On 12/06/2022, I spoke with the complainant. No additional information was provided. On 12/06/2022, I completed an unannounced onsite inspection. I spoke with child care staff member (CCSM), Georgette Antanios. Ms. Antanios identified herself as filling in for the Program Director position. Ms. Antanios identified CCSM's, Eylin Mansour and Juliet Esho as the fulltime staff members in the toddler classroom. She provided Luma Bashir and Gaida Aoss as floaters who assist in the classroom when needed. Ms. Mansour is the lead caregiver. Ms. Antanios denied any concerns regarding the temperament of any of the identified staff in the toddler classroom. She shared her room is next to the toddler classroom, and with the open space design of the building, all the staff members can hear and see what is going on in the other classrooms. She denied that any staff members or parents have brought concerns to her attention. I spoke with lead caregiver, Eylin Mansour. Ms. Mansour shared she has worked at the child care facility since 2015. I asked Ms. Mansour about the temperament of herself and other staff members. She shared "we are nice to them. They are just small kids and trying to learn." She denied having any disciplinary action regarding yelling at children or her temperament. She also denied ever having a parent address concerns with her regarding the way she speaks with the children. I spoke with CCSM, Juliet Esho. Ms. Esho shared she has worked with Ms. Mansour as an assistant staff member for approximately 1 year. Ms. Esho denied yelling at the children and said, "We don't yell at the children. We have to be patient with them because they're young. They have to learn." She denied hearing other CCSM's yell at the children. She shared the room is open and they can all hear one another, and no one yells at the children. I spoke with CCSM, Luma Bashir. Ms. Bashir shared she's a floater and works in the toddler classroom. She denied having any concerns regarding CCSM's yelling at the children and specifically stated, "I don't yell at them because I know they're just kids." I spoke with Sultana Samana. Ms. Samana is a CCSM in the preschool classroom which is adjacent to the toddler classroom. She shared she's been employed at the child care facility for 7 years. She denied having any concerns regarding the temperament or yelling in the toddler classroom. She shared she's able to hear everything that takes place and able to observe due to the half walls. 3 On 12/13/2022, I spoke with Child A mother. She shared her child has only been enrolled for a month. She denied having any concerns regarding the temperament of any of the staff and denied hearing any staff yelling at the children during drop offs or pickups. She said her time in the actual facility is very limited as she only (quickly) drops off and picks up her child. On 12/13/2022, I spoke with Child B mother. She denied having any concerns regarding the staff yelling. She shared Child B does talk but is still too young to tell her if someone is yelling at him. On 12/13/2022, I spoke with Child C mother. She denied having any concerns regarding the care Child C is receiving. She denied any concerns for the temperament of the staff and said she does not "really see how they treat them because I don't stay."

On 12/06/2022, I spoke with lead caregiver of the toddler classroom, Ms. Mansour. I asked Ms. Mansour about water intake for the children. She shared there's always water available for the children and pointed out the pitcher of water on the counter in the classroom. She shared some of the children bring their own water bottles and have access to them whenever they want them. They aren't allowed to walk around with the water bottles but can take a drink and place it back in their cubby area. She denied knowledge of any child being refused to have water. She denied that any parent or CCSM has ever brought concerns regarding this to her attention. During my in-class observation, I noted a pitcher filled with water and plastic cups on the counter. I also observed multiple water bottles which appeared to be brought in by the children. I observed one child go to their cubby area, take a sip of their water 4 and return to the group. This behavior appeared to be freely done and without guidance from the CCSM's. I also watched a CCSM provide a child with a cup of water upon request. This observation was completed prior to the CCSM's having knowledge of the complaint. I spoke with assistant staff member for the toddler classroom, Juliet Esho. I asked Ms. Esho about water being served in the classroom. She shared most of the children bring their own water bottles and they are easily accessible. In addition, they keep a water pitcher and small cups so children can have water whenever needed as well as with their meals. She denied any knowledge of any children going without water or any staff refusing a child water. I spoke with CCSM, Luma Bashir who works as a floater for the toddler classroom. I asked about water intake in the classroom. She shared she's observed children drinking from their water bottles and she has observed them being served from the water in the classroom. She denied knowledge of any children being denied water and shared no concerns. On 12/13/2022, I spoke with Child A's mother. She denied having any concerns or knowledge of concerns regarding the water intake for Child A. On 12/13/2022, I spoke with Child B's mother. She denied having any concerns surrounding water intake for Child B. On 12/13/2022, I spoke with Child C's mother. She denied any concerns.

On 12/06/2022, I spoke with Georgette Antanios. Ms. Antanios said written daily records are only completed for the infant classroom. When asked if there was a reason for this, she shared she did not know why but "that's just the way we've been doing it for a while." I spoke with Eylin Mansour. Ms. Mansour denied completing and providing parents with a daily record regarding their children during pickup. She shared they have conversations about the child with the parent during pick up. I spoke with Child A's parent. She confirmed the facility does not provide her with a daily record with information pertaining to Child A. I spoke with Child B's parent. She shared since Child B moved to the toddler classroom, she has not received the daily record as she had when Child B was in the infant classroom. I spoke with Child C's parent. She denied receiving a written daily record for Child C. 7

I emailed the complainant on 06/28/2022 and 07/01/2022. The emails were returned undeliverable. There was no additional contact information available for the complainant. On 06/30/2022, I interviewed Ms. Walker at the center. She said child care staff members remain in the classroom that they are assigned to and do not move from room to room. When staff call in sick or are unable to work their scheduled shift, a staff, who is a floater, will cover the classroom of the absent staff. The center does not mix different age groups in the classrooms. School-age children attend the center for school breaks and summer camp. School-age children are kept in separate rooms from the younger children. On 06/30/2022, I interviewed Ms. Antanios at the center. Ms. Antanios is a child care staff member in the GSRP 1 room. She does not work in other classrooms. Child care staff members do not move from room to room. She is not asked to work in other classrooms when staff call in for their shift. No other children except for the GSRP children are in her classroom. The center does not mix different age groups. On 06/30/2022, I interviewed Ms. Alqahwagi at the center. Ms. Alqahwagi is a lead caregiver in the GSRP 2 room. She does not work in other classrooms. Child care staff members do not move from room to room. She is not asked to work in other classrooms when staff call in for their shift. No other children except for the GSRP children are in her classroom. The center does not mix different age groups. On 06/30/2022, I interviewed Ms. Mansour at the center. Ms. Mansour is a child care staff member in the early preschool room. She does not work in other classrooms. Child care staff members do not move from room to room. She is not asked to work in other classrooms when staff call in for their shift. Only the 2 to 3-year-old 1/2 children are in her classroom. The center does not mix different age groups. On 06/30/2022, I interviewed Ms. Aoss at the center. Ms. Aoss is a child care staff member in the preschool room. She does not work in other classrooms. Child care staff members do not move from room to room. She is not asked to work in other classrooms when staff call in for their shift. Only 3- to 4-year-old children are in her classroom. The center does not mix different age groups. On 06/30/2022, I interviewed Ms. Shaba at the center. Ms. Shaba is a lead caregiver in the infant and toddler room. She does not work in other classrooms. Child care staff members do not move from room to room. She is not asked to work in other 3 classrooms when staff call in for their shift. Only infants to 2 year old children are 1/2 in her classroom. The center does not mix different age groups. On 06/30/2022, I interviewed Ms. Samano at the center. Ms. Samano is a child care staff member who works with the school-age children. She works with school-age children during school breaks and during summer camps. She works with the other children when there are no school-age children at the center. Child care staff members do not move from room to room. Summer camp began last week and since it has begun, she has not been asked to work in other classrooms when staff call in for their shift. Only 5- to 9-year-old children are in her classroom. The center does not mix different age groups. On 06/30/2022, I completed a walk through of the child care center. I did not observe any classrooms with mixed age groups. I reviewed the child care staff members' work schedule beginning June 6, 2022. Each child care staff member was assigned to work in a specific classroom.

Ms. Walker said staff have not expressed any frustrations to her about any of the children having challenging behaviors. She has not observed any staff yelling or appearing to be frustrated with any of the children. Ms. Antanios, Ms. Alqahwagi, Ms. Mansour, Ms. Aoss, Ms. Shaba, and Ms. Samano said staff have not expressed any frustrations to them about any of the children having challenging behaviors. They do not get frustrated with any of the children. They do not yell at any of the children. They have not observed any staff yelling or appearing to be frustrated with any of the children.

On 02/17/2022 Child A's Mother stated that on 01/27/2022 Child A informed her that Ms. Walker pinched his ear and had him sit in timeout during nap time. She was not sure how long Child A had to sit in timeout. Child A does not sleep at the center because he is dropped off after 10:00 am. and he is not sleepy when the other children are going down for nap. There were no marks on the child's ear. On 01/28/2022 Child A's Mother asked Ms. Antanios, based on what Child A told her, if she could review the video from the previous day. Ms. Antanios stated that Ms. Alhoussaini would have to access the video for her, and she would inform Ms. Alhoussaini that she wanted to review the video. Child A's Mother was scheduled to 2 review the video on 02/03/2022, and when she arrived the center was closed. She called the center several times to reschedule with Ms. Alhoussaini to review the video, and she was not contacted until 02/14/2022. Ms. Alhoussaini stated that she did not have the video because the recording system deletes the videos after seven days. Ms. Alhoussaini told her that she reviewed the video, and she did not observe Ms. Walker pinch Child A on the ear. Child A's Mother is convinced that Ms. Alhoussaini did not show her the video because Ms. Walker pinched Child A's ear. She disenrolled Child A from the center on 02/14/2022. Child A's Mother believes that Ms. Alhoussaini was hiding the fact that Ms. Walker pinched Child A's ear, which was why she would not show her the video. Child A's Mother heard Ms. Walker yelling at the children when she would pick-up Child A. She could not remember the exact dates that she heard Ms. Walker yelling and she did not inform the staff. On 02/23/2022 Ms. Alhoussaini stated that she was not at the center when the alleged incident occurred. She was informed by Ms. Antanios that Child A's Mother had requested to review the video from 01/27/2022 because Child A informed her that Ms. Walker had pinched his ear during nap time. On 02/02/2022 Ms. Alhoussaini reviewed the video, and she did not observe Ms. Walker pinch Child A's ear. She scheduled for Child A's Mother to review the video on 02/03/2022. The center was closed on 02/03/2022 for a snow day. She contacted Child A's Mother on 2/11/2022 and informed her that the video was not stored in the system any longer so she would not be able to review the video. Ms. Alhoussaini offered to have Child A's Mother observe in the classroom so that she could witness firsthand the type of care that Child A receives at the center. She said Ms. Walker does not yell at the children, and she has never observed her being inappropriate with the children. Ms. Mansour, the toddler child care staff member, stated that Child A is in her classroom, and he does not nap. On 01/27/2022 Child A was waking up the children that were going down for nap. She asked Ms. Walker if Child A could sit with her in her classroom until she was able to get the children to sleep because her children were asleep. Child A was with Ms. Walker sitting at the table between twenty and thirty minutes and he then returned to the classroom. She could see him sitting and talking with Ms. Walker at the table. When he returned to the classroom, she had him play with toys at the table while the other children were sleeping. He did not say anything to her when he returned, and he was not upset when he returned. The only staff members that had contact with Child A on 01/27/2022 were herself and Ms. Walker. She did not observe Ms. Walker pinch Child A's ear and he was not in timeout when he was sitting with Ms. Walker. Ms. Walker does not yell at the children, and she has never observed her being inappropriate with the children. On 03/16/2022 Ms. Antanios stated that she does not work with Child A, and she was not working on 01/27/2022. She works with Ms. Walker in the GSRP 1 classroom. On 01/28/2022 Child A's Mother informed her that she wanted to review the video from 01/27/2022 because Child A infor

Child A's Mother stated that Child A was enrolled in distance learning at Dreamy Children's Center in September 2020. She disenrolled in October 2020, because distance learning was challenging for Child A. In May 2021, Child A's Mother felt comfortable sending Child A back to Dreamy Children's Center for in-person learning. Child A was reenrolled in May 2021 and attended her first day of in-person learning on 5/10/2021. This is the only day that Child A had attended in-person learning at Dreamy Children's Center. Child A's mother stated Child A was scratched, had her hair pulled, and was spit on by Child B. She said staff members observed this behavior and did not try and stop Child B from hurting Child A. She is concerned that Child B is out of control and bullies' other children in the class. Staff do not know how to handle Child B's behavior, so they ignore it and just provide comfort to children who are injured. Child A `s Mother stated that none of the staff members told her about the bullying incident and that Child A is traumatized by the incident. She is having toileting accidents and has been referred to a child psychologist from the doctor. Child A's mother said Child A's father picked up Child A from care on 05/10/2021 and took her home. At home, Child A's mother noticed that Child A's right arm was scratched and bruised, and she also had a scratch on her face and a cut lip. She asked Child A what happened at school and Child A told her that Child B choked her and spit on her. She called the center and spoke to Child Care Staff Member, Eylin Mansour. Ms. Mansour told her she did not observe Child B do these things to Child A. She asked to speak to Program Director, Kristi Walker, but she had already left for the day. On 09/28/2021 I spoke to Child A's Mother. She said she was concerned about the physical and mental effects of Child B bullying Child A, so she took Child A to her doctor. She could not recall what day she took Child A, but said it was either 05/10/2021 or the following day 05/11/2021. She stated Child A was treated for a scratch, bruising, and mental trauma. The complaint stated if she finds any documentation from the visit, she will send it to me over email. She stated she did not have any photographs of the injuries to her child but if she found them, she would send them to me. As of 10/04/2021, I have not received additional information. On 08/16/2021 I conducted an onsite inspection at Dreamy Childcare Center in Troy. I interviewed Program Director Kristi Walker and Child Care Staff Members, Gayda Aoss and Eylin Mansour. Ms. Walker said Child A started off in a distance preschool class in September 2020 but disenrolled the following month because Child A did not like the program. Child 3 wanted to return to school when in-person options were available. In May 2021, Ms. Walker informed Child A that in-person options were available. Child A was enrolled in the preschool classroom and attended in-person learning at the center on 05/10/2021. Ms. Mansour worked in the preschool room on 05/10/2021. Child A's father picked up Child A that evening and dropped off a check. He signed Child A out and left for the night. Ms. Walker said that Child B has never shown aggression towards other children, and staff have not had any concerns with her behavior. Ms. Walker stated she observed Child A on 05/10/2021 during the day but did not see any marks, cuts, or bruising on her body. Ms. Walker said Child A's Mother requested a meeting with Child B's mother and father, in which the center agreed; however, the complainant declined the meeting and disenrolled Child A from care instead. Ms. Walker said she is not able to obtain camera footage of 05/10/2021, due to the length of time that has passed since the incident. I spoke to Ms. Aoss. She works as an Infant/Toddler Staff Member. She stated she provided care to Child B on a regular basis over the last year. She said she has not had any behavioral concerns with Child
