    R 400.8125 · R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times. 4 ! ! • ," liii] i • • INVESTIGATION: Child A's Mother stated she doesn't remember the exact date, but sometime during the week of 01/17/2022 she told Ms. Alexander and Ms. Moore to not give Child A milk because Child A may be lactose intolerant. Child A's Mother stated even though she told them this Child A was still given milk on 01/20/2022. Child A's Mother stated she provided juice for Child A instead of milk, but Ms. Moore told her they could not give Child A juice, they could only give Child A water or a milk alternative instead of milk. Ms. Alexander stated last week (week of 1/24/22) Child A's Mother stated Child A may be having bathroom accidents because he can't tolerate milk. Ms. Alexander stated she asked Child A's Mother if she thought it was just milk or all dairy products, and Child A's Mother said just milk. Ms. Alexander stated their program provides breakfast, lunch, and afternoon snack to the children, and children are provided milk at breakfast and lunch. Ms. Alexander stated since Child A's Mother stated this, they have not given Child A milk; however, Child A's Father has dropped off Child A in the morning with milk in his drink bottle. Ms. Alexander stated they made Child A's Mother aware of this. Ms. Alexander stated she gave Child A's Mother a Great Start Readiness Program food contract form last week to fill out and return about the concern with Child A drinking milk, and Child A's Mother returned it today, 01/31/2022. Ms. Moore stated Ms. Alexander told her last week about Child A's Mother's request to not serve milk to Child A, and she hasn't given Child A milk since then. Ms. Moore stated Child A's Mother initially brought in juice for Child A, which they don't give to children, so now Child A's Mother has provided almond milk for Child A. During my on-site inspection, I observed Ms. Moore give Child A a cup of almond milk with his lunch. Child A's Father stated Child A can't drink milk because it either "blocks up" or causes Child A to have diarrhea. I asked Child A's Father when he became concerned about this, as staff said he was sending Child A to school with milk until recently. Child A's Father stated he was not sure exactly when, but he has stopped sending milk with Child A and Child A's Mother has told the school not to serve milk to Child A.

    On 01/28/2022 I spoke with Child A's Mother. She stated since September 2021 there have been two or three incidents when Child A had a bathroom accident and staff did not clean up Child A. Child A's Mother stated the most recent incident happened on 01/11/2022. Staff called her and stated Child A had wet his pants. Staff changed Child A's clothes; however, when Child A was picked up an hour later Child A was still soiled. Child A's Mother stated she spoke with Ms. Alexander about it, and Ms. Alexander stated staff have no obligation to wipe Child A after Child A goes to the bathroom or has an accident. Child A's Mother stated she does not agree with this and thinks staff should have to wipe Child A when it is needed. On 01/31/2022 I completed an on-site inspection and interviewed licensee designee Kathleen Alexander. She stated Child A enrolled in their program in September 2021 and since then Child A has had several bathroom accidents. Ms. Alexander stated all parents are informed during an orientation meeting in September that staff will instruct children on how to wipe themselves, but staff do not wipe the children. Ms. Alexander stated at enrollment neither Child A's Mother nor Child A's Father reported any concerns with Child A having bathroom accidents or Child A needing any help with toileting. Child A's Mother has stated she doesn't understand why Child A is having toileting issues at school. Ms. Alexander stated staff Ms. Vilma Moore works daily in Child A's classroom, and when Child A has had bathroom accidents, Ms. Moore has assisted Child A in getting cleaned up and changing clothes. Ms. Alexander stated when Child A has had an accident, they send a text message to both Child A's Mother and Child A's Father. Ms. Alexander stated when Child A had a bathroom accident a few months ago, Child A's Father came to the school and took Child A home for the day. When Child A had an accident in December, Child A's Mother picked up Child A, took Child A home, bathed Child A and returned Child A to school that same day. When Child A had the last bathroom accident on 01/11/2022 it was so bad that Child A was covered in feces from the waist down, it was on his shirt, pants, underwear, and socks. They sent a text message to Child A's Mother and Child A's Father at 2:28pm right after it happened. Child A's Father responded back at 2:30pm informing them where they could locate Child A's change of clothes. Child A's Father informed them that Child A's Mother's friend would pick up Child A at 3:30pm. Ms. Alexander stated Ms. Moore helped Child A get cleaned up and change clothes. Ms. Alexander stated Child A's Mother didn't look at the text message until 01/19/2022. I interviewed staff Vilma Moore. She stated Child A has had a few bathroom accidents since he started in September. She believes Child A waits too long and then can't make it in time to the bathroom. Ms. Moore stated one time Child A had an accident and didn't say anything to her, but she noticed he was walking funny. One other time another child said something smelled. Ms. Moore stated the last incident happened during lunch time. Child A stood up fast and said, "I have to go" Ms. Moore immediately took Child A to the bathroom, which is located down the 3 hallway, but by the time he got to the bathroom he had already gone, and it was all over him. Ms. Moore stated she helped Child A clean up the best she could. She wiped his legs and handed him wipes so he could wipe himself and she had Child A change his clothes. Ms. Moore stated she takes the class to the bathroom many times throughout the day; after breakfast at 9:15am, when they come back from outside or the gym at 9:50am, before lunch at 12:00pm, after lunch at 12:45pm, when they come back from outside at 1:15pm, and after nap time at 2:30pm. Ms. Moore stated when they go to the bathroom she always asks Child A if he has to use the toilet. Ms. Moore stated she has only spoken with Child A's Father about Child A's toi
