On 05/01/2023, I received an incident report that was submitted to child care licensing consultant Rachel Arens by Autumn Ames. According to the incident report, Child A told Autumn Ames that Child B (5-year-old, female) pushed her off a child-sized chair on 04/27/2023 at approximately 10:30am. Autumn Ames, CCSM 1, and CCSM 2 were dressing children, so they did not witness the incident. Child A initially cried but calmed down after CCSM 2 comforted her. Child A cried a few more times towards the afternoon, which was unusual, so Autumn Ames called Child A's Mother. Child A was not acting herself. Child A is very shy so perhaps that is why she did not complain of any pain or discomfort. Child A never complained of shoulder pain until Child A's Mother took her to the doctor. Child A's doctor examined Child A and determined that she was "fine." Child A continued to complain to Child A's Mother, so Child A's Mother took her to urgent care the morning of 04/28/2023. Child A's Mother called Autumn Ames to inform her that Child A sustained a broken shoulder. First aid was not provided to Child A because Autumn Ames, CCSM 1, and CCSM 2 were not aware of Child A's injury. Upon review of the child care licensing file, an increased capacity from 12 to 14 unrelated children and a variance for the personnel to child ratio of 1:7 has been approved by the bureau. On 05/09/2023, I completed an on-site inspection. I interviewed Autumn Ames, CCSM 1, and CCSM 2. Autumn Ames stated that she was caring for the infants, CCSM 1 was caring for the "older children", and CCSM 2 was in the same room with Child A and other children. CCSM 2 was doing the children's hair when Child A fell, so she did not see what happened. Child A cried for approximately two minutes after she fell. Child A never told her, CCSM 1, or CCSM 2 that Child B pushed her or that she was in any pain. Child A's Mother told her that Child A reported to her (Child A's Mother) and the doctor that Child B pushed her off a chair and that her shoulder hurt. She described Child A as very verbal but is very shy and does not talk very much. She stated that there were four related children, and twelve unrelated children present at the time of the incident on 04/27/2023. She noted that all 16 children were under the age of six. I inspected the area and the chair where Child A was sitting at the time of the incident. The chair was in good repair with no notable defects. The wall near the area in which Child A fell had a lower ledge. Autumn Ames stated that Child A could have possibly hit her shoulder on the wall's ledge. Autumn Ames provided me with children's attendance records for the week of 04/24/2023 and child information cards. She noted that Child B, Child N (9-month- 3 old, male), Child L (3-year-old, female), and Child M (21-month-old, female) are her I took photographs of the attendance records and information cards. Autumn Ames stated that she did not have arrival and departure times for her child care staff members because she was not aware that she was required to maintain such records. CCSM 1 stated that Child A was slightly "whiney" on 04/27/2028, but she seemed fine otherwise. CCSM 2 stated that she was in the same room as Child A, but she did not see how Child A fell and hurt herself on 04/27/2023. Child A began crying, so she asked her what was wrong. Child A did not tell her what happened. She comforted Child A for a brief time before Child A went off to play. Child A was "whiney" all day, but never indicated that she was in pain. She noted that it was not "normal" for Child A to be whiney. On 07/18/2023, I sent an email to Autumn Ames requesting child information cards for Child P (4-year-old, male), Child Q (3-year-old male, Child R (4-year-old, male), Child S (4-year-old, female), Child T (21-month-old, female), and Child U (3-month- old, male) and a date of birth for Child 0 (30-month-old, female). On 7/18/2023, I left a voice mail message for Child A's Mother requesting a
