(10) The playground equipment, use zones, and surfacing in the outdoor play area must be inspected by a certified playground safety inspector and an approval granted for playground equipment and areas used before issuance of an original license, on request of the department, and before using any newly added playground equipment. The center shall provide documentation of the inspection to the department on request and keep it on file at the center. All equipment present on a center playground must comply with one of the following: (a) Centers licensed after January 1, 2014 are required to obtain documentation of compliance with the 2010 edition of the Consumer Protection Safety Commission Handbook for Public Playground Safety for all playground equipment, including equipment for children under age 2. The 2010 edition of the Consumer Protection Safety Commission Handbook for Public Playground Safety from the United States Consumer Product Safety Commission is adopted by reference and applies only to this subdivision of this subrule. Playground equipment was installed in the summer of 2025 and used by the children prior to a certified p layground equipment inspection being conducted.

Child A was injured at the center on 10/09/2025 and Parent A reported the child received medical treatment. The center did not notify the licensing consultant within 24 hours of being notified by the p arent that child received medical treatment.

Child A was injured at the center on 10/09/2025 and Parent A reported the child received medical treatment. The center did not enter an incident in CCHIRP within 72 hours of the incident or being n otified the child received medical treatment.

During the onsite inspection, I observed a child in the toddler classroom standing on a toddler table and jumping up and down. I also observed another child in the toddler room climb a shelf to pull a large bin of equipment down causing the bin to fall to the floor.

During the 05/09/2025 onsite inspection, I observed three infants awake and standing in their cribs while other children were on the floor playing. Child care staff members explained that the children were fighting so they were placed in the cribs to keep them apart.
