On May 12, 2022, I called and left a message with the Complainant. On May 12, 2022, I completed an unannounced on-site inspection. I interviewed program director and licensee designee, Ms. Julie Sweder, and child care staff members, Ms. Sara Richard, Ms. Melissa Walter, and Ms. Brooklyn Bennett, separately. Licensee designee, Ms. Giovanna Lacascio-Morrill, and child care staff members, Ms. Tosha Nelson and Ms. Jaime Lopez were not present during the inspection. Upon my arrival at the child care center, I spoke with Ms. Richard who said Ms. Sweder had already left for the day. Ms. Richard called Ms. Sweder who said she would return to the center. I interviewed Ms. Richard who works Monday through Friday from 9:00 A.M. to 5:00 P.M. in the 3's Classroom. According to Ms. Richard, Child A and Child B's Grandfather came out of the Infant Classroom and was upset. He was yelling and pointing fingers. Child A and Child B's Grandfather said Child A (male, 10 months old) was poked in the eye by another child in the classroom. After Child A and Child B's Grandfather left the center, Ms. Richard spoke with Ms. Jaime Lopez who was in the Infant Classroom. Ms. Lopez told her that Child A and Child B's Grandfather was in the Infant Classroom to pick up Child A. While Ms. Lopez was signing out Child A on the attendance records, Child E (male, 13 months old) poked Child A in the eye. Ms. Richard did not remember the date of the incident. During the inspection, I observed Ms. Walter in the Infant Classroom with Child C (male, 6 months), Child D (female, 12 months old) and Child E (male, 13 months old). I interviewed Ms. Walter who said she works Tuesday through Friday from 9:00 A.M. to 5:00 P.M. or sometimes from 10:00 A.M. to 6:00 P.M. She is a floater but is usually in the 3's Classroom. Ms. Walter said she was not present when Child A was poked in the eye. Ms. Walter does not have any concerns regarding supervision. She said the child care staff members are always paying attention to the children. Ms. Brooklyn Bennett works Monday through Friday from 10:00 A.M. to 6:00 P.M. or sometimes from 9:00 A.M. to 5:00 P.M. She is a child care staff member in the 3's Classroom. Ms. Bennett did not witness the incident between Child A and Child E. She was in the 3's Classroom. I interviewed Ms. Sweder who said Child A and Child B were enrolled at the child care center on April 18, 2022. Ms. Sweder said Child A and Child B's Mother and Child A and Child B's Father are both very nice. Ms. Sweder described Child A and Child B's Grandfather as "abrasive and is always looking for a fight." Ms. Sweder said Child A and Child B's Grandfather had issues with the child care center from day one. He was upset that Child A and Child B didn't have cubbies. Ms. Sweder said both children had cubbies, but Child A and Child B's Grandfather didn't see 4 them. The next day, on April 19, 2022, Child A and Child B's Grandfather was upset because Child A's Jacket was missing. Child A's and Child B's Mother had taken the jacket home that morning, but Child A and Child B's Grandfather didn't want to listen. Ms. Sweder gave me a written document indicating events that occurred between her, Child A and Child B's Grandfather and the child care staff members. Ms. Sweder said she started documenting shortly after the incident in the Infant Classroom on May 5, 2022. According to the written document: "On Thursday, May 5, 2022 at 5:00 P.M., we had the following attendance (all classrooms are within ratio): Classroom # Students E!I•Efii Infants 3 1 (Jaime) Toddlers 7 2 (Brook, Alex) 2's 2 1 (Sara) 3-4's 10 2 (Hannah, Melissa) Child A was on the floor when Child A and Child B's Grandfather arrived to pick him up. Jaime (Ms. Lopez) stood up and went to sign him out. When Jaime stood up, another child poked Child A in the eye (not maliciously, the child is one year old). Child A and Child B's Grandfather became very mad and started talking loudly about how another child wasn't going to t

On May 12, 2022, I called and left a message with the Complainant. On May 12, 2022, I completed an unannounced on-site inspection. I interviewed program director and licensee designee, Ms. Julie Sweder, and child care staff members, Ms. Sara Richard, Ms. Melissa Walter, and Ms. Brooklyn Bennett, separately. Licensee designee, Ms. Giovanna Lacascio-Morrill, and child care staff members, Ms. Tosha Nelson and Ms. Jaime Lopez were not present during the inspection. I interviewed Ms. Richard who said she's assisting Child B (female, 29 months old) with potty training. Child B wears pull-ups. Ms. Richard said there has never been an incident involving Child B being left in soiled diapers. She has never observed feces on Child B's legs. During the inspection, I inspected the Infant Classroom and observed Ms. Walter with three children, Child C (male, 6 months), Child D (female, 12 months old) and Child E (male, 13 months old). Ms. Walter changed Child C and Child E's diapers. Ms. Walter cleaned and sanitized the changing pad between each diapering. Ms. Walter washed her hands after diapering, and Child C and Child E's hands were also washed. I interviewed Ms. Walter who said the children's diapers are checked every hour and changed as needed. The daily activity sheet is completed and given to parents at the end of the day. I reviewed the daily activity logs for Child C, Child D and Child E: Child C's diaper was changed at 9:40 A.M. (wet) and at 12:40 P.M. (wet) Child D's diaper was changed at 10:00 A.M. (bowel movement) Child E's diaper was changed at 9:15 A.M. (wet) and at 10:30 A.M. (bowel movement) The daily activity logs did not indicate that the children were checked every hour. The diaper log section has an hour increment starting from 7:00 A.M. to 5:00 P.M. Ms. Walter said each child care staff member completes the log differently. Children are checked hourly, but the diaper log may be left blank unless a diaper is changed. Ms. Walter said Child A and the other children in care are never left in soiled diapers. She has never seen Child A with feces running down his legs and genitals. I interviewed Ms. Bennett who said Child B has never soiled her pull-up. Ms. Bennett has never seen feces on Child B's legs. According to Ms. Bennett, she checks the children's pull-ups or diapers every 30 minutes or an hour. If the child is soiled, she changes the child's diaper. Ms. Bennett said when she checks a child's diaper and the diaper is dry, she writes "dry" on the daily activity log. Ms. Bennett said most child care staff members leave it blank when they check diapers. They only indicate wet or soiled on the daily activity log when diapers are changed. Ms. Sweder said she has never seen Child A or Child B with feces running down their legs. According to Ms. Sweder, if a child had diarrhea, they are sent home per the illness policy. Child A and Child B never had diarrhea while attending the child care program. She recalled Child A being changed at pick up because his diaper was wet, but there were no incidents of feces running down his legs. I reminded Ms. Sweder to have the child care staff members update and complete the daily activity log, especially when diapers are checked. Ms. Sweder believed that child care staff members are marking on the logs when diapers are checked hourly. I informed her that I reviewed Child C, Child D and Child E's daily activity logs and there were no marks indicating the children's diapers were checked. On May 16, 2022, I called and spoke with child care staff members, Ms. Tosha Nelson, and Ms. Jaime Lopez. Ms. Nelson and Ms. Lopez both said the children's diapers are checked every hour and changed if needed. Ms. Lopez said Child B never had diarrhea nor has he been left in a soiled diaper. On May 24, 2022, I called and spoke with Child A and Child B's Grandfather. Child A and Child B's Grandfather said he picked up Child A and Child A had a full diaper; there was so much urine. I interviewed program director and licensee designee, Ms. Julie Sweder, and child care staff members, Ms. Sara Richard, Ms. Melissa Walter, and Ms. Brooklyn Bennett, separately. Licensee designee, Ms. Giovanna Lacascio-Morrill, and child care staff members, Ms. Tosha Nelson and Ms. Jaime Lopez were not present during the inspection. I interviewed Ms. Richard and Ms. Walter who said the car seats are kept in the Cubby Room. They haven't seen any ants in the Cubby Room or on the car seats. Ms. Walter said sometimes there are ants in the Kitchen and Ms. Richard said there were ants in the Office. Ms. Richard believes an exterminator came out, but she does not know the date. Ms. Bennett said there were ants in the Cubby Room on Tuesday, May 10, 2022. A child left a bag of Skittles in the cubby. The candy was thrown away and she cleaned the cubby. Ms. Bennett said there were no ants on the car seats. During the inspection, I inspected the Cubby Room, Kitchen and Office and did not observe any ants. In the Cubby Room, there was one car seat on the floor. I did not see any ants on the car seat. Ms. Sweder said she has not seen any ants on the car seats. Ms. Sweder said they have experienced seasonal ants, but nothing infested. The exterminator is coming out this weekend to assess if the building needs to be sprayed. Ms. Sweder said she will notify parents if it is determined that pesticides are needed. A copy of the Pest Management Policy was obtained during the inspection. According to the sections on notification/posting and re-entry of the policy: " Notification/ Posting — The principal (or lead administrator) or Integrated Pest Management (IPM) coordinator of Leapin' Lizards is responsible for timely pre- notification to students' parents or guardians and the school staff of pesticide treatments pursuant to the requirements under the Natural Resources and Environmental Protection Act 451, Part 83." 11 " Re-entry — In accordance with the Natural Resources and Environmental Protection Act 451, Part 83, reentry to a pesticide treated area may not occur less than 4 hours after application unless product label requires a longer reentry period. `Outdoor ornamental and turf applications of liquid spray pesticides shall not be made on school grounds within 100 feet of an occupied classroom during normal school hours or when persons are using the treatment area." Ms. Sweder gave me a written document indicating events that occurred between her and Child A and Child B's Grandfather on May 10, 2022. According to the document, "Child A and Child B's Grandfather called (and spoke with Ms. Sweder). He stated that one time there were ants on Child A's car seat that had been in the Cubby Room. I thanked him for letting me know, and I told him that we occasionally get ants when the weather warms up and that we're taking care of that situation." On May 16, 2022, I called and spoke with child care staff members, Ms. Tosha Nelson, and Ms. Jaime Lopez. Ms. Nelson and Ms. Lopez both said they have never seen ants on the car seats. Ms. Nelson said there are seasonal ants in the kitchen, and she believes an exterminator has either been out or scheduled to come out. On May 24, 2022, I called and spoke with Child A and Child B's Grandfather. Child A and Child B's Grandfather said he picked up Child A's car seat from the Cubby Room. The child care staff member put Child A in the car seat. He walked out to his car and noticed there were at least 100 small black ants on Child A. Child A and Child B's Grandfather said he wanted to go back inside to complain but instead he left with Child A. He said he wanted Child A and Child B's Mother and Child A and Child B's Father to take care of the situation. When they arrived home, he cleaned Child A and washed the car seat. On 12/3/20, the department received a complaint that the center does not provide required training for their employees on a yearly basis. There was no contact information for the complainant to get more details about the nature, type, or time period of the training. On 12/7/20, I interviewed the licensee designee Ms. Lacasci- Morrill who stated that she was not sure where the complaint was coming from, but she had all the documents to prove that they had provided training to the staff on a regular basis. 611 W. She indicated that as far as they know they had completed all the required training hours for the staff members. She said that they had all completed the refresher course for 2020 but not sure if they had taken the 8 hours health and safety training. Ms. Sweder said that they assumed that if they took the refresher course, they did not have to complete the 8 hours class. Ms. Sweder said that she would make sure that all staff members take the courses if they were not completed. On 12/7/20, I conducted an onsite inspection at the center. I reviewed the following training documents for 14 staff members including current and ex-employees of the center from 2018 to 2020. The staff files that were reviewed for training were as follows: Lead caregiver for pre-k room: Ann Leigh Brast Lead caregiver for the Toddler room: Sara Richard Lead caregiver for the Infant room: Giovanna Lacascio Morrill Lead caregiver for the preschool room: Julie Sweder The above employees are currently employed. Assistant Caregivers: Kennedi Smacks (July 2020 to November 2020) Kellie Hope (April 2019 to August 2020) Elaine Karas (April 2018 to January 2020) Delaney Fox (currently working since July 2020) Selyna Zorraida (currently working since July 2020) Allie Richard (currently working since September 2018) Sidney B (summer of 2019 and 2020 only) George Morgan (November 2019 to September 2020) Isabelle Ownby (summer of 2019 and 2020 only) Ashley Robinson (September 2018 to September 2020) The following training documents were reviewed for all the above staff members: 1. Safe sleep practices, Abusive head trauma, shaken baby syndrome, child maltreatment, Recognition and reporting of child abuse and neglect. 2. 16 annual professional development hours. 3. Health and safety training 4. Health and safety refresher training for 2020. 5. Training on prevention and control of infectious diseases, including immunizations. 611 W. OTTAWA. P.O. BOX 30664.
