On 12/04/2020 I spoke with Child A's Mother regarding the allegations. She stated AppleTree has an application called Nurture that allows staff to send information live regarding the care of any child. This includes incidents, updates, and diaper changes. There were numerous days where diaper changes were not updated on a regular basis. There would be changes in the morning but 2 nothing in the afternoon. Child A was also sent home in the wrong diaper and with a diaper on backwards on 11/23/2020 and 11/25/2020. Staff were spoken to regarding these issues. On 11/30/2020 Child A was sent to the center with no diaper rash. When Child A was picked up, he had diaper rash that was severe and up his leg. There were no updates in the Nurture Application so Child A could have gone the entire day without a diaper change. On 12/04/2020 I completed an unannounced visit to AppleTree Learning Center. I met with Assistant Director Savannah Tolar. Ms. Tolar stated she spoke with Child A's Mother regarding the diapering issues and making sure staff were on top of updating the program. Ms. Tolar stated they hired a new employee, and they were having some issues with them remembering to add information into the Nurture system. This employee was also changing the diapers of Child A and had put diapers on backwards. Ms. Tolar stated they have spoken to this employee and they will be moving them to a new classroom. Ms. Tolar stated they will be having a staff meeting and will also be reminding staff the importance of correctly entering information into their Nurture application. I interviewed staff member Leaha VanPutten. Ms. VanPutten stated she is the Lead Caregiver in Addabelle room. Ms. VanPutten stated they have just hired a new employee to help out in the afternoon in the room. She stated this employee was having some issues remembering to enter diaper changes into the center's Nurture application system. Ms. VanPutten stated this employee was also new to changing diapers and did put a few diapers on backwards. She stated she is in the classroom during the days and this employee would help in the afternoons when she was gone. It was during this time that Child A would have some issues with diaper changing. Ms. VanPutten stated this employee is getting help and more training and sometimes it takes a little bit to learn. She stated they are all aware and are working to make sure things get better in the classroom. She stated she was not aware of any diaper rash, but it could very well be due to Child A not being changed on a regular basis. On 12/09/2020 I spoke with staff member Connor Sanford. Mr. Sanford stated he was a new employee at AppleTree and was working in the Addabelle room. He stated he did change Child A's diaper in the afternoon. He stated he tried to remember to enter all the diaper changes in the center's Nurture system, but admits he missed some data entry. Mr. Sanford stated he was spoken to by management and he is making sure he pays closer attention to changing diapers regularly and on a consistent basis. Mr. Sanford stated he could have put diapers on backwards but was not positive. He also stated he was not sure about seeing a diaper rash, but if there were a rash, he would put diaper cream on it. Mr. Sanford stated he is working with staff to continue his learning and training on a regular basis. 3
The center does not change diapers correctly. During the month of November, Child A's Mother was not notified of diaper changes on a consistent basis. Child A (Male, 2-year-old) suffered a severe diaper rash due to his diaper not being changed appropriately. On 11/23/2020 and 11/25/2020, Child A was sent home in the wrong diaper and with his diaper on backwards. On 11/23/2020, child care staff member Connor Sanford yelled in the classroom scaring Child A (Male, 2-year-old).

On 12/4/2020 Child A's Mother stated she dropped off Child A and staff member Connor Sanford was yelling in the classroom at another child. This caused Child A to be frightened. She spoke with staff immediately regarding what she saw. On 12/04/2020 I spoke with Assistant Director Savannah Tolar regarding Connor Sanford yelling in the classroom. Ms. Tolar stated she was made aware of the incident from Child A's Mother. She stated she spoke with Mr. Sanford and he stated he was not meaning to yell or frighten anyone; he was just speaking louder because the children were not listening. She has spoken to Mr. Sanford and he will be using a lower tone in the classroom. Ms. Tolar will also be addressing yelling and loud voices in their staff meetings. 4 On 12/09/2020 1 spoke with staff member Connor Sanford. Mr. Sanford stated he does remember being a little loud in the classroom on 11/23/2020. He stated he was getting the children together for circle time or another activity and the children were not listening. As he was trying to continue asking, a child was trying to pull the fire alarm. Mr. Sanford stated he was frustrated and used his voice a little louder to get the attention of the children. He stated he did not mean to yell loud or frighten anyone. Mr. Sanford stated he knows that because he is male his voice will be louder to the children in care. He stated he will be working on using a controlled voice and not raise it.

Staff member Connor Sanford yelled at children in care trying to get them to listen. Mr. Sanford stated he raised his voice to prevent a child from pulling the fire alarm. He did not mean to frighten anyone but did state he was frustrated at the time.
