On 06/16/2023, I made an unannounced onsite inspection to the child care center. Program director and licensee designee, Stephane Medina was not present during my inspection. During the onsite inspection, I observed the infant/toddler, preschool 1, and preschool 2 classrooms. In the preschool classroom, I interviewed Child Care Staff Member 1 (CCSM 1). CCSM 1 denied the allegation that the children do not go outside. CCSM 1 shared that the children go outside everyday unless there is inclement weather. The children have not gone outside as often due to the poor air quality recently. The children go outside for up an hour in the morning and sometimes a half hour in the afternoon. CCSM 1 stated that the children went outside yesterday and were looking for worms while playing in the wet sand. CCSM 1 shared that the center uses the software app HiMama to communicate with parents and families. CCSM 1 showed me photos of the children outside yesterday and previous days of the children engaging in different activities such as the children playing on the swings and digging in dirt. CCSM 1 also shared pictures of the children engaging with different activities such as tracing letters, writing, coloring and painting activities. CCSM 1 showed me their weekly lesson plan that includes a letter and shape of the week. The daily routine includes: greeting time, large group, breakfast, learning centers, small group, outside time, lunch, quiet/rest time, snack time, and departure. When I asked CCSM 1 about the tablets, CCSM 1 stated that children were in work time, and the children on the tablets have chosen tablets for this time. The children are not forced to be on the tablets, and only educational activities are allowed on the tablets. CCSM 1 stated that children are only allowed to have tablets during work time or nap time if the children do not nap. On 06/16/2023, I interviewed CCSM 2. CCSM 2 works in the infant/toddler classroom. CCSM 2 shared that they typically take the children outside everyday unless it's raining. CCSM 2 stated that all the children go outside, but sometimes the infants don't go outside as much as the older children. The infant/toddler classroom has a posted daily schedule, but they mainly follow the children's lead when it comes to a daily routine. While CCSM 2 is assigned to the infant/toddler classroom, she has been working in the preschool classrooms as well. CCSM 2 stated that the children in the preschool classrooms have a block of time where they can choose which 611 W. OTTAWA. P.O. BOX 30664. CCSM 2 shared that she has not observed children in the preschool classrooms on tablets for more than 15-20 minutes during choice time. CCSM 2 also stated that they have large group time and they read books to the children. CCSM 2 has recently been hired at the child care center and was currently assisting in the kitchen preparing lunch and providing breaks to the other child care staff members. On 06/16/2023, I interviewed CCSM 3 and CCSM 4 in the infant/toddler classroom. When I asked about the children's schedule and daily routines, CCSM 3 stated that the children all have a slightly different schedule depending on the child's age. Many of the children were mobile (walking or crawling) so they are mostly on the same eating and sleeping routine. CCSM 3 stated that the children eat lunch at 11:35 am because they are younger, so their lunch is served first. CCSM 4 stated that the children typically start naptime at 12:15 pm, and some of the children will nap until about 3:00 pm, but most of the children get up around 2:45 pm and then have snack. When I asked the staff members about the television, and if they ever turn it on for the children, CCSM 4 shared that they turn it on around 4:00 pm, and they turn on Mickey Mouse Clubhouse for the children to watch while
The child care staff members do not have a daily schedule or do activities with the children, and the children do not go outside. The children watch television all day or are on tablets. The child care center does not provide a variety of food, and children eat the same foods every day.

On 01/20/2023 in the infant/toddler classroom, an unidentified staff member was heard yelling in an aggressive tone at a child (name unknown) for apparently biting her. The child then walked up to the staff member and tried to hug her, but she refused and left the child crying on the floor. On 01/20/2023 in the infant/toddler classroom, the unidentified staff member threatened to put the unknown child in time-out. At approximately 10:00am on 01/20/2023 Staff Member A left the children in the preschool/preK classroom unattended while she went to the main office to make copies.

On 06/16/2023, during my unannounced onsite inspection, I asked CCSM 1 about the center's menu and food provided. CCSM 1 denied that children eat the same foods every day, and the center provides a good variety of meals. CCSM 1 shared that children are served meals such as spaghetti, quesadillas, fish sticks, and sometimes sandwiches. CCSM 1 stated that the menu is posted in the kitchen and a new menu is posted weekly. While onsite I received a copy of the weekly menu for June 12-16, 2023. Children were served the following meals on the following days: Monday Tuesday Wednesday Thursday Friday June 12 June 13 June 14 June 15 June 16 Breakfast Cereal, Cereal, Cereal, Cereal, Cereal, banana, milk banana, milk banana, milk banana, milk banana, milk Lunch Grilled Nachos, Fish sticks, Hot dogs, Turkey and cheese, corn, mixed Cali medley, green beans, cheese, broccoli, fruit, and oranges and pears, and peas, pears, and milk milk milk oranges, milk and milk Snack Strawberry pretzels and String cheese Pretzels and applesauce yogurt Colby and pretzels cheese parfait cheese `Wednesday lunch substitution- Spaghetti, corn, mixed fruit, and milk *Thursday lunch substitution- Turkey and cheese sandwich, green beans, pears, and milk `Friday lunch substitution- Hot dogs, broccoli, mixed fruit, and milk On 06/16/2023, I spoke to Ms. Medina via telephone. On 01/25/2023 I spoke with the complainant via telephone. The complainant stated that on 01/20/2023 she overheard an unidentified staff member working in the infant/toddler classroom. The staff member was yelling in an aggressive tone at a child who appeared to be approximately 1 years of age, name unknown. This staff member was yelling at the child to stop biting her and yelling that she was going to put them in time-out if he bit her again. The complainant then saw the child try to go to the staff member for comfort and the staff member said, "I am not hugging you; you have been biting me all day long." She then walked away from the child, who was visibly upset and crying. The complainant stated that there were other staff members in the classroom at the time of this incident but could not definitely identify them. On 01/31/2023 I conducted an unannounced on-site inspection. I interviewed Staff Member D (assistant program director), Staff Member B (lead infant/toddler staff member), Staff member C (assistant infant/toddler staff member), Staff Member A (lead preschool/preK staff member), Staff member E (assistant preschool/preK staff member), Staff Member F (GSRP lead staff member), Staff Member G (GSRP assistant staff member), and Stephanie Medina (owner/licensee designee). Interviews were conducted privately in the program director's office. Staff Member D was not at the center on 01/20/2023, although she denied ever hearing any of the staff members in the infant/toddler classroom yell at the children or talk with them in an aggressive tone on any other day that she was in the office. Staff Member D also indicated that she would immediately step into a classroom and address any situation involving a staff member yelling at a child, but again that this has never happened in the past. She also denied hearing complaints from parents or staff members regarding any staff members acting inappropriately with children in the infant/toddler classroom or any other classroom. Staff Member D indicated that staff members are trained on the center's discipline policy and are aware that time- outs are not to be used as part of that policy. Staff Member D provided me the attendance records showing that Staff Member B and Staff Member C, were working in the infant toddler room with six children on 01/20/2023. 3 Staff Member B was working in the infant/toddler classroom on 01/20/2023 and denied hearing or seeing any staff member yell at any of the children for biting them. Staff Member B said that they do not have any children that are in a "biting phase" right now, so she is unsure which child could have purportedly bitten a staff member. She also indicated that in the past she has advised children that she would not pick them up because they had bitten her, but that she always made sure to engage with them on the floor as an alternative. She also insisted that staff members all know to comfort children when they are upset, and therefore it would be very unusual of them to just walk away from a crying child. Staff Member B denied hearing any staff members or parents complain about staff members yelling at children. She also denied having ever placed an infant or toddler in time-out in the classroom and that, per the center's policy, time-outs are never to be used as discipline in the infant/toddler room. Staff Member C stated that she was working in the infant/toddler classroom the morning of 01/20/2023. Staff Member C stated that she was not bitten by any child, nor did she yell at any child or observe anyone else yell at a child that day. She denied that she or any other staff members have ever used time-outs as a form of discipline for infants/toddlers at the center. Staff Member C indicated that she has never received any complaints from parents or other staff members regarding a coworker yelling or being aggressive with any of the children in care. The complainant stated that on 01/20/2023 around 10:00am, she observed Staff Member A leave the preschool/prek classroom unattended while she went to the office to make copies. The complainant stated she has overheard other staff members make comments that Staff Member A often leaves her classroom unattended while she goes to the bathroom or the front office to grab supplies. Staff Member D stated that earlier today (on the day I conducted interviews at the center), Staff Member Band Staff Member I came to her with concerns that Staff Member A was repeatedly walking out of the preschool/preK classroom and leaving the children unattended. Staff Member D indicated that she spoke with Staff Member A about this, but she denied ever leaving her classroom unattended. Staff Member D stated that there is a printer in the office that Staff Member A uses to print out resources for the classroom. Staff Member D indicated that staff members are supposed to get someone to cover their class if they need to use the bathroom or leave the classroom for any reason. Staff Member D provided the class attendance sheet which shows that Staff Member A was alone in the preschool/preK classroom on 01/20/2023 with two children, Child A and Child B. Staff Member B stated that staff members working in the preschool/prek classroom must walk through the infant/toddler classroom in order to exit the area, and she sees Staff Member A leave the preschool/preK classroom unattended on a daily basis. Staff Member B said that she usually runs to the front office to grab something and is only gone a few minutes, but Staff Member A does not ask one of the infant/toddler staff members to step in and watch her class when she does this. Staff Member B indicated that she brought her concerns to Staff Member D earlier that day because of the frequency of these events and her concerns about children in the preschool/prek classroom being left unattended. Staff Member C stated that she does not really know what occurs in the preschool/preK classroom with Staff Member A because she tends to stay to herself and usually keeps the classroom door closed. However, she acknowledged that she often sees Staff Member A step out of her classroom and close the door behind her when she runs to the office to grab something. Staff Member C stated that she assumes the children in the preschool/prek classroom are left alone when this happens, as Staff Member A does not usually ask for anyone to step into her classroom when she leaves. Staff Member E stated that she works in the preschool/preK classroom with Staff Member A but indicated that she does not start on Fridays until 11:00am. Staff Member E says that Staff Member A steps out of the classroom while she is there to go to the front office, but she always remains in the classroom to supervise the children when this happens. Staff Member E stated that the classroom is usually low in attendance, so only one staff member is needed to maintain ratio. Staff Member F stated that the GSRP classroom is right by the main office, so she sometimes sees Staff Member A walk over to the office to grab things. Staff Member F indicated that she is unsure who watches Staff Member A 's classroom when she steps out, but she said that the center usually has plenty of staff to assist in classrooms, if needed. Staff Member G stated that she has witnessed Staff Member A leave the preschool/preK classroom unattended numerous times to grab things from other classrooms or the office. Staff Member G said that she recently approached Staff Member A and told her that she cannot leave the children in the classrooms unsupervised. Staff Member A stated that she is usually the only staff member in the classroom in the morning until around 11:00am (when Staff Member E arrives), but she denied that she has ever left the children unattended in the classroom.
