On May 16, 2022, a complaint was received alleging that Brianna Deacons, program director, pulled on a blanket that was wrapped around Child A's neck. Child A received a rug bum that was the size of a half dollar on his neck. It was unknown if the incident was an accident to out of anger. 3 On May 16, 2022, 1 interviewed the complaint source. The complaint source observed Child A on May 13, 2022, and noted a mark on Child A's neck. When the complaint source inquired about the mark to Child A's Mother, it was stated that Child A received the mark on his neck on May 11, 2022, while at school. Child A had a blanket around his neck and Brianna Deacons pulled on the blanket. The complaint source did not directly witness this incident. shape and was scabbing over. On May 18, 2022, 1 completed an unannounced onsite inspection I interviewed nna at the child care facility and has significant behavior issues. = a copy of this plan was provided by Ms. Deacons on May 18, 2022. The plan outlines positive activities for when Child A becomes aggressive and upset. Child A hits and kicks child care children and staff, he will throw all the materials off the classrooms shelves and climb on top of tables. On May 11 2022, Child A did not have any marks on his neck when he left the child care facility. During naptime, Child A kept pushing child care children while on the rug. Child A was asked to pick a spot on the rug to sit. Child A was kicking and hitting and threw a blanket at Ms. Deacons. Ms. Deacons took Child A's blanket and placed it on her desk. Ms. Deacons did not observe Child A have a blanket around his neck. Child A was asked to lay down, Child A cried, and Ms. Deacons sat by Child A to help him calm down. Child A was not physically forced to lay on his cot and fell asleep within five minutes. Child A did not receive his blanket from Ms. Deacon's desk before he fell asleep. Following naptime, the classroom went outside. Child A was hitting other children while outside and some of the other children will fight back. Child A did not have a mark on his neck when he left the facility on the bus. Child A does not take his blanket on the bus the blanket remains at the child care facility. If Ms. Deacons would have observed a mark on Child A's neck, she would have taken a picture and informed Child A's Mother. Child A's Mother contacted Yvonne O'Conner, Ubly Elementary Principal about her concerns. She did not contact Ms. Deacons. On May 18, 2022, I interviewed Kimberly Peplinski, child care staff member_ On May 11, 2202, Ms. Deacons took Child A's blanket folded it and placed n her deck. Child A threw his blanket at Ms. Deacons when he did not want to lay down at naptime. Ms. Deacons stayed with Child A in the classroom when Ms. Peplinski took the other child care children to the restroom prior to naptime. When Ms. Peplinski returned from taking child care children to the restroom Child A's blanket was still folded on top of Ms. Deacons' desk. Child A is aggressive and has Ms. Peplinski does not have any concerns with Re rNe and supervision provide by Ms. Deacons to child care children. Ms. Peplinski has not received any child care children or parent concerns regarding Ms. Deacons. On May 11, 2022, Ms. Peplinski did not observe any marks on Child A's neck. On May 18, 2022, 1 interviewed Samantha Sweeney, child care staff member On May 11, 2022, Child A threw a blanket at Ms. Deacons. He as thrown his blanket at child care staff many times prior to this incident. Child A was unable to remain at the carpet during group time because he was hitting other child care children. Ms. Deacons took Child A to his cot, and he was directed to lay down. Child A then kicked Ms. Deacons. Ms. Deacons sat on the floor with Child A and attempted to help calm him down by placing her hand on his stomach. Child A has a history of hitting, kicking, and scratching child care staff and child care children. Ms. Deacons was in the classroom along with Child A when Ms
