    On 5/19/2022, I interviewed program director, Allison Lee. She was gone for the day when the incident occurred. Child care staff member, Allana Robinson was present with three children in her classroom. Ms. Robinson was talking to a parent who was picking up a child. She observed the remaining two children during her conversation with him. At some point, Child A left the classroom and Ms. Robinson was not aware. She was not sure if the doors were closed or not. Ms. Robinson informed Ms. Lee that three other CCSM's were present including a preschool parent. On 5/19/2022, I Interviewed Child A's Mother. She is a CCSM at the center. She was in her classroom singing to her children with her classroom door closed. It took 2 a couple of seconds and she thought she heard Child A crying. She listened closer and went to the door, where she observes Child A in the hallway crying. She brought him in the classroom with her. Child A's Mother tried contacting Ms. Lee through CCSM, Tabitha Locklear. She did not explain to Ms. Locklear why she was looking for Ms. Lee. Child A was with her for about five minutes. CCSM, Autymn Lazenby was in the infant room and informed Ms. Robinson that Child A was with his mother. Child A's Mother said she observed a parent from the infant room, in the hallway at the time that she found Child A. She does not know who the parent is. Child A's Mother and Ms. Robinson talked following the incident and Ms. Robinson was very apologetic. On 05/19/2022, I received written statements from CCSMs, Allana Robinson, Tabitha Locklear, and Autynm Lazenby. CCSM, Allana Robinson's statement says, on 5/17/2022, around 4:30 p.m., Child C's Father arrived for pick up. Ms. Robinson checked to make sure Child A and Child B were still in the class before speaking with him about Child C's reports. Another child from the preschool room was being picked up at the same time. During this time there were CCSMs, a child, and other parents at the door. Child C and his father left, and she noticed Child B still playing and Child A was no longer in the room. She grabbed Child B and went to look for Child A. Ms. Locklear was asked if she saw Child A, she replied "no" and began to help her look for him. Ms. Robinson came back to the classroom and Child A was standing with his mother and Ms. Lazenby. She spoke with Child A's Mother afterward explaining what happened. According to Ms. Locklear's statement in writing. On 5/17/2022, around 5:15 p.m., Child A's Mother asked her to get Ms. Lee. Ms. Locklear informed Child A's Mother that Ms. Lee left for the day, but she was willing to assist her if she needed. Child A's Mother never responded. A few seconds later Ms. Robinson came into the preschool room and asked if Child A walked into the room. Ms. Locklear responded "no" and immediately grabbed her three children to assist in looking for him. She looked in the usual places Child A has hidden in the past and did not see him. She walked into the hallway and Child A's Mother was standing by her door holding Child A. A moment later, Ms. Robinson walked in the hallway and she and Child A's Mother went in toddler two's classroom to talk. According to Ms. Lazenby's statement in writing. After the last infant left for the day she started cleaning her room. Ms. Robinson came through the baby gate between both rooms and asked if Child A was in the infant room. She grabbed Child B and put her shoes on and left the room. Ms. Lazenby did the same and went to look for Child A in toddler one. She looked in some of the spots the toddlers like to hide and did not see him. After going in the hallway to look for Child A, she opened the door across the hall and found Child A in his mother's arm. 3 During the interview with Ms. Lee, I inquired about the parent who was in the hall when Child A left the room. Ms. Lee reviewed the camera footage and stated that the parent was Child D. On 7/13/2022, I interviewed Child D's Mother. She said she does not recall seeing Ch

    On 5/12/2022, I interviewed program director, Allison Lee. She was not in the classroom when Child A was injured. She was made aware of the injury to Child A's teeth. Child A's Mother was contacted immediately regarding the incident. Child A received first aid and her father was contacted. Child A's Father arrived and took her home. Child A's Mother contacted the center afterward and told them that Child A was going to the dentist. Ms. Lee stated that Child A was wearing a dress that covered her feet and might have gotten stuck underneath her feet while playing on the floor. On 5/12/2022, I interviewed child care staff member, Samia Elmawair. She walked into the classroom to put something away. She observed Child A playing on the floor by herself with the toy train. Child A was lying on her stomach on the floor with the train under her. Ms. Elmawair walked over to assess Child A and picked her up. She observed blood coming from her mouth and jumped into action. First aid was provided immediately to her mouth. Both parents were notified immediately, and her father picked her up. He stated that he would take her home for the rest of the day. Child A's Mother followed up with the center and stated she was taking Child A to the 611 W. OTTAWA. P.O. BOX 30664. LANSING, MICHIGAN 48909 www.michigan.gov/lara . 517-335-1980 dentist. Child A did not attend Friday 5/6/22 but returned the following Monday. Both teeth are being assessed by the dentist. On 5/12/2022, I interviewed CCSM, Tahani Silma. On 5/5/2022, during circle time Child A was playing with a toy train. Child A began to cry and Ms. Elmawair went to pick Child A up. She saw the blood and began first aid. Child A's Father was called first and then her mother was called to notify her of their findings. Child A left with her father and was later taken to the dentist. She did not return on Friday. Child A returned on Monday with over-the-counter medicine if needed for pain. On 5/16/2022, I interviewed Child A's Mother. The center was super sweet and called Child A's Father and followed up with him. He was made aware of everything that happened with Child A. She was contacted through messaging and aware of what was happening as well. Ms. Lee followed up with her. Child A was taken to the dentist and one of her teeth were embedded near the bone.     On 02/23/2022, an incident report was submitted to the department by the licensee designee, Mignon Murray. The incident reported stated that Child A showed his "no no" to other children while attending the center. The names of all parties were submitted. I received the complaint intake on 03/02/2022. I called Child A's Mother to obtain information about the incident. She said she was made aware of the incident at the time of pick up from the center. Child A does not regularly attend the Stevenson 2 location, he was there only for Winter break. Child A's Mother said Child A told her that someone punched him in the stomach and pulled his pants down. Child A did not know the name of the person he said pulled his pants down. Child A's Mother also stated that she had received conflicting information regarding the incident. I called Child B's (5-year-old female) Mother on 03/02/2022. She stated that Child B was asked what occurred at the center, and Child B stated that Child A came over to her dropped his and pants and underwear. He then pulled them back up and walked away. Child B's Mother stated they do not know any of the children or teachers that were present as Child B does not usually attend Champions at Stevenson and was only there for winter break. I called Child C's (7-year-old, male) Mother on 03/02/2022. She stated that Child C reported to her that he, Child A and Child B were in a group singing and dancing. He said Child A pulled out his "pee pee" and showed it to them. Child C does not know any of the children or CCSMs, as he does not usually attend Stevenson's and was only there for winter break. On 03/03/2022, I called the two child care staff members that were listed as present at Stevenson at the time of the incident. CCSM, Ms. Broadnox said they were in the gym with the children. The children were set up in small groups playing games or whatever they wanted to do. She stated that Child A, Child B, and Child C were together in a small group near a shelf. Child B came to her and told her that Child A showed his "no no". Child C then came over and said he saw it also. She said no other kids saw the interaction, but they did hear about it. Ms. Broadnox stated that she could see the children's head from where she was seated but could not see the lower parts of their body. She saw them interacting with each other and playing. She was not informed about Child A stating another child allegedly punched him in the stomach and pulled his pants down. CCSM, Ms. Terri Comer said during small group time, all groups were in their areas. She was helping students and was informed by Child B that Child A had showed his "no no". She immediately asked other children and Child C confirmed the incident. They interviewed all children individually. Child A denied that the incident happened and acted as if he had no idea what they were talking about. Child B and Child C were the only two children who said they saw what occurred, all the other children stated they only heard about the incident. There was 15 children present (ages 5- 12) with two CCSMs. She was not informed about Child A stating another child allegedly punched him in the stomach and pulled his pants down. Both child care staff members that were present on the date of the incident work at other Champion locations. Therefore, they were not interviewed onsite. All the children involved in the incident also attend other Champion locations and was only 3 at Stevenson because their respective locations were closed for winter break. I completed the onsite inspection to the facility on 03/07/2022. The incident occurred in the gymnasium. The gym was split down the middle, so that only half of the gym was being utilized. I was shown the area where the incident allegedly occurred, as the gym was no longer set up the way it was during the winter break.     On 11/10/2021, the department received an anonymous complaint that a preschool child had a family member test positive for COVID. The child was allowed to return to school and the parents were not notified. On 11/16/2021, 1 conducted an unannounced inspection at the center. I interviewed both the licensee designee Mignon Murray and the program director Alethea Valentine. Around 10/26/2021, Child A's Mother contacted the school and stated a family member tested positive for COVID and Child A would not be in as they had to quarantine for 8 days according to the Wayne County Health Department. Ms. Valentine also stated before Child A could return, she had to have documentation of a negative COVID test, per the school district policy. After providing a negative COVID test, Child A attended school, for a half a day on Friday 11/5/2021. As of 11/16/2021, she has not returned to school. Ms. Valentine and Ms. Murray both stated all parents and child care staff members were provided COVID information. The information is posted all over the building. 2

    On 8/17/2021, Consultant Timika Hailey initiated the compliant via telephone with Child A's and Child B's (3-year-old female) Mother. She reported about two weeks ago at pick up she was told to bring diaper cream for Child A. She didn't understand why, but when she got home and changed him, he had a diaper rash. He didn't have the rash at drop off. She receives diapering reports and says it appears he is only getting changed like four times a day and that's not enough. I interviewed the Program Director, Ann Winter. She reported that Child A usually has red buns daily, which is why they had asked the mother to provide diaper cream. She reported since the first time he started coming to the center, they have always used diaper cream on Child A at changing time. She provided a daily diaper log for Child A from the week of 08/10/2021 thru 09/01/2021. According to the diaper logs, Child A was change at least 4-5 times per day. At each diaper change, cream was applied to Child A every time except for when he was dry. On 08/12/2021 Child A was only changed two times, but on that day Child A also left early and was not present at the facility until the normal time of 4:30 pm. Child A's mother had previously signed a medication form for staff to use the diaper cream. I interviewed staff Anne Beauchamp and Danielle Decker. Both staff denied any children in the infant room having diaper rashes due to the lack of being changed consistently. They documented all diaper changes and have proper documentation for the children that require diaper cream. Child A has had an issue with a red bottom which is why they started requiring the mother to bring diaper cream immediately after he was enrolled. I called Child C's (5-month-old female) mother. She stated that she has no concerns about the care of Child C. She has provided diaper cream for Child C to have applied while at the center. The diaper log shows that Child C was being checked or changed every two hours. Child C has never had a diaper rash while attending the facility. I called Child D (10-month-old female) and Child E (10-month-old female) mother. She was pleased with the care the staff provided for the twins. She has never had a problem with diaper rashes. RULE Diapering; toileting. (6) Diapers and training pants must be checked frequently and changed when wet or soiled. K ANALYSIS: According to daily diaper logs, Child A was checked and changed frequently and had cream applied when he was wet. Child A's mother had been providing diaper cream for Child A since being enrolled. The random parents contacted had no concerns about the care provided to their children. CONCLUSION: VIOLATION NOT ESTABLISHED

    On 08/17/2021, Child B's Mother stated that the children attend this facility regularly. Child B was diagnosed with Hand Foot and Mouth Disease (HFM) about four days ago (which was Friday 08/13/2021). She reported that when she called to notify the school on Monday, 08/16/2021, she was told "oh yeah there are about three others that have that too." She was not made aware of this prior to that conversation. She feels that another child had it first and they did not notify the parents. Ms. Ann Winters reported that she did receive a call from Child A and B's Mother on 08/16/2021 informing her that the children had been diagnosed with Hand Foot and Mouth (HFM). She admitted that she made the statement "oh yeah there are about three others that have it too." She said that because she had just been notified that same morning right before Child A and B's Mother called. She was in the process of sending out an email notification when she received the call and had not hit the send button to send the email. I obtained verification of the email that was being sent out to the parents dated 08/16/2021 at 12:38 pm. The email was a notification to the parents of a Health Issue of HFM in the toddler room. It also stated that there were four children who had been diagnosed with HFM at that time. It detailed the definition of HFM, the symptoms, what they were doing, and what the parents should do. It directed them to Center for Disease website and asked them to contact their children's pediatrician and or the Local Health Department. Ms. Winters also provided documentation to show proof of her contact with the Health Department, with names of all the children in the classroom and contact information for each child who had been diagnosed with HFM. Ms. Winter sent out an email to the parents on 08/17/2021 with notice of closure of the classroom due to the outbreak of HFM. 4 The parents of the children that were at the center on Friday, 08/13/2021 who were diagnosed with HFM over the weekend, call to notify the center of their children having HFM on Monday morning 08/16/2021. Those children who had been diagnosed with HFM over the weekend did not return to the center on Monday, and the classroom was closed effective 08/17/2021 after being directed by the local Health Department. I called and spoke to Child D and Child E's Mother to inquire of the notification of the HFM break out at the center. She reported she received a phone call and an email with the notification of HFM outbreak at the center. She was also provided with additional information at the time they went to pick up the children from the center after receiving the notification.

    On 07/19/2021, the facility self-reported an incident that had occurred during the after-school program. On 07/20/2021, I conducted an on-site investigation at the facility. I interviewed Ms. Anne Winter, the Program Director at the facility. She reported that the incident occurred on 07/16/2021 around 12:35 p.m. The children and staff members were in the gym. There were two classrooms of children in the gym that was divided by a partisan. There was a childcare staff member present with each class. At the time of the incident, it was reported that Child Care Staff Member Ashly Long was attending to a child using the bathroom, and the other Child Care Staff Member Ayah Chami was standing in the middle to watch both classrooms. There was a teacher's station located in the gym, this is where all the equipment and a bottle of cleaner was located. At some point, Child B (6-year-old female) and Child C (6-year-old female) grabbed the cleaner to play a prank on the teacher while 611 W. OTTAWA. P.O. BOX 30664. LANSING, MICHIGAN 48909 www.michigan.gov/lara . 517-335-1980 she was gone. They had planned to spray the teachers seat so that she would get her pants wet. However, Child B decided to squirt some of the cleaner, Oxivir, into 2 of the cups of lemonade that was placed on the table for the children. Child B and Child C convinced Child A that it was juice and watched him drink a few sips of it. Another child ran to tell a teacher of the incident and it was then reported to the office via radio. Child A drank a couple of sips of the mixture and spit the rest of it out. He reportedly told Child Care Staff members that he was "fine." Staff called 911 and Poison Control. They were informed that once the Oxivir was mixed with a concentration that it would not be harmful to Child A. The Emergency Medical Staff, (EMS) also examined Child A and determined that he did not need to be transported for medical treatment. I interviewed both Child Care Staff Members, Ashly Long and Ayah Chami. They were unaware of Child B and Child C removing the Oxivir and pouring it into the cups. They did not see the incident but were informed by other children what had happened. I spoke to Child A's mother. She reported that she was called immediately and informed of the situation. She had agreed to allow EMS to take Child A for medical treatment but was informed that it was not necessary to do so. She also followed up with his appointment for the day and was informed that that there were no complications. Child A will no longer be attending the program.
