On March 14, 2022 the bureau received an incident report stating that Child A was found in a classroom restroom unattended at the end of the day. This occurred once the rest of the classroom transitioned to a neighboring classroom. It was estimated that Child A was alone in the classroom for approximately 5 minutes. On March 15, 2022, I completed an unannounced onsite inspection at this facility and spoke to program director Amanda Lovell and child care staff members (CCSM) Kimberly Perry, Jacqueline Kuyt, and Agatha Carter. I spoke to Jacqueline Kuyt who works in the Faris classroom typically but was working in the Pedal classroom on March 11, 2022. She identified that on March 11, 2022, she was working the whole day in the Pedal classroom. At the end of the day when the number of children were depleting, she took one child from the Pedal classroom and transitioned with that 2 one child to the Lyric classroom. This left Ms. Agatha Carter in the Pedal classroom with four children. They planned to transition to the Lyric classroom shortly after she did and when the numbers allowed for them to combine groups. Ms. Kuyt identified that she signed herself and the one child into the Lyric classroom but she forgot to sign herself and that child out of the Pedal classroom when she transitioned. She was unsure of the exact time Ms. Carter came to the Lyric classroom but observed her to transition with three children. She guessed that this occurred around 4:30pm but could not be sure. Ms. Kuyt stated that Ms. Carter began signing the three children she transitioned with into the Lyric classroom, and this is when she asked where Child A was. Ms. Kuyt stated that it was assumed that Child A got picked up for the day and that is why he was not in the Lyric classroom. Ms. Kuyt stated that she and CCSM's Ms. Carter, Kimberly Perry, and Kelsey Geary were all present in the Lyric classroom. She identified that Ms. Perry was changing diapers in the classroom restroom for another 3-5 minutes after Ms. Carter transitioned before heading over to the Pedal classroom to clean and take out trash for the day. When Ms. Perry went into the restroom located in the Pedal classroom she found Child A unattended. Ms. Kuyt stated that the Pedal and Lyric classrooms are next door to each other and share a half door between them. She estimated that Child A was left unattended for approximately 5-10 minutes. I spoke to CCSM Kimberly Perry who is the assistant in the Lyric classroom. She stated on March 11, 2022 once the Pedal classroom transitioned to the Lyric classroom she went to the Pedal classroom to close the room. She identified that closing a classroom entails cleaning and taking out the trash. Ms. Perry stated that when she went into the Pedal classroom she immediately went to the restroom to grab the trash. She identified that the restroom door was closed and when she opened the door she observed Child A alone. Ms. Perry was unsure of the exact time she found Child A but stated she clocked out for the day at 4:42pm which was a while after she found Child A. She guessed that she found him around 4:30pm. She could not provide an estimate of time Child A was left unattended because she was unsure when Ms. Carter transitioned with the rest of the Pedal children. I spoke to CCSM Agatha Carter who works in the Pedal and Lyric classrooms. She stated she was working in the Pedal classroom on March 11, 2022. She identified that towards the end of day the classroom was a little "crazy". Ms. Carter stated children were moving around, transitioning, and getting picked up by parents. Ms. Carter was unsure of the time but guessed it was around 4:45pm when she had four children left in the Pedal classroom. Ms. Kuyt transitioned with one child to the Lyric classroom before she eventually transitioned with the four children she had left in the Pedal classroom.
