On 04/26/2022, I interviewed Complainant by telephone. Complainant stated Ms. Ouellette's backyard fence was taken down on or about 04/13/2022 by the licensee and not replaced or fixed. Complainant has not observed any child care children in the backyard by the fence. A complaint was made with the City of Warren ordinance department. On 04/27/2022, I made an unannounced onsite inspection at the home. I interviewed Ms. Jacqueline Ouellette. Ms. Ouellette stated a week ago because of weather the fence was damaged and appeared to be coming down. She decided to have the fence taken down but was waiting to see if the insurance would cover the cost. She was leaving the fence until the insurance company had a chance to come out an inspect to determine if the cost of replacement would be covered. The child care children have not been in the backyard around or near the downed fence. Child care child are not allowed in this area of the backyard. On 04/27/2022, I observed the backyard fence. There were four to five wooden panels down lying close to the ground. The backyard has to be accessed through a locked gate which needs a key to open. The area where the child care children play is separated by fencing and is enclosed. 3 Over the last couple of years, she had problems with her neighbors with; calling police officers over parking, a trailer in front of her home, this last weekend her next door neighbor was in her backyard looking through her back window, she yelled at her to get off her property. Also, the police was called to her home three times in one day a week prior. There just seems to be an increase in the harassment she is receiving. It is upsetting her that neighbors would want to jeopardize her business with unsubstantiated calls to her home. She been in contact with both the police and the City of Warren ordinance department in wanting to make sure there was not a problem while waiting for her fence to be fixed. A call was made to the ordinance department, she did not get the name of the person she spoke with but thought the lady she spoke with was the head of the department. There were no violations on her home at the time she called. I informed Ms. Ouellette to document names and phone numbers of people who are involved with this matter for her records and the possible need to provide that information to me during this investigation. On 04/27/2022, I contacted the City of Warren zoning/ordinance department and spoke with Ms. Krystal Pierson. Ms. Pierson stated an inspector was headed out to inspect the childcare group home today. She provided the name and number for the inspector and directed that I try and contact him later in the day or the following day for results. On 05/03/2022, I spoke with a Detective Cusack from the City of Warren Police Department. Detective Cusack stated he was investigating a matter between Ms. Ouellette and a neighbor. He asked about my procedures as it related to completing the investigation at the child group home and outcome. I provided Detective Cusack with an overview of a special investigation. Detective Cusack did not need any further information on his end of his investigation. On 05/03/2022 and 05/05/2022, I attempted to contact Mr. David Padelutt, zoning/ordinance to determine the outcome of his inspection at the child care group home. Mr. Padelutt did not return any of my phone calls. On 05/06/2022, I received a copy of zoning enforcement document stating that complaint was closed on 04/27/2022 as there were no violations visible from sidewalk. On 06/09/2022, I completed the Exit Conference with Ms. Ouellette. I informed Ms. Ouellette no violations were being cited in the investigation. She will receive a copy of the special investigation report once approved. Ms. Ouellette stated a new fence was installed and that the issue with her neighbor will be addressed in court. She will send pictures of the fixed fence. On 06/13/2022, I received pictures from Ms. Ouellette of the brand new
