Teacher 1 and Teacher 2 did not observe any major marks, bruise, cuts or opens wounds on Child A. However, Teacher 1 and Teacher 2 did report seeing a minor surface level scratch on Child A’s face near her nose. Child A was offered first aid and/or ice pack several times by both teachers but Child A declined. Parent was notified of Child A's incident at the time of pick up.

On 07/26/2022 I completed an unannounced visit to the child care center. I met with interim Program Director Cindy Jones. Ms. Jones explained she has been working at the center as the interim Program Director for three weeks. She denied yelling at children or yelling at anyone in the building. She is not sure why someone would say she or anyone else yells at children in the building. She does not know of any staff that yell at children. She has not seen or heard any staff members yelling at students throughout the day. Ms. Jones said when parents are ready to pick their children up, they come to the desk in the hallway. She is the person who then radios the classrooms over a walkie talkie and the children are then sent down to leave for the day. This is the process they have used for the entire summer. She said she is familiar with parents and their children so she will send the children down. She said she has not sent a child down to a parent that she does not know. If there is someone that comes to pick a child up that she does not know, she will ask for their driver's license or identification card. Ms. Jones said children do not run around the child care center without supervision. She has not seen children wandering the hallways. I was able to walk around the building and view classrooms. There were three classrooms operating the day I was at the child care center. Rooms were organized. I did not hear any staff yelling. There were not children wandering the hallways. Ms. Jones showed me the desk and the area where parents drop off and pick up their children. There is a desk in the hallway where the entrance to the child care center is. Parents come to the desk, sign their children out, and wait for their child to come down to be picked up. Parents are able to view their child walking down the hall to the exit. On 08/24/2022 I completed an unannounced visit to the child care center. I met with Principal Nathan Vanduinen. Mr. Vanduinen said he is not a part of the child care center, but he is the principal of the school. He is aware of Cindy Jones and his school is connected to the child care center. He has not heard Ms. Jones yell, act rude, or be mean to any child care children. He has seen nothing but professionalism from Ms. Jones while she has been working at the child care center. Staff at the child care center do a great job watching the children and he has never noticed children wandering any hallways unattended. The child care center has a process for letting children out for the day. They have parents enter the building in the hallway and they do not go to the classrooms. Children are radioed by walkie- talkies and sent down to their parents. He has not heard of any issues or concerns regarding the child care center. He has not heard that any staff are yelling. I interviewed staff member Sarah Nikodemski. She has not heard Ms. Jones yell at anyone. She said Ms. Jones has a very clam demeanor. She said she does not yell at children, and she has not heard any other staff members yell at children. Ms. Nikodemski said children do not roam the hallways unsupervised. Children are always with a staff member when walking in the hallway. When children are checked 3 out for the day, parents come to the desk in the front hallway. Ms. Jones will radio the classroom on her walkie-talkie and the child will come down the hallway and walk to their parent. She said Ms. Jones and staff know who parents are. If there is a parent they are not familiar with, they will ask for identification or a drivers license. I interviewed staff member Delaney Becker-Baratta. She said she has never seen Ms. Jones yell at anyone. She does not yell at children. She has not seen or heard any staff members yell at children. She has not seen children wandering the hallways unsupervised. She said children are always with their staff members. Ms. Becker-Baratta said when a child is ready to leave for the day, the parent enters the building through the ent
