gunderson learned the pervious program director was not returning to the center on 06/06/2022. Ms. Haley called her regarding another incident on 06/10/2022. Ms. Gundersen said it was her plan to inform Ms. Haley via email, that the program director was no longer at the center. However, a CCSM at the center provided Ms. Haley with the information before she could.

On 6/13/2022, Ms. Hussey stated that Child A was transitioning to preschool and visiting this day. At the end of the day, preschool, and prekindergarten children combined to go outside. Child A's Mother arrived and picked up her infant first. CCSM, Tori Kopack took her children outside. Child A's Mother went outside to pick up Child A and she was not there. She came back inside the center and stated that Child A was not outside. Ms. Kopack stated she took her out and went to check for herself. CCSM, Jamiya Stone informed her that Child A never came out. Everyone began to look for Child A by searching all rooms. Ms. Godell heard Child A at the door trying to open it. Child A was still in the classroom by herself. On 6/15/2022, I interviewed CCSM, Courtney Godell. She is the acting program director. It was the end of the day, and the classrooms were combining. Ms. Kopack was asked to take her few children outside with Ms. Stone and her children. Ms. Godell continued to combine rooms. Child A's Mother arrived at the same time the children were transitioning. After leaving the infant room, she walked into the 3 preschool room and Ms. Kopack informed her Child A was outside. Child A was in the restroom when she walked in. Child A's Mother went outside looking for her. Child A's Mother came back inside stating Child A is not outside. Ms. Godell and Child A's Mother both walked to the preschool door together and heard crying. Child A was in the classroom by herself for at least two to five minutes. Ms. Kopack admitted to Ms. Godell that she did not conduct name to face because she only had a few kids. On 6/15/2022, I interviewed Ms. Stone. She had around 10 to 12 children outside. Ms. Stone always conducts name to face when children transitioned to her. She informed Child A's Mother that Child A never came out with her class. Ms. Kopack came out looking for her also and Ms. Stone confirmed that she did not bring her out. Everyone inside the center began looking for Child A. Ms. Stone remained outside with the children. On 6/15/2022, I interviewed Ms. Kopack. She transitioned her children from the classroom to outside with Ms. Stone. She conducted a name to face check at the door and again before leaving for outside. The children ran outside with Ms. Stone she counted as they ran out. Two children from Ms. Stone's class came inside to use the prekindergarten restroom. Ms. Kopack stated that she failed to conduct her third name to face count like she normally does and realized she made a mistake. Child A's Mother arrived, and she informed her that she had taken her outside. She came back inside and Child A's Mother informed her that Child A was not outside. Ms. Kopack went to check for herself and Ms. Stone informed her she never came out. Ms. Kopack came back inside to look, and Ms. Godell was at the preschool door with Child A's Mother. Child A was twisting the doorknob.

On 6/10/2022, Ms. Haley informed me that she received the written incident report. Ms. Gunderson made a verbal report regarding the incident on 06/03/2022. She contacted Ms. Gundersen one week after she verbally reported the incident to her because she had not received the written report.

On 03/31/2022, I contacted Child A's Mother. He's doing well but will not be returning to the facility. On 03/21/2022, she received a call from an "unknown staff' who told her Child A was vomiting. They discussed the vomiting and later in the conversation the staff told her he had been bit by another child in the room. No additional information was provided by the caller. Approximately an hour later Child A's Mother arrived to pick him up and observed 3 bite marks on his right leg. They looked severe to her and the skin was broken. Staff never indicated during the call how severe the bites were and offered little information as to what happened. She immediately took Child A to Canton Center Pediatrics for an exam. He was prescribed amoxicillin due to it being tender to the touch. Photos of the bite marks were submitted and are on file with this department. On 04/05/2022, I conducted an unannounced investigation at the facility. I inspected the infant room where the biting happened and discussed the incident with school education manager, Ms. Courtney Godell and program manager, Danielle Turcsak. I reviewed the attendance records and the completed incident report. The documents show Child A arrived at 08:10 a.m., and the biting incident occurred at approximately 10:30 a.m., in the infant room. Child A `s Mother was notified at approximately 11:00 a.m., and she signed him out at 12:50. The day of the incident 11 children were present with three child care staff members. Ms. Godell was on duty and recalls staff bringing Child A to the office and showing her what she recalls was two bite marks. She went into the classroom to get additional information and staff explained Child A was in the bouncer and Child B (17-month-old female) was next to him. None of the child care staff members saw the actual biting take place. Ms. Godell requested an incident report be written and she would call his mother. Shortly after returning to the office, she was called back into the classroom and told Child A was vomiting. Child A's Mother was immediately called and informed about both incidents. She was not concerned about the vomiting and stated it was common for him to "spit up." Child A's Mother stated she would pick him up in a few minutes. Upon arriving she saw the bite marks and left without any communication with management and did not get a copy of the completed incident report. She did send an email the following morning stating he would not be returning. Ms. Godell reached out to her but no calls have been returned. Ms. Turcsak was on vacation when the biting incident occurred, but Ms. Godell updated her when she returned. On 03/28/2022, Ms. Turcsak called Child A's Mother to check on him and ask about the withdrawal. She stated, he was fine but she was unable to discuss anything else with her. 3 •' :• •' •' i I! • • ! On 04/14/2021 I interviewed Child A's mother over the phone. She said the doctor said the injury doesn't seem to match the story of what happened. She is concerned about supervision. She can look on the app on her phone to see in the classroom and she can tell that Ms. Kayla is not supervising properly. She said that her son has told her that Ms. Kayla is mean. He sometimes acts scared when he goes to the center. 3 On 04/14/2021 I conducted onsite interviews at the center. I interviewed program director, Brittany Payton. She said she was present in the building when Child A fell off the playscape on 04/09/2021. Childcare staff member, Kayla Vogt was outside supervising 10 preschool age children. The lead caregiver, Kelnisha Foster, was inside the building. Neither childcare staff member has a history of inappropriate supervision. "This was just an unfortunate accident." The center has a camera system, but the outside area was not in view when Child A fell. After the incident Child A's mother was notified by phone and first aid was administered. Ice was placed on his swelling wrist. Child A was able to move his hand and fingers. The playground inspection indicated that the space was approved for children ages 3 to 12 years old. Mrs. Payton provided me with a copy of the attendance record for the day of the incident. She also gave me the contact information for both childcare staff members as they were not present for a face-to-face interview. I also obtained child information cards for three of Child A's classmates. On 05/05/2021 I interviewed both caregivers over the phone. Ms. Vogt said she transferred to Canton location for an unrelated reason. She has been working with the organization since January of this year. She believes she has been properly training on how to supervise children. She said on the day of the incident she was too far from Child A to catch him before he lost his footing and fell. She said he was crying and holding his arm, so she knew he had hurt himself. Ms. Foster said she was inside the classroom cleaning and preparing for the children to return. She is the lead preschool caregiver and has been with the organization since 2017. She described Child A as an age-appropriate male child. She denied any concerns regarding Ms. Vogt's ability to proper supervise the children. Both caregivers are aware of that they are mandated reporters. On 05/05/2021 I requested a copy of Child A's medical records from his mother. On 05/20/2021 I interviewed the parents of Child A's classmates over the phone. Each respective parent denied any concerns with supervision at the center. On 06/02/2021 Child A's mother said over the phone that she was not able to get the medical records from Healthy Urgent care or St. Mary's Hospital. She said the cast was taken off Child A after about a month and he is healed. He is still attending the center and there has not been any concerns since.

On January 6, 2021, I initiated this complaint via telephone with Britny Peyton, program director. Ms. Peyton reiterated the information that was on 2 the incident report. She stated that Shardae Ford, child care staff member was placed on an administrative leave pending further investigation. On January 6, 2021 via telephone, Child A's mother reported that Child A has returned to child care. She reported that Child A stated, "Ms. Shardae grabbed me by my coat and pushed me." She stated that she believes her son because Ms. Shardae is more "strict and stern." Child A's mother stated that Child A has said things in the pass about Ms. Shardae, but she had not reported it. She said Child A has attended this child care center for 8 months. On January 14, 2021, via telephone, Shardae Ford, child care staff member reported that Child A and another child had party blowers. She stated that Child A spit his blower out at the other child. She said that she asked Child A to stop, but he did it again. At that point, Ms. Ford stated that she took the party blower away from Child A and he began to cry and kick. Ms. Ford said that she then bent down to his eye level, put her hand on his chest, and asked him to stop and to calm down. She said that he eventually calmed down and was going to sleep when his grandmother arrived. Ms. Ford said that she heard Child A tell his grandmother, "Ms. Shardae pulled my dinosaur jacket and threw away my blower." She said that she never grabbed Child A by his jacket and she never became angry with him. On January 21, 2021, during an unannounced site inspection, Courtney Godell, assistant director stated that she has never witnessed Ms. Ford mishandling a child. Ms. Godell reported that they were working with Ms. Ford in regard to her tone. She reported that Ms. Ford spoke "like a dictator" and not nurturing. She said that Ms. Ford was employed there for 2 years. On January 21, 2021, in a face-to-face interview, Sophie Thompson, child care staff member reported that on the date in question she took children into Ms. Ford's class. She stated that she did not see Ms. Ford grab Child A. She said that she has never witnessed Ms. Ford grab or act in anger with any of the children in care. Ms. Thompson stated that she has not had any concerns with Ms. Ford. On January 26, 2021, via telephone, Child B's mother reported that Child B has been at Childtime since 4 months old. She said that Child B is in Ms. Ford's class and loves her. She has never had any concerns with the care that Ms. Ford provides. 3
