On 5/6/2021, I completed an onsite inspection with '. We interviewed program director and licensee designee, "awa=naPaf=ii1lo- Edwards. Ms. Patillo-Edwards stated that caregiver and licensee designee, Jasmine Patillo, did not spank Child A and that the allegations were a miscommunication. Ms. Patillo-Edwards stated that on 4/29/2021, Child A had been acting up and Jasmine Patillo asked Child A if she should call Child A's mother and told Child A "you know you will get a whooping", meaning from Child A's mother. At pick up time on 4/29/2021, Ms. Patillo-Edwards relayed what had happened with Child A that day to Child A's mother. According to Ms. Patillo-Edwards, the following morning, 4/30/2021, Child A's mother dropped Child A off and said she wanted to talk to Jasmine Patillo about whooping her child. Later that morning when Jasmine Patillo arrived at work, she called Child A's mother to clear things up. Child A's mother was aggressive and yelling in the phone, so Jasmine Patillo stated they would talk in person when Child A's mother arrived back at the center. When Child A's mother came to the center at 1:30 p.m. on 4/30/2021, she brought a friend with her. A verbal altercation took place. Child A's mother left and filed a police report. Child A has Ms. Patillo-Edwards stated that at no time has Jasmine Patillo or any other staff member used physical punishment on any child at the center. Ms. Patillo-Edwards provided me with written statements about the events from caregivers, Jullian Collins, Jasmine Patillo, Jawana Patillo-Edwards, Lorenzo Liby, and a parent who was witness to the conversation that took place on 4/29/2021. On 5/6/2021, and I interviewed caregiver, Jullian Collins. She stated that she was working on and never saw any staff members whoop a child and did not recall seeing Child A c in that day. Ms. Collins was aware that Jasmine Patillo did tell Child A that would get a spanking from Child A's mother. Ms. Collins stated that Child A uses the bathroom alone and that no staff go into the bathroom with . When children get in trouble at the center, they have time alone, but nobody gets it. On 5/6/2021, and I interviewed Jasmine Patillo. She denied that she ever spanked Child A or took ■ to the bathroom. No staff have ever spanked any children in care. She stated the staff use time alone or sometimes stretching and yoga to help the children get their energy out. Jasmine Patillo stated that on 4/29/2021, she asked Child A "do you want me to call your mom? You know she is going to whoop you". Jasmine Patillo stated that she never really called Child A's mother though. 4 On 413012021, when Jasmine Patillo arrived at work, Ms. Collins told her that Child A's mother was upset that morning, because someone told her Jasmine Patillo had spanked Child A. Jasmine Patillo called Child A's mother around 7 a.m. Child A's mother stated that Ms. Patillo-Edwards told her that Jasmine Patillo whooped Child A. Jasmine Patillo tried to assure Child A's mother that she did not whoop Child A, but Child A's mother was tired and aggressive, so they planned on speaking about it at pick up time. Jasmine Patillo stated that she then went to ask Child A if Child A's mother told■ to say that Jasmine Patillo spanked and Child A reportedly answered "yes." When Child A's mother did come to ie center, she brought a friend and confronted Jasmine Patillo. Child A's mother reportedly lunged at Jasmine Patillo a few times, but the altercation ended with Child A's mother leaving the facility. On 51612021, and I interviewed caregiver, DaQuisha Cooper. She has never seen Jasmine Patillo whoop a child or take them into the bathroom to discipline them. On 5/6/2021, and I interviewed caregiver, Sarah Cross. She stated that on 412912021 Child A hadgotten into trouble a few times and Ms. Cross sat$r on the time out bench. She does not recall anyone else disciplining Or and did not see anyone whoop . Despite being aware of the mandated reporter obligations, staff did not report their suspicions or concerns to Protective Services. The staff failed to protect Child A by knowingly allowing Child A's mother to abuse Child A verbally and physically without taking appropriate measures to stop the behavior or to prevent it from recurring. Their inaction shows they were not capable to make the appropriate judgements to aromote the safety and wellbeina of Child A.

On 4/29/2021, Jasmine Patillo threatened Child A with a phone call to Child A's mother and saying, "you know she is going to whoop you". The staff at Little treasures allowed Child A's mother to inflict mental and emotional punishment in their presence on what is reported to be on a continuous and almost daily basis.
