On 06/17/2022, the complainant was contacted regarding the allegations. The complainant stated that she did not see incident occur and could not provide any additional information. She was told this information by Diane Johnson and as a mandated reporter she had to report the information. On 06/17/2022, I completed an unannounced onsite inspection at Destinys Children child care center. We spoke to Destiny Grant, licensee/ program director and Diane Johnson, assistant director. The child care center was observed. In the infant room, there were four children, ages 6 weeks to 18 months, with one child care staff member (CCSM). The center was celebrating their anniversary and the children were outside enjoying a water day. There were six toddlers, ages 18 months to 2 years, 12 preschoolers, ages M12 3 to 4 years, and four schoolers, ages 5 and up with seven CCSMs. Video footage from 06/08/2022 was reviewed and child information cards was provided. *hhe prreesscchhooooll room video footage from 06/08/2022 was observed by me, ■ and Ms. Grant. The entire day was reviewed; there was no sound but the and their interactions with their peers and the child care staff were observed. There were 10 children with two CCSM. The CCSM was Bryanna Stewart and Ms. Heffner on the video. Ms. Stewart was in the room at the beginning of the morning for the first hour. Then she left the room several times, leaving Ms. Heffner alone with the children. Ms. Grant stated that Ms. Stewart was completing observations and that was the reason she was in and out of the room on 06/08/2022. The children participated in circle time, they played, left the room, ate lunch and laid down for nap. While the children were playing, Child A (3-year-old, female) seem to have gotten into trouble. Ms. Heffner could be seen picking up Child A and putting her in a chair ruffly for timeout. As Ms. Heffner walked away, Child A quickly jump up and got out of the chair and dance around for a minute. Then she sat back into the chair by herself. Child A did not appear to be hurt from the incident. Ms. Grant stated that Child A does not listen at times. She must be directed by grabbing her hand and then walked over for her to take a break. children. Ms. Grant, Ms. Johnson and other CCSM's are in and out of the preschooler room and no one else has reported any concerns. Ms. Heffner is a CCSM in the preschool Mass she works with Ms. Stewart., the lead caregiver. Ms. Stewart currently works part-tune. Ms. Heffner was at the child care center but left the center before she could be interviewed on 06/1712022. According to Ms. Grant and Ms. Johnson the allegations were not discussed with Ms. Heffner, and they were unsure why she left the center. Child A's Mother arrived at the child care center to pick up Child A. She was interviewed regarding the allegations. She stated that Child A has been in the preschool room since . She stated that the center provides good care, and she had no concerns about the CCSMs in Child A's room. On 06/28/2022, 1 interviewed, Ms. Heffner and Rahannie Tackoor, CCSMs, regarding thePRe"gaions. Ms. Heffner stated that she is a CCSM at the center. She works in the preschool room, Monday through Friday, .0 a.m. to 5.00 p.m. She has been working at the center since . The child to CCSM ratio for the preschool room is 10:1. There is usua y 10 to 12 children each day. Ms. Heffner stated that she was made aware of the allegation by Ms. Grant and Ms. Johnson the morning of 06/17/2022. All the CCSM's at the center was aware of the allegations. She stated that she left the center on 06/17/2022 due to personal issues that day. Ms. Heffner stated that the allegations are not true. She does not hit, pinch, or push the children in the preschool room. She is naturally a loud person and to others it may seem like she is yelling after she says something for the 4 or 5th time, but she is not. She works with Ms. Stewart each day until 11:30 a.m. In the afternoon, Ms. Heffner works
The child care staff member, D'Vonna Heffner, shoves and pinches the preschool children at Destiny's Children child care center. She yells at them and is verbally aggressive with the children.

She was observed on the premises on 06/17/2022, 06/28/2022 and several CCSM states that she continues to work with children at the facility.
