At the time of the incident on 11/18/2025, Witness had unsupervised contact with children and had not been fingerprinted and determined eligible by the 2 LANDMARK BUILDING • 105 W. ALLEGAN STREET • LANSING, MICHIGAN 48933 Michigan.gov/MiLEAP department. Witness had only completed a Michigan Public Sex Offender Clearance (PSOR). As of 12/03/2025, Witness has been fingerprinted and determined eligible by the department.

The child care center has a written plan for parent notification if a child has an accident/injury/incident at the center. The plan was not implemented how it states within the plan for the incident that occurred on 11/18/2025. Child A's Mother was only notified when she arrived to pick up Child A from the child care center.
