On 12/22/2021, I interviewed Complainant by telephone. Complainant stated Child A was placed in the room with the younger children to supervise them. Child A attended two weeks in July (July 12-16, July 26-July 30) and one week in August (August 9-August 13). Child A has to keep another child from playing in the toilet. On 12/22/2021, I interviewed Child A's Father by telephone. Child A's Father stated he attempted to verify with Ms. Linington that Child A attended the child care home on 12/22/2021 by speaking with her on the telephone as he was concerned about what Child A was telling him about not being supervised. Child A has a younger brother that attends. Ms. Linington told him that Child A was never enrolled but was only "visiting." Child A's Father was also trying to obtain records of Child A's attendance. Child A's Father was unable to obtain information from Child A's Mother about the child care home. On 12/27/2021, I made an unannounced onsite inspection. I interviewed Ms. Linington. Ms. Linington stated the child care home was closed at this time due to illness in the home. I went over the allegation with Ms. Linington. Child A was not enrolled for care in her child care home. She thinks the last time she saw Child A would have been May or June of 2021. Ms. Linington did speak with Child A's Father by telephone and informed him she did not have any paperwork for Child A as she was not enrolled. Child A's Father did not like the answer and was adamant she turns over paperwork she did not have. Children in her care have always been supervised she would never leave an older child to attend to younger children. She been in business for many years and prides herself on providing great care for children enrolled. She would never place a child in harms way by not providing appropriate supervision. Parents are still picking their children up by the door and are not allowed in the child care home. On 01/21/2022, I received a text message from Ms. Linington stating she was no longer providing any child care services to Child A's Mother. 3 On 01/21/2022, I attempted to contact Child A's Mother by telephone. Left voice message requesting contact. On 02/19/2022, I attempted to contact Child A's Mother by telephone, unsuccessful with attempted contact. On 02/19/2022, I interviewed Child B's Mother by telephone. Child B's Mother stated she did not have any concerns about the care Child B receives in the home. On 02/19/2022, I interviewed Child C's Mother by telephone. Child C's Mother stated she did not have any concerns about the care Child C receives at the home.

(1) Prior to child's initial attendance, a licensee shall obtain the following documents: (a)A completed child information cared on a form provided by the department or a comparable substitute approved by the department. (b) A child in care statement or receipt using a form provided by the department and signed by the parent certifying all of the following. (i) Receipt of a written disciplinary policy. (ii) Condition of the child's health. (iii) Acknowledgement that the parent has been offered either a copy of the licensing rules for a child care home or has been given the website for an electronic copy of these rules. (iv) Agreement as to who will provide food for the child. (v) Acknowledgement that firearms are on the premises, if applicable. (vi) If the child care home was built prior to 1978, then the licensee shall inform the parents of each child in care and all personnel of the potential presence of lead-based paint or lead dust hazards, unless the licensee maintains documentation from a lead testing professional that the home is lead safe. (c) Documentation that immunizations and boosters, as recommended by the state department of health and human services, are any of the following: (i) Have been completed. 5 (ii) Are in progress. (iii) Are not being administered due to religious, medical, or other reasons based on a waiver signed by the parent. (d) If a parent objects to emergency medical treatment on religious grounds, the parent shall provide a signed statement that he or she assumes responsibility for all emergency care
Child A attended the child care home for care for approximately two days per Ms. Linington. Ms. Linington did not complete required paperwork prior to initial attendance.

Child A did attend the child care home for approximately two days per Ms. Linington. Child A attendance was not recorded.
