On 01/26/2021 I received an incident report, video, and phone call from Ms. Strange. Ms. Strange stated that Ms. Wall-Warren left Child A in the preschool classroom unsupervised for over 15 minutes. There were six children present. Ms. Strange was working in the infant classroom at the time. On 01/26/2021 I reviewed the video and I observed Child A alone in the preschool classroom, which is next to the pre-K classroom. She was sitting on the floor. There was not a time stamp on the video, so I could not determine the exact time. The two classrooms are in the fellowship hall of the church and separated with dividers that the caregivers cannot see over. In the twenty-minute video that did not have sound, Ms. Wall-Warren walked over to the preschool classroom twice to observe Child A leaving five children in the pre-K classroom. On 02/02/2021 Ms. Trombino stated that she has worked with Ms. Wall-Warren, but she was not at the center when Child A was left alone in the preschool classroom. She has never observed Ms. Wall-Warren leaving children alone in the preschool classroom. On 03/12/2021 Ms. Wall-Warren stated that she was a floater and working alone with the children and she did not know what to do when Child A left the classroom and would not return. She called the office and there was not anyone that could help her. She could not remember the name of the program director that was at the center when the incident occurred. On 03/12/2021 I called Ms. Bohler and her phone number was not correct. Ms. Bohler worked with Ms. Wall-Warren earlier in the day.
