R 400.8152(9) · R 400.8152(9) Medication; administrative procedures. A center shall maintain a record as to the time and the amount of medication given or applied, with the exception of medications described in subrule (8) of this rule, on a form provided by the department or a comparable substitute approved by the department. One form per medication is required. The signature of the child care staff member administering the medication must be included.
The center is not using medication forms provided by the department. The form they use is not a comparable substitute because it does not include all of the components of the form provided by the department. It does not include space to document time, amount and staff member who administered the medication. The center also currently lists more than one medication on the same form and logs administration on separate sheets not attached to the parent's signature. The separate sheets have only the child's name but not the type of medication, amount to be given or medication instructions.
