On 9/9/2025, during an unannounced investigation inspection, I observed infants relocated to the toddler classroom. Three book cases were used to divide the area and establish an infant section which created an unapproved use of space. The space contained four cribs, a changing table and infant appropriate toys. This altered the approved classroom layout according to the license.

On 9/9/2025, during the onsite inspection, I observed a total of 13 children, ages 8 weeks to 30 months, with four teachers present in the toddler classroom. The combined group of infants and toddlers exceeded the licensed capacity of 12 children for this age group.

On 9/9/2025, during unannounced investigation inspection, I observed a teacher wipe a child's nose and dispose of the tissue. The teacher used hand sanitizer to clean her hands and did not wash the child's hands or sanitize the toys that the child was using.
