On 01/06/2023, I completed an unannounced onsite at the center. I interviewed Ms. Michelle Daigle, program director/licensee designee, Ms. Jenny Foster and Drita Shkreli, child care staff member. Ms Daigle stated there was one issue that was brought by a parent about inappropriate music being played on the bus, but this occurred back in September and/or October of 2022. It was the beginning of the school year. Not all of the transportation bus has radios to use, however child care staff members are aware they are not to play music children should not be listening to but after this she told staff not to use the radio at all while transporting. She has not had any other complaints since that time and was surprised it was being brought up at this time. I discussed the allegation of Ms. Bauman smelling of marijuana when at work. Ms. Daigle stated although she is not always the first to see Ms. Bauman in the morning, she has not had any knowledge of Ms. Bauman entering the center smelling of marijuana. She would not allow any child care staff member to care for children, let alone transport any child if the evidence of the smell of marijuana on their person. None of the staff or any of the parents have mentioned this was a concern for Ms. Bauman or any other child care staff in the building. Ms. Daigle would provide me with a coy of their drug policy for the center and would send radio policy use while transporting. Ms. Bauman was not present at the center as she left early due to not feeling well. Ms. Foster works with Ms. Bauman the most and would also be another person to interview. On 01/06/2023, I received documentation from Ms. Daigle of the center's Drug and Alcohol Policy: "KCC considers drug and alcohol abuse a serious matter that will not be tolerated. KCC absolutely prohibits employees from using, selling possessing, or being under the influence of illegal drugs, alcohol, or a controlled substance or prescription drug not medically authorized while at their job, on KCC property, or while on work time. Therefore, it is KCC policy that: (1) You may not report to work under the influence of alcohol, illegal drugs, or any controlled substance or prescription drug not medically authorized. (2) You may not possessor use alcohol, illegal drugs, or any controlled substance or prescription drug not medically authorized while on KCC property or on KCC business.... 3 I interviewed Ms. Shkreli. Ms. Shkreli stated she is normally sitting in the front office when Ms. Bauman arrives in the morning, typically at 7:00 a.m. She has never smelled marijuana on Ms. Bauman and has never felt she has been not appropriate to drive. She also reported that she was only aware of one incident complaining of inappropriate music on the bus during the transport which was several months ago. She has not heard of any further concerns about music on buses. I interviewed Ms. Jenny Foster on 01/06/2023. Ms. Foster been employed for approximately three years in the preschool room. She has worked with Ms. Bauman numerous times. She has never smelled marijuana when working with Ms. Bauman, never had concerns about how Ms. Bauman cared for children, and would have reported if there was ever a problem. On 01/06/2023, I received an email from Ms. Daigle with the following information dated 09/22/2022: "Just a reminder please do not have a radio on when you have the children on the bus, unfortunately some of the music is not appropriate for them to listen to. If you have any questions, please come see me. " On 02/16/2023, I interviewed Child A's Mother by telephone. Child A's Mother stated she had not heard about any problems with inappropriate music playing on the bus during transport. Child A may or may not pay attention with what's going on around him. On 11/28/2022, I interviewed Ms. Michelle Daigle, licensee designee/program director and Ms. Jordan Bauman, childcare staff member. Ms. Daigle stated on 11/18/2022 Ms. Bauman was driving one of the bus transporting children to school in the morning and became concerned about the brakes as they did not "feel right." She was able to safely pull over to the side. The second bus that was on transport came and took the remaining children to school. No children were harmed. She had to contact the mechanic and the bus towed for repair. The annual maintenance for the bus was completed several months previous and there were no issues noted at that time. She will provide copies of the maintenance report. The bus is still in the shop at this time. There was a note placed on the door to alert parents of the breakdown, but she did not send out an email or other notice to parents. I did inform Ms. Daigle that a phone call should have been made to the drop-off schools to alert for potential tardiness. Ms. Bauman stated she started working at the center spring of 2022. She currently holds a chauffeur license. On 11/18/2022, as she initially started driving, she did not notice any issues with the bus. As she continued to drive is when she began to feel that something was not quite right with the brakes. She was able to pull over safely and stop the bus. There was no notice from the bus to indicate that something was wrong, no lights or indicators came on while she was driving. She contacted the center to let them know something was wrong. It took approximately 5-10 minutes for the second bus to arrive to transport the children to their destination. She rode on the second bus with the children. None of the children appeared upset. Children that were being transported to Hevel Elementary School were fine as it related to time: children attending Washington Elementary School would be a few minutes late. The bus that broke down had not given her any problems up until 11/18/22, as she had driven the bus for the entire summer and the last couple of months without any issues. On 11/28/2022, I received bus maintenance copies from Ms. Daigle. The three documents were completed from Expert Automotive on 05/19/2022: (2002 Chevrolet Express, 2011 Chevrolet Express G3500, and 2004 GMC Savana 3500 "Expert Automotive has inspected the 2002 Chevrolet Express, 2011 Chevrolet Express G3500, and 2004 GMC Savana 3500 and we have found it to be in good working order. As of right now there are no problems or concerns with the bus nor can we foresee any near future repairs. We are unable to guarantee anything from happening. Every time the vehicles comes in for service, we do a complete safety and general inspection of all systems, etc. Completed by Steven A Beauchamp, Master Technician. 3 On 12/21/2022, I received an email from Ms. Daigle in regard to an update as it relates to the repair of the breaks. Expert Automotive informed Ms. Daigle they were currently working on the breaks and would replace all brake lines, flex brake hose and brake hose cylinders. On 01/06/2023, I completed an unannounced onsite at the center. I interviewed Ms. Daigle. Ms. Daigle stated the bus was still in the shop as the repair shop was waiting for part to fix the braking issue. On 02/16/2023, I interviewed Child A's Mother by telephone. Child A's Mother stated she did not have any concerns about how Child A is transported by the center. Child A's Father did observe the bus breaking down on the side of the road. She feels the situation was handled well but would have liked a courtesy email about the break- down. Did observe a notice on the door about the breakdown. I interviewed Child B's Mother by telephone on 02/16/2023. Child B's Mother stated she had no complaints about Child B's transportation to and from school. I interviewed Child C's Mother by telephone on 02/16/2023. Child C's Mother did not have any concerns about transportation. I interviewed Child D's Mother by telephone on

On 08/18/2022, I completed an onsite unannounced at the center. Upon arrival, I observed taped on the door the following document: "Washington Township under a water Advisory, Water Boil in Affect" I interviewed Ms. Daigle and Ms. Megan Gazley, child care staff member. Ms. Daigle stated the notice of the boil water advisory was placed on the door on 08/15/2022, at opening. Since they been under a boil water advisory, water that is used is boiled in the kitchen, children are able to wash their hands but dried immediately, parents are sending children with bottled water and some parents have donated cases of bottled water to the center. Each room has their own pitcher of boiled water and is able to obtain more from the kitchen. All outside water activities have been cancelled. The center has been taking the needed precautions, infant/toddler rooms children have readymade bottles and food brought from home. None of the parents and/or child care staff members have brought any concerns to her how the center is handling the boil water advisory. 3 Ms. Daigle provided me a copy of the center's written emergency plan for manmade disasters. Step (1) In the event of a gas leak, power outage, flood, or other man- made disaster.... Step (7) We will begin to contact parents to inform them of the situation through emails and phone calls. I completed a walk through to all of the rooms with Ms. Daigle to observe how the boil water is being handled. In the kitchen there were numerous water pitchers filled with water that was boiled earlier waiting for use by classrooms. There were several pots on the stove that were used and continue to be used as needed. Each classroom was observed to have a pitcher of boiled water ready for use. I asked Ms. Megan Gazley in the toddler room how they were handling items that needed to be cleaned by water for the children, i.e., pacifiers etc. Ms. Gazley stated she does not use sink water faucet at all to clean any toys or children's items that need cleaning. She will only use water that been boiled to clean with and did not have any concerns. On 09/08/2022, I completed an interview with Child A's Mother by telephone. Child A's Mother stated she did not have any concerns about how the center handled the boil water advisory. Emails were sent frequently to parents from the center with reminders to bring bottled water if wanted, water activities cancellation and any updates. There were parents that donated bottled water as well to the center. On 09/08/2022, I interviewed Child E's Mother by telephone. Child E's Mother stated there were no problems with how the center was handling the boil water advisory. There was a sign on the door that effect, emails were sent to parents about updates and boiled water being used. Even when the boiled water advisory was lifted the center took extra day and extra steps to ensure everything was running smoothly before lifting at the center. I interviewed Child B's Mother by telephone on 09/08/2022. Child B's Mother stated the center is very good with communication She received emails detailing was occurring and what to expect during the advisory. There was a note on the front door about the center had boiled water in effect. On 08/15/2022, an email went out to parents in the morning about the center being under the water advisory. On 09/09/2022, I completed the exit conference with Ms. Daigle, licensee designee. I informed her no violations were being cited in the investigation and a copy of the report would be forwarded to her once approved.

On March 3, 2022, Child Care Licensing received a complaint regarding the non- compliance with cleaning and handwashing rules in the toddler room at Krambrooke Child Care. The complainant stated in their description that Child A (one year-old female) "soiled her diaper with pee at around 10:30-h am." When mentioned to Child Care Staff Member (CCSM) Lead Megan Gazley that she had soiled her 2 diaper and needed changed the complainant was told "they'll be fine till 12pm, I only change them every two hours unless they poop. The next time she (Megan) will be changing diapers is at 12pm" Ms. Gazley proceeded to leave the children in their soiled diapers till 12pm. The complaint also mentioned that children were not instructed to wash hands before or after they ate. I left a message for the complainant prior to the on-site inspection on March 3, 2022. I received a message from the complainant after the inspection reiterating the allegations in the complaint. The complainant was clear that children's hands were not washed by staff prior to eating on March 1, 2022. She stated that she only worked at the facility for one day (March 1, 2022) and would not be continuing her employment. On March 3, 2022, I completed an unannounced inspection at Krambrooke Child Care facility in Washington Twp. Upon arrival I met with program director, Michelle Daigle. Ms. Daigle knew about the allegations in the complaint. She stated that she had received an email message from a former CCSM about the care being provided by staff in the toddler room. The complaining CCSM had only worked a day in the facility but had had prior child care experience. Ms. Daigle was told through the email that Ms. Gazley had not changed soiled diapers in a timely manner and provided the reasoning that diapers were changed every two hours unless soiled with feces. As soon as Ms. Daigle received the email, she went right to Ms. Gazley to investigate. Ms. Gazley admitted to Ms. Daigle that was indeed what had happened, and she did not know why she had said that to the new CCSM. Ms. Gazley knew the rule about changing when soiled but said something different. Ms. Daigle stated that she spoke with the two CCSM assistants that worked in the room with Ms. Gazley, Lisa Elmer, and Rachel Nicks. Both stated to Ms. Daigle that diapers are changed when any child soils their diapers regardless of when the two hours is up or how they soiled themselves. Both Ms. Elmer and Ms. Nicks were aware that Ms. Gazley told the new CCSM that diapers are changed every two hours unless there is feces. Due to the information provided to Ms. Daigle about the incident by four different CCSMs, Ms. Gazley received a written write-up. Ms. Gazley will receive training regarding the correct rules regarding diapering and Ms. Daigle will increase monitoring in the room to make sure the proper procedures are being implemented by staff. As for the handwashing complaint, Ms. Daigle was unaware that had occurred. None of the regular Toddler room CCSMs were available but Ms. Daigle showed me the room. I found it to be within ratio with two CCSMs present, Gabriella Gegovic and Daniella Hill. When questioned about hand washing times both Ms. Gegovic and Ms. Hill gave the correct rule response with no prompting. I also asked them about diapering procedures. Again both Ms. Gegovic and Ms. Hill provided the correct rule response. 3 On the wall next to the diaper changing area, I saw the proper, written diapering posted for any CCSM to see and review when changing diapers. Ms. Daigle stated that she has not received any complaints from parents about rashes or marks for the children in Ms. Gazley's care. On March 8, 2022, I attempted to leave a voice message for Ms. Gazley. The voice message box for the phone number provided was full.

(b) A child experiences an accident, injury, or incident. (c) A child is too ill to remain in the group.
On 05/17/2021, I completed an unannounced onsite inspection at the center. I interviewed Ms. Michelle Daigle, licensee designee/program director, Megan Gazley, lead caregiver and Ms. Drita Sckreli, office assistant. I informed Ms. Daigle that a complaint was received with no identifying information to any children in the center. Ms. Daigle stated she was unaware of any issues with reporting to parents of any incidents or accidents. I informed Ms. Daigle there was not a specific child named or classroom identified where this was occurring. She was aware of a posting on the web on concerns about an incident but the incident was not specific to a child. She does not know about any bullying incident. Parents are informed when there is a problem, if a child is sick or there is some other concern to report. Information is provided to parents by telephone, email, etc. Child A (2 years-old female) was playing outside approximately 1 month ago, receiving a scratch on child's left cheek. Once they entered the building child care staff observed a scratch on her face. An incident report was completed, picture taken, scratch was washed and treated. Child A's parent were informed of the incident. On 05/03/2021, Child A was bitten by another child on her right hand. The bite mark was discussed with Child A's Mother and Child A's Father. Child A's Father appeared to understand there was no intent of harm. Child A's Mother wanted the child moved out of the classroom. Ms. Daigle explained that is not their protocol to move a child from the room for one incident who is not a perpetual biter. She wanted to meet with Child A's parent to discuss if they had any concerns that could be addressed at this time. On 05/17/2021, I received a copy of the center's reporting policy in the parent handbook. The policy stated the following: "For minor accidents or incidents, such as but not limited to a minor scrape, bug bite, minor scrape, bug bite, minor cut, bruise etc. the affected area will be washed and ice or cold compounds will be applied as necessary. Parent will be notified when child is picked up at the end of the day. For accidents or incidents that do not warrant a call to 911 but are more serious in nature, such as but not limited to a head injury of any kind, severe swelling, medication issues, unexplained pain, etc. Parent will be called immediately." I interviewed Ms. Megan Gazley. Ms. Gazley stated she has worked for the center for the last four years. None of the parents have brought any concerns to her attention that have not been addressed. There has been only one recent incident of child being bitten in the classroom. All of the children were in a group and Child A started crying. She observed what appeared to be a bite mark on Child A's hand on 05/03/2021. She put an ice pack on the bite (located on Child A's right hand), she contacted the front office to alert staff, a picture was taken. Staff in the front office 2 contacted Child A's Mother to alert of her the incident. Child A has not been bitten before as they do not have any child in the classroom that is a habitual biter. She completed an incident report of the bite mark and Child A was back in class the very next day. On 05/17/2021, I received a copy of the center's incident report dated, 05/03/2021 with time of 9:30 a.m. Incident report stated the following: "Child A was sitting in little people house area when friend bit her. " "Ice and tic administered. " Ms. Sckreli stated on 05/03/2021 she contacted Child A's Mother by telephone to let her know about what happened as it related to Child A's bite mark. The front office will often make the contact with the children's parent when there is an incident, accident, or illness. I interviewed Child C's Mother by telephone on 09/20/2021. Child C's Mother stated she has no concerns about the center. Child C Mother has had a total of three children to be enrolled and attend the center.
