R 400.8161(1) · R 400.8161(1) Maintenance of premises. (1) The premises must be maintained in a clean and safe condition and must not pose a threat to health o r safety.

The center was not maintained in a clean and safe condition and posed a threat to the health or safety of the children. When I arrived on site on 03/02/2026, I observed an exposed light switch, and the toilet had been removed. There was freshly cut drywall in the front bathroom that was not mudded or taped. During interviews Teachers reported there has been drywall occurring while children were in care that caused dust to be in the air. There was a long extension cord in front of the classroom door that was a trip hazard, and children had to use the restroom with exposed wiring.

R 400.8510(1) · R 400.8510(1) Plans and specifications; submission; approval; inspections (1) A complete set of plans and specifications of any proposed center or proposed addition, alteration, or remodeling to an existing center must be submitted to the department for review and approval. If the total cost of the project is $25,000.00 or more, including labor and materials, the plans must bear t he seal of a registered architect or engineer.

Licensee designee failed to submit a complete set of plans and specifications when the center was altered/remodeled. The center altered drywall, electrical, plumbing, and constructing a wall where a door was in the bathrooms.
