On 05/13/2023, I interviewed Child A's Mother. Child A's Mother explained that Child A was enrolled and started child care with Star Bright mid-February 2023. Based on Child A's age, Child A was placed in the toddler room upon enrollment and attends Monday, Tuesday, and Wednesday typically from 1:30 p.m. to 6:00 p.m. Recently Child A has been seen placed with the preschool children/room for naps and play. Child A is under 30 months old, and it was agreed that Child A stays in her designated toddler room. Child A's Mother does not understand why Child A 2 moves from the toddler room to the preschool room and does not feel like mixing the ages of the children is appropriate. On 05/15/2023, I conducted an unannounced onsite inspection where I observed all the classrooms and interviewed child care staff members, including Licensee Designee/Program Director Rojan Yousif. I observed all the child care rooms during the onsite inspection: • Infant Room is approved for children from four weeks old to 12 months old and there was a total of seven children and two child care staff members present with the youngest child in the classroom being seven weeks old. • Toddler Room is approved for children from 13 months old to 30 months old and there was a total of seven children and two child care staff members present with the youngest child in the classroom being 14 months old. • Young Preschool Room is approved for children 2 years old to three years '/2 old and there was a total of five children with one child care staff member with the youngest child in the classroom being 2 years old. '/2 • Preschool Room is approved for children three years old to six years old and there was a total of nine children and one child care staff member present with the youngest in the classroom being three years old. All the classrooms were within their approved ratio and group sizes, and Child A was observed in the Toddler Room. There were no classrooms that had children of mixed ages in the same room. The child care staff members in the toddler room, Child Care Staff Member 1, and Child Care Staff Member 2, explained that Child A's assigned classroom is in the toddler room. Child A is dropped off in the afternoon three times a week and it is typically during nap time. Sometimes Child A does not nap and rests with the children in the young preschool room depending on staffing and ratio. I interviewed both Program Director Ms. Yousif and Child Care Staff Member 3 during the onsite inspection. Ms. Yousif explained that Child Care Staff Member 3 is the appointed child care staff member that assists her when she is not in the center. Both Ms. Yousif and Child Care Staff Member 3 stated that Child A is almost at the age where she can be transitioned to the Young Preschool Room once she turns 2 years old, and they have been placing her in that room for short periods of times 1/2 when in ratio to assist her in that transition. Also, Child A typically arrives in the middle of nap time and due to ratio, they at times have her in the Young Preschool area until the children wake up. On 08/07/2023, I interviewed enrolled parents Child B's Mother, Child C & D's Mother, Child E & F's Mother, and Child G's Mother. Child B's Mother, Child C & D's Mother, Child E & F's Mother, and Child G's Mother all consistently stated that they have no concerns with ratio and group size of their children's placements at the center. 3

On 05/13/2023, I interviewed Child A's Mother. Child A's Mother explained that on 03/30/2023 her husband went to pick up Child A from the center at 6:23 p.m. and noticed as they were going home that Child A's diaper was soiled. There is concern that Child A's diaper was not checked on that day. On 05/15/2023, during the onsite inspection I interviewed Child Care Staff Member 1 in the toddler room. Child Care Staff Member 1 explained that she is the lead child care staff member in the toddler room and her and the assistant child care staff members regularly check and change the children's diapers. While in the toddler room, Child Care Staff Member 1 presented verification of the daily recordings in each of the children's toddler's log where they document the children's feeding, diaper changes, and sleeping times for the children present in the room today. Per the daily recording log, child care staff members are consistently changing diapers. Regarding Child A, Child Care Staff Member 1 remembers changing Child A before she was being picked up by her parent and apparently had a soiled diaper. There was a short period of time between when Child A's diaper was changed and when 4 Child A's parent arrived. Child Care Staff Member 1 would never intentionally leave a child in wet or soiled diapers. During the onsite, Child Care Staff Member 3 and Ms. Yousif were able to pull past children's records and provided me with verification from Child A's daily toddler log sheet documented and signed by Child Care Staff Member 1 on 03/30/2023 that Child A's diaper was checked and changed at 11:55 a.m., 2:58 p.m., 4:40 p.m., and 6:15 p.m. Child Care Staff Member 3 stated that she knows that Child A's Mother called the center that day and wanted to know why Child A's diaper was soiled upon pick up. It was explained that Child A and all the children's diapers are checked and changed regularly, and Child A must have just soiled herself upon pick up as she was changed before Child A's parent arrived at the center. Child Care Staff Member 3 discussed with the child care staff members in the toddler room to make sure when the parents arrive to do a last check and change before the child is released. On 08/07/2023, I interviewed enrolled parents Child B's Mother, Child C & D's Mother, Child E & F's Mother, and Child G's Mother. Child B's Mother, Child C & D's Mother, Child E & F's Mother, and Child G's Mother all reported that they have never had any concerns with the diapering at the center. The children are frequently checked and changed, and the child care staff members provide copies of daily logs to the parents.

On 05/13/2023, I interviewed Child A's Mother. Child A's Mother stated that Child A was not at the center from 03/14/2023 — 03/27/2023, as they did not need care during that period. Upon returning and dropping off Child A, Child Care Staff Member 3 provided a copy of a letter emailed to the parents on 03/20/2023. On 05/15/2023, during the onsite inspection I interviewed Child Care Staff Member 3 and Ms. Yousif. Ms. Yousif explained that the center does their best to notify parents as soon as they are made aware that an enrolled child or an enrolled child's family member has a communicable disease. At the end of March there was a case of Hand, Foot, and Mouth Disease that was confirmed with an enrolled family's older sibling that was not attending care. As a precaution, notifications as well as informational sheets were provided to the parents. Child Care Staff Member 3 was able to provide a copy of the notification letter dated 03/20/2023 and the informational sheet provided to the parents. On 08/07/2023, I interviewed enrolled parents Child B's Mother, Child C & D's Mother, Child E & F's Mother, and Child G's Mother. Child B's Mother, Child C & D's Mother, Child E & F's Mother, and Child G's Mother all consistently stated that any time the center was made aware of a child or staff illness that was communicable, they have properly notified the parents and provided informational sheets to assist in the matter. There was communication in March and the center closed during the weekend to clean the center as an extra precaution. On 08/07/2023, I conduced an exit conference with Program Director/Licensee Designee Rojan Yousif. I informed Ms. Yousif that there were no licensing violations found during the investigation.

On 08/31/2022, I completed an unannounced onsite investigation. I interviewed Ms. Sameeah Zuhra, office manager, Julia Durdur, child care staff member, Rugaya Hussan, child care staff member, Madleen Issak, child care staff member, and Fahmida Munny, child care staff member. I informed Ms. Zuhra, I needed to see and observe the infant and toddler rooms as Ms. Yousif was not currently at the center at the time of the investigation. I did not observe in the toddler or infant room documentation of primary caregivers' assignments for the children. There was only one child care staff member (Ms. Issak) in the toddler room. There were three child care staff members (Ms. Hussan, Ms. Durdur and Ms. Munny) in the infant room at various times caring for the infants in the room. I observed both rooms for approximately an hour. As there were only two children currently in the toddler room, Ms. Issak was caring for the children. Ms. Zuhra would periodically enter the toddler room and infant room to assist if needed. Ms. Zuhra stated there was another child care staff member assigned to work in the toddler room but she was not currently there. In the infant room, I observed Ms. Munny, Ms. Durdur and Ms. Hussan interacting, feeding and/or diapering various child care children as needed. Primary caregiving by the child care staff members appeared to be based on the need of the child not assignment. I asked Ms. Zuhra how the center determined primary caregivers for the toddler and infant room. The same caregivers are typically in a specific room and each caregiver will help out where needed. I explained that there should be a primary caregiving system in place, to allow for both parents and children to know who to go to for questions and for care. I also informed Ms. Zuhra there needed to be documentation of primary caregiving assignments. I suggested she could place a white board with the required information at the front where parents first come in or before the entrance to either room. I interviewed Ms. Issak. She stated she is typically the child care staff member in the toddler room. They have from two to four children daily at this time. On 08/31/2022, I observed the infant and toddler rooms from 10:40 a.m. until 12:05 p.m. I observed Ms. Issak (toddler room), Ms. Durdur, Ms. Fahmida Munny and Ms. Rugaya Hussan (infant room), documenting on written daily records. I asked Ms. Zuhra about the daily logs. Ms. Zuhra stated if a parent asks about a child's day child care staff member can talk to them about it and daily logs are kept in the office for parents to see them. I informed Ms. Zuhra that the written daily logs must be given to parents daily. Ms. Zuhra had a stack of written daily logs kept in the office. On 08/31/2022, I spoke with Ms. Yousif by telephone. Ms. Yousif stated child care staff members were completing the written logs but were being kept in the office. I informed Ms. Yousif the logs must be given to parents daily. Ms. Yousif would begin to provide the parents in the infant and toddler rooms the daily written log. On 09/08/2022, I interviewed Child A's Mother by telephone. Child A's Mother stated, Child A is enrolled in the infant room. She has concerns about how much Child A is eating daily as Child A is sent to the center with three bottles and return home with two full bottles. Child A's Mother did not receive any written daily log from the center. I interviewed Child B's Mother by telephone on 09/08/2022. Child B started attending child care June of 2022 and is in the infant room. She did not receive any written daily logs previously but the last few days have received them. She did not have any concerns with Child B's diapering and feeding schedule. On 09/08/2022, I interviewed Child D's Mother by telephone. Child D is currently enrolled in the toddler classroom for approximately two weeks. She recently started receiving written daily log for Child D. She did not receive them when Child D first 5 started at Starbright. She only had one concern with Child D being wet when picked up but she had a discussion with the child care staff member (name not given) and has not had any issues since then.

On 06/13/2022, Child A and Child B were found alone by an employee from the Child A and Child B were left unsupervised rom when t ey exited the center at 4:57 p.m. until returned safely to the center at 5:20 p.m. Child Care Staff Member 1, Child Care Staff Member 2, and Child Care Staff Member 3 were unaware that the children walked out of the center.

Child care staff members during the late afternoon and evening shifts have been witnessed to not provide appropriate care and supervision to the children while in their care.
